    Notes: In serial samples of amniotic fluid from 24 patients in induced labour, the rate of change in the lecithin/sphingomyelin (L/S) ratio correlated significantly with the duration of labour. Cord serum 11-hydroxycorticosteroid levels were lowest in the group of patients in which the L/S rose during labour, and highest in the group in which the L/S ratio fell. Cord serum 11-hydroxycorticosteroid levels correlated positively with the duration of labour. It would seem unlikely that the changes in L/S ratio occurring during labour are mediated endogenous corticosteroids.

The lecithin/sphingomyelin (L/S) ratio was estimated and found to rise in 52 per cent, remain almost unchanged in 33 per cent and fall in 15 per cent. The rate of rise in the L/S ratio seemed inversely related to the length of labour.

    Notes: The lecithin/sphingomyelin (L/S) ratios and optical densities at 650 nm were determined for 158 samples of amniotic fluid obtained by amniocentesis. A further 24 samples collected by vaginal aspiration from patients with spontaneous rupture of the membranes were also analysed. The relation between the L/S ratio and the optical density suggests that the more rapidly obtainable optical density measurement could be used as a screening procedure for fetal pulmonary maturity. A valid estimate of maturity by optical density is not possible in amniotic fluid aspirated from the vagina.

    Notes: Objective  To examine the outcomes of statutory health assessment of children looked after by an English City Council.Design  Retrospective longitudinal documentary analysis.Setting and participants  Health records of all children looked after by Southampton City Council who had attended at least two statutory health assessments within a designated paediatric service from 1996 to 1999.Main variables studied   Demographic characteristics of the children; physical and mental health problems identified at assessment and extent to which health recommendations were implemented.Results  Twenty-seven boys and 22 girls aged 6 months−15 years were identified who had attended at least two assessments. One-hundred and four physical and mental health needs were identified at care entry requiring further assessment or intervention. More health problems were identified for girls than boys. At care entry 15/49 of the children were not fully immunized. At review, on average 14 months later, recommendations had only been implemented in just over half of children.Conclusions  In common with previous studies this work confirms that the statutory health assessment identifies health need and health neglect that may otherwise go unrecognized. Whereas children's needs and problems were diverse, many continued to suffer health neglect in the system of public care designed to help them. In order to be effective, statutory health assessments must be a health promoting rather than disease screening exercise delivered by professionals skilled to address diverse health needs. Crucially, the heath assessment can only succeed as a tool for health advocacy if complementary to and integrated with local authority care and review.

    Notes: Abstract This paper examines the physical consequences of increased catchment sediment yields on the sediment budget and the hydrodynamic setting of the South Johnstone River estuary in North Queensland. A combined study involving hydrological monitoring, assessment of sediment sources, estimation of riverine sediment budget and hydro-sedimentological numerical modelling for estuarine sediment transport is currently underway. Initial field and laboratory observations indicate that the sediment delivery from highly erosion-prone sugar cane cultivations in the tropical catchment has increased dramatically during the last 10 years. This has subsequently given rise to elevated flood levels in both the lower and upper catchment areas, as well as significant modifications to the river bed morphology.
