    Notes: Summary The influence of Laser radiation on neuroma formation was investigated in rat sciatic nerves. Peripheral nerve trunks were divided by focussed Neodym Yag Laser and compared with those transected by microscissors. The nerves were re-exposed at different time intervals up to 240 days after initial operation. True neuroma formation could not be observed after laser transection. Sharp division resulted in widespread amputation neuromas consisting of regenerating axons and connective tissue. Neodym Yag laser radiation by its scattering effect may cause deep coagulation necrosis and thrombosis of vasa nervorum thus sealing the dome of the divided nerve trunk. The formation of amputation neuromas may be suppressed by laser application.

    Description / Table of Contents: Zusammenfassung Anhand der Erfahrungen von 179 frahen Relaparotomien unter 2550 Eingriffen in der Bauchhbhle der Chirurgischen Univ.-Klinik Bonn von 1967–1970 werden die Indikationen zum Zweiteingriff vorgetragen. Bei der mit 5,2% relativ hohen Frequenz ist zu berücksichtigen, daß mehr als ein Viertel der Patienten auswärts voroperiert und dann zugewiesen waren, daß dazu am Magen bei jedem 4., an den Gallenwegen bei 40% frühere Eingriffe vorangegangen waren. Unter den Komplikationen dominieren Blutung, Ileus und Peritonitis mit kennzeichnenden Unterschieden in der Haufigkeit bezogen auf Magen, Leber, Gallenwegs-und Darmoperationen. Bei der stark abweichenden Symptomatik dieser Krankheitsbilder in der frühen postoperativen Phase ist im begriindeten Verdachtsfalle die Situation operativ abzuklären. Im Durchschnitt erfolgte die Relaparotomie 7 Tage nach der Erstoperation, wobei sich das Schwergewicht der Indikationen mit zunehmendem Zeitintervall von der Blutung zum Ileus und zur Peritonitis verschiebt. Reinterventionen nach dem 8. Tag gehen fast ausschließlich zu Lasten von Passagebehinderungen, Drainagen meist subphrenischer Abscesse und Bauchdeckendehiszenzen. Als Zugangsweg dient die ursprüngliche Incision; jedenfalls hat man fur adäquaten Zugang zu sorgen. Die hohe Gesamtletalität von 47% unterstreicht die ernste Prognose. Dabei steigt mit zunehmendem Alter die Letalität kontinuierlich und erreicht jenseits des 7. Dezenniums fiber 80%. Unter den Todesursachen steht die Peritonitis — überwiegend fiber eine Nahtinsuffizienz — mit 2/3 der Fälle an erster Stelle. Dahinter rangieren mit weitem Abstand organspezifische Ursachen, wie Blutung infolge Koagulopathie, Leberkoma und der nicht beherrschbare Ileus in etwa gleicher Haufigkeit. Unter den operativen Fehlern sind unbeabsichtigte iatrogene Läsionen bei schwierigem Situs oder infolge Unachtsamkeit und das Übersehen vorliegender Krankheitsveränderungen als vordringlich zu nennen. Die Erfahrungen geben Anlaß, fur eine freiziigigere Indikation zur frnhen Relaparotomie einzutreten. Vielfach ist jedoch ein Zweiteingriff etwa wegen Anurie oder Koagulopathie weder möglich noch ratsam. Hier ist die Entscheidung, die zweite Operation vorzunehmen oder zurückzustellen schwerwiegend, die Letalitat extrem. Bei Komplikationen nach einer Laparotomie müssen wir uns die Frage der Reintervention unmittelbar vorlegen, die Entscheidung bald fallen, bevor der richtige Zeitpunkt verpaßt wird und im Zweifelsfalle fur eine frühe Relaparotomie eintreten.
    Notes: Summary Based on experiences with 179 early re-laparotomies after 2550 abdominal operations performed at the Surgical University Clinic, Bonn, from 1967 to 1970, the indications for the second operation are discussed. With regard to the relatively high rate of 5.2% one has to take into account that over a quarter of the patients had their first operation elsewhere before admission here and that in the gastric cases earlier operations had preceded the last in 25%, in bile duct operations in 40%. Predominant complications are haemorrhage, ileus and peritonitis, with characteristic differences in frequency after stomach, liver, bile duct and intestinal operations. In view of the diverse symptomatology of the clinical pictures in the early postoperative phase, the situation must be clarified by operation in cases of reasonable doubt. On average, re-laparotomy took place 7 days after the first operation, the frequency of indication shifting from haemorrhage to ileus and then to peritonitis with increasing time intervals. Re-interventions after the 8th day are almost exclusively done for passage obstructions, drainage of abscesses, mostly subphrenic, and dehiscence of the abdominal wall. The original incision serves as access. In any case, adequate access must be secured. The high total mortality of 47% underlines the seriousness of prognosis. It rises steadily with increasing age and reaches over 80% beyond the 7th decennium. Among the causes of death peritonitis—mainly due to suture insufficiency—occupies first place with 2/3 of cases. This is followed after a long interval by organ-specific causes, such as haemorrhage due to coagulopathy, hepatic coma and uncontrollable ileus at about the same frequency. Among operative mistakes unintentional iatrogenic injuries in difficult situations or due to carelessness must be listed first, also failure to notice the presence of pathological changes. Our experience gives cause to advocate more liberal indications for early relaparotomy. However, a second operation is often neither possible nor advisable because of anuria or coagulopathy. In these cases the decision to perform or delay the second operation is grave and the mortality extremely high. In complications after laparotomy the question of re-intervention must be considered immediately and the decision made promptly, before the right moment is missed. In doubtful cases early re-laparotomy should be performed.

    Notes: Abstract Using the 93.31 keV Mössbauer transition in67Zn we have investigated hyperfine interactions and lattice-dynamic effects of nanostructured hexagonal (wurtzite) ZnO. The nanocrystals with particle sizes between 3 and 30 ran were produced by a reactive-gas sublimation method and were subsequently compacted into solid bodies under external pressure. The nanocrystalline materials were used as sources in Mössbauer emission experiments. A drastic increase of the asymmetry parameter was found fromη=0 (single crystal) toη≈0.5 (nanostructured material). Our theoretical Hartree-Fock cluster calculations show that displacements of the Zn atoms off the hexagonal symmetry axis as small as 0.005 Å can lead to such enhancedη parameters. The Lamb-Mössbauer factorf drastically drops when the particle size is reduced below ∼ 10 nm. Two lattice-dynamic models are discussed which successfully describe this behavior.

    Notes: Summary After removal of the mucosa in the distal colon stump over a length of 1 inch, the anastomosis is performed in an end-to-end fashion and one-layer sutures are used between the proximal colon and the distal rim of the mucosa. Thus the proximal colon and the anastomosis are covered by the outer layer of the sero-muscularis. The combination of this technique with astronautic food reduces the frequency of suture insufficiency of large bowel anastomoses.
