Summary We report on two patients with myelomeningocele and hydrocephalus in whom hypothalamo-pituitary growth hormone deficiency and neurosecretory dysfunction were diagnosed. The patients were treated with human growth hormone (0.5–0.7 IU/ kg/week) since the age of 4.2 and 8.7 years. Short stature, retarded growth velocity, retarded bone age, reduced arm span and low IGF1 and IGFBP-3 serum levels were found. During growth hormone therapy (duration: 24–33 months) patients improved in growth velocity, supine length and arm span. In one girl with thoraco-lumbar scoliosis a rapid progression of the deformity developed. Tethered cord and syringomyelia were present. Discussion: Because of several growth limiting factors (skeletal deformities, tethered cord), growth response to therapy shoud be judged carefully and interdisciplinarily. Further prospective studies are necessary to assess the long-term benefits of growth hormone treatment on the physical and psychosocial state of myelomeningocele patients.

Zusammenfassung Wir berichten über 2 Patientinnen mit Meningomyelozele und Hydrocephalus internus, die ab dem Alter von 4,2 bzw. 8,7 Jahren nach biochemischem Nachweis eines Wachstumshormonmangels (bzw. einer neurosekretorischen Dysfunktion) mit rekombinantem Wachstumshormon (0,5–0,7 IE/kg und Woche) behandelt wurden. Hinweise auf einen Wachstumshormonmangel gaben neben Kleinwuchs, Knochenalterretardierung und verminderter Wachstumsgeschwindigkeit pathologische Serumspiegel von IGF1 und IGFBP-3 sowie eine verminderte Armspannweite. Im Verlauf der Wachstumshormontherapie (Therapiedauer 24–33 Monate) konnte eine Verbesserung der Wachstumsgeschwindigkeit, Körperlänge sowie der Armspannweite erreicht werden. Eine Patientin mit vorbestehender Lähmungsskoliose entwickelte eine Progredienz der Wirbelsäulendeformität auf dem Boden einer Syringomyelie und eines symptomatischen Tethered-cord-Syndroms. Diskussion: Eine individuelle Auswahl der Patienten für eine Wachstumshormontherapie sowie eine interdisziplinäre Langzeitbetreuung vorausgesetzt, scheint Wachstumshormon nach den derzeitigen Erfahrungen Wachstum, Mobilität und soziale Integration der Patienten positiv zu beeinflussen. Prospektive Studien zur Beurteilung des Langzeittherapieerfolgs von Wachstumshormon bei Patienten mit Meningomyelozele sind erforderlich.
