    Notes: An integrated all-optical switch based on a high-Q nonlinear cylindrical microcavity resonator is proposed. The switch consists of single mode planar waveguides that allow coupling light in and out to a microresonator, exhibiting whispering gallery modes. Due to the high Q factor and the small dimensions, fast switching at low power is feasible for devices based on presently available nonlinear polymers as the active material. In this approach, the transmission of an integrated optical waveguide close to a microcavity has been measured and related to the resonances of the cylindrical microcavity. © 1997 American Institute of Physics.

    Notes: Abstract  Osteosarcoma (osteogenic sarcoma) metastasizes primarily to the lung. With the introduction of neoadjuvant chemotherapy as part of the treatment, the overall and disease-free survival rates have dramatically improved. In this case report, a young man with multiple soft tissue and bone metastases, including a rare large bone-forming retroperitoneal metastasis, is described. Despite the extensive extrapulmonary metastases, the patient did not develop pulmonary metastases in the 4 years following initial presentation of the primary tumour.

    Notes: Abstract Background:To establish the effect of chemotherapeutics on thegrowing skeleton, male Wistar rats were studied. Design:Between the ages of 4 and 13 weeks the rats were giveni.v. doxorubicin 15 mg/m2 body surface area (BSA), methotrexate 60mg/m2 BSA or cisplatin 7.5 mg/m2 BSA. For each group ofdrug-treated rats there was a diet-matched control group that was injectedwith a placebo only. Rats fed ad libitum served as the basic control group forlength and weight growth. Body weight and tibial length were measured weekly.Kidney and liver weight were determined at the end of the study. Results:Weight gain and length growth were significantlydecreased in the diet controlled groups (P 〈 0.05). Doxorubicinreduced length growth with 4.12 mm or 18 % (P 〈 0.05).Methotrexate reduced length growth with 1.11 mm or 5 % (P 〈0.05). Length growth in the cisplatin treated rats did not differ from thediet controls. Conclusions:Doxorubicin and methotrexate decrease length growthin the rat tibia by, respectively, 18% and 5%. Cisplatin doesnot affect length growth. The decrease in growth might be a direct effect ofdoxorubicin and methotrexate on the tibial growth plate and metaphysis, butmay be more pronounced due to the malnutrition.

    Notes: Abstract New 2D scalar beam propagation methods for both TE and TM polarization are presented. Both second- and fourth-order accurate schemes, in the lateral stepsize, are shown. The methods use uniform discretization and can handle arbitrary positions of interfaces between materials with different refractive indices. Either Transparent Boundary Conditions or Perfectly Matched Layers are used at the boundary of the computational window.

    Notes: Abstract Efficient interface conditions (EICs) are derived for the propagation equation using the slowly varying envelope approximation for the dominant electric field component. At the interface between two different media, the two lateral second derivatives in the discretized propagation equation are adapted such that the discretized modal field equation is correct up to second order in the lateral grid spacing. Since the error term is then of the order of the lateral grid spacing, our EICs are first-order EICs. These interface conditions are compared with well-known zero-order EICs derived by Stern and Kim and Ramaswamy. It is shown that the first-order EICs yield faster convergence to the exact effective index value as the lateral grid spacing is decreased than do the zero-order EICs. It turns out that our EICs are very much like those derived by Vassallo. Using essentially the same method, he derived EICs of second and first order for the field component respectively parallel and perpendicular, to the interface. Hence the accuracy of his EICs is one order higher for the field component parallel to the interface, although it introduces an extra asymmetry in the propagation matrix.

    Notes: Abstract A new 5-point finite difference operator is developed for integrated optics simulations. Interfaces are taken into account accurately. The eigenvalues of the discretization matrix converge rapidly, according to O(Δx4). Implementation of the operator in a beam propagator leads to a highly efficient computation scheme.

    Description / Table of Contents: Zusammenfassung Osteosarkome des Femur werden heute, wenn, möglich, mit Chemotherapie, Tumorresektion und Rekonstruktion mit Hilfe von Endoprothesen behandelt. Dabei besteht als Folge der Chemotherapie nicht nur das Risiko einer Epiphysenfraktur der langen Röhrenknochen, sondern auch einer Fraktur im Rest des behandelten Femur. Wegen des Risikos von Infektion und Heilungsstörung bei operativer Therapie sollten solche Frakturen konservativ behandelt werden.
    Notes: Summary Osteosarcomas of the femur are treated nowadays, if possible, by chemotherapy, resection of the tumor and reconstruction with an endoprosthesis. There is not only a risk of epiphyseal fractures in the long bones due to chemotherapy but also of a fracture in the remnant of the treated femur. These fractures should be treated conservatively because of the risk of infection and non-union if treated operatively.

    Notes: Summary Between 1978 and 1983, 14 patients suffering from rheumatoid arthritis were treated by bilateral total hip and knee replacement. At the time of this follow-up study (32–92 months postoperatively) one patient had died of concomitant disease and was excluded. The postoperative results regarding pain relief, improvement in performing daily activities, and function were studied in the remaining 13 patients. A 200-point rating scale was used, which compared the pre- and postoperative findings at follow-up. All patients showed marked relief from pain, good function and a good functional score. Although walking ability did not always improve, all patients were enthusiastic.
