Zusammenfassung Auf 4000 Menschen kommt einer mit einem atrio-ventrikulären Herzblock; das rind in Deutschland 15000 Menschen. Oder: auf 1 Million Einwohner kommen pro Jahr rund 50 neue Fälle hinzu; das sind in Deutschland Weise erreicht. Bei der Indikation bleibt weiter zu berücksichtigen, daß die konsevative Therapie eine schlechte Prognose hat. Aus diesem Grunde sollte die Indikation ausgeweiter werden. Bisher werden vorwiegend Patienten mit totalem a.-v. Block and AdamsStokesschen Anfällen operiert, gleichgültig, ob es sich um einen Block II. oder III. Grades handlet. Es sollten aber auch die Fälle mit totalem a.-v. Block oder Bradyarrhythmie oder Sinusbradykardie ohne Adams-Stokessche Anfälle operativ behandelt werden, um die häfigen Sekundärschäden (Niere, Herz, Gehirn) zu vermeiden. Bei operativem Herzblock oder Block nach Herzinfarkt ist die Indikation zur Operation dann gegeben, wenn mehrere Wochen nach dem Ereignis keine genügend hohe Eigenfrequenz zurückgekehrt ist. Der angeborene Herzblock bedarf aber nur dann der Implantation, wenn Adams-Stokessche Anfälle auftreten. Abschließend wird die Indikation zum Einsatz der verschiedenen Schrittmachertypen erörtert: festfrequente Reizung bei totalem a.-v. Block und niedriger Eigenfrequenz: der vorhofgesteuerte Schrittmacher bei jungen Leuten mit guter Vorhofaktion; der ventrikelgesteuerte Schrittmacher bei allen Fällen wechselnder Frequenz.

Summary One person in every 4.000 has an atrio-ventricular block, which means there are 15.000 such cases in Germany alone. 50 new cases occur per 1 million inhabitants each year, which means that for Germany 3,000 new pacemaker implantations are necessary. However, that number of operations is by no means reached. When considering the indication one has to bear in mind that with conservative treatment the prognosis is poor. For this reason, the range of indications should be widened. Hitherto mainly patients with total AV block and Adams-Stokes symptoms were operated on, irrespective of whether a second-degree or third-degree block was present. However, cases of total AV block, brady-arrhythmia or sinus brady-cardia without Adams Stokes symptoms should also be operated, to avoid secondary damage to the kidneys, heart and C. N. S. In heart-block following surgery or cardiac infarcation, an indication for surgery exists if several weeks after the event an adequately high spontaneous cardiac rate has not been restored. Congenital heart-block requires implantation of a pacemaker only if Adams Stokes symptoms occur. In conclusion, the indications for the use of various types of cardiac pacemaker are discussed: pacing at a steady rate in cases of total AV block and low spontaneous rate, atrially-controlled pacemakers in young people with good atrial function and ventricularly-controlled pacemakers in all cases with irregular cardiac rate.[/ab]
