    Notizen: Summary The Mini-Finland Health Survey was designed to analyse the epidemiology of major public health problems. The study covered a representative sample of the Finnish population aged 30 years or over, and initially comprised 8 000 people. Serum rheumatoid factor was determined in 7 124 cases (89%). There were 138 cases of clinical arthritis in the series, corresponding to a prevalence of 1.9% (1.0% in males and 2.7% in females). Fifty-nine of the cases (0.8% of the population) were seropositive, and 50 cases (0.7%) had in their hand radiographs changes characteristic of rheumatoid arthritis. Twelve of the X-ray positive cases were seronegative. One-half of these proved to be seroconversion cases or rheumatoid variants typically seronegative, such as early-onset juvenile rheumatoid arthritis.

    Notizen: Summary A community-based cohort comprising 150 patients with recent-onset seropositive arthritis or seronegative oligoarthritis or polyarthritis was monitored for 8 years. Of the different rheumatoid factor (RF) tests, the initial positivity in the most sensitive assays (latex slide test and IgM-RF by enzyme immunoassay) was the most reliable factor when distinguishing between the erosive and nonerosive cases. The individual RF isotypes fluctuated in four serial specimens, but each patient tended to preserve his or her reactivity pattern. Seroconversion from positive to negative within the 1st year of follow-up did not indicate a particularly good long-term prognosis.

    Notizen: Abstract Leukotriene B4 (LTB4) synthesis in rheumatoid synovitis was studied using peripheral and synovial fluid polymorphonuclear leukocytes (PMNs) and rheumatic synovial lining cells. No differences were found in LTB4 synthesis between peripheral PMNs from healthy volunteers and rheumatoid arthritis patients. When peripheral and synovial PMNs from the same RA patient were compared, arachidonic acidinduced LTB4 synthesis in synovial fluid PMNs was increased 1.7–7.2 fold, whereas the response to Ca ionophore A23187 stimulation was similar. This suggests 5-lipoxygenase stimulating factor(s) in inflamed joints. Rheumatic synovial lining cells in a primary cell culture produced small amounts of LTB4, the concentrations being less than 0.1 per cent of those of prostaglandin E2 (PGE2). PGE2 synthesis in synovial cells was increased when arachidonic acid or interleukin-1 was added to the culture, whereas LTB4 production remained unaltered. The present results suggest that in inflamed joints LTB4 originates mainly from PMNs whereas synovial lining cells are the source for PGE2.

    Notizen: Summary The relation between the loss of height and bone mineral density (BMD) at the lumbar spine and proximal femur was determined in 61 women aged 56–70 years suffering from rheumatoid arthritis. Statistically highly significant negative correlations were found between the loss of height and the spinal and femoral BMD. A loss of 4 cm or more in height over 10 years seems to be associated with a significant decrease of BMD, and it can be recommended as a clinical marker of osteoporosis.

    Notizen: Abstract: We studied whether the high incidence of secondary amyloidosis (SA) is a consistent finding in patients with inflammatory joint disease. A total of 4508 biopsies of patients with rheumatoid arthritis, psoriatic arthritis or ankylosing spondylitis were studied at the Rheumatism Foundation Hospital during 1987–1997. The results show that the annual number of findings of SA was reduced from 68 to less than 10. We suggest that a change in medication towards more frequent use of cytostatic agents is the reason for the reduction in incidence of SA.

    Notizen: Summary Incidence and outcome of inflammatory joint diseases (IJD) were studied by an epidemiologically based follow-up study of unselected adult arthritis patients. The annual incidence of all IJD together was 2.18‰ in the population of 16 years and older; 5.67‰ in the HLA B27 positive and 1.62‰ in the B27 negative population. Almost half of all arthritides were of non-specific nature. Incidence of definite rheumatoid arthritis was 0.42‰ per year. The outcome after 8 years was worst in seropositive rheumatoid arthritis and best in seronegative oligoarthritis of unknown etiology. Outcome of seronegative polyarthritis was bimodal: majority of patients with a favourable and minority with a poor outcome. In Yersinia arthritis, patients with a mainly IgA-class antibody response had a more severe clinical picture than those with a mainly IgG-class response. Diagnosis of rheumatoid arthritis at the early stage of the disease should be based on specific features like rheumatoid factor, radiological signs, symmetric peripheral polyarthritis and morning stiffness with a long duration. Erosive rheumatoid arthritis is almost always seropositive, at least at some stage of the disease.

    Notizen: Summary The effects of 10 ml of evening primrose oil or olive oil, administered twice daily for 12 weeks, on clinical and laboratory signs and on plasma prostaglandins were studied in 18 patients with rheumatoid arthritis. The plasma concentration of PGE2 decreased and that of TxB2 increased in both treatment groups, but no significant improvement could be seen in either group.

    Notizen: Summary The usefulness of ultrasonography in detecting intraarticular effusion of the hip joint is discussed in the light of three cases. In all these patients, effusion was demonstrated by ultrasonography and confirmed by aspiration of inflammatory synovial fluid from the joint, though the joint was clinically symptomless. It is recommended that all rheumatologists use ultrasonography.

    Notizen: Summary The clinical synovitis and radiological destruction of wrist and subtalar joints were followed over a 3-year period in 107 patients with rheumatoid arthritis of no longer than 6 months' duration. The joints were examined clinically and by X-ray on entry to study, at one year and at three years of study. The progress of joint destruction was highly significantly correlated with the frequency of clinical synovitis at the three examinations in both wrist and subtalar joints.
