Zusammenfassung Fortschritte auf dem Gebiet der Leber, Gallenwegs- und Pankreaschirurgie betreffen heute vor allem die präoperative Diagnostik: Die gezielte Arteriographie der Oberbauchorgane, aber auch Leber- und Pankreasszinti graphie lassen pathologische Prozesse lokalisieren, z. B. Leberabscesse oder endokrine Pankreastumoren. Ohne jede Belästigung und Gefährdung des Patienten läßt sich die Ultraschalltomographie durchführen, die ebenfalls bemerkenswerte Ergebnisse zeigt. Die intraoperative Diagnostik, vor allem die Radiomanometrie der Gallenwege 1äßt Steine, Tumoren und Stenosen im Gallenwegssystem ausschließen. Wird sie nicht routinemäßig angewendet, so ergeben sich mehr als 10°f, Fehldeutungen, die dann zum „Postcholecystektomiesyndrom” führen. In der Pfortaderchirurgie ergibt eine zusätzliche druckadaptierte Arterialisation des Pfort-aderstumpfes bessere Ergebnisse; die postoperative Encephalopathie 1äßt sich dadurch verhindern. In der Pankreatitisbehandlung geht der Trend zur aktiven operativen Behandlung, bei der Behandlung maligner Pankreastumoren zur Radikaloperation, wenn Operabilitdt besteht.

Summary Today Progress in surgery of the liver, biliary system and the pancreas concerns mainly the preoperative diagnosis: The selective arteriography of the upper abdominal organs but also the scintillation scanning of liver and pancreas allows to localize a pathologic process as for instance abscesses of the liver or endocrine tumors of the pancreas. The ultrasonic tomography which can be performed without any harm or danger for the patient also shows remarkable results. The intraoperative diagnostic mainly the radiomanometry of the bile ducts makes it possible to exclude stones, tumors and stenosis in the biliary system. If it is not applied routinely it will result in more than 10°/u of wrong diagnosis which then will lead to the “Postcholecystectomysyndrome”. In the surgery of portal hypertension the additional pressure adapted arterialization of the stump of the portal vein results in better results thus preventing the postoperative encephalopathy. In the management of pancreatitis there is a trend to-wards active operative treatment and in the management of malignant tumors of the pancreas towards radical surgery if operability is given.
