    Notes: A new technique has been developed to probe the region of apparent bistability due to a tunneling resonance in the characteristic of a semiconductor asymmetric double-barrier structure. The measuring circuit uses a voltage supply designed to have a load line with positive slope, equivalent to a voltage source and negative series resistance. The appearance of bistability and hysteresis in the characteristic is an artifact of the conventional measuring technique, which employs a load line with negative slope. The complete characteristic is found to be a continuous Z-shaped curve between 50 and 150 K, corresponding to tristability. Equivalent circuit models for the device and voltage supply predict a narrow range of circuit parameters for which a static operating point exists inside the tristable region.

    Notes: The term baboon syndrome was introduced 20 years ago. It was proposed for a particular eruption mimicking the red gluteal area of the baboon after systemic exposure either to the contact allergens nickel and mercury or to oral ampicillin. Previously this phenomen was e.g. termed flexural exanthema to mercury. Since then similar incidences have been reported after systemic exposure to betalactam antibiotics and other drugs, sometimes under different terms. Clinically sharp demarcation of a V-shaped erythema is present in inguinal/genital and gluteal/perianal areas. Additionally papules, vesicles and hemorrhagic lesion may be found. In most patients presence of exanthema in other anatomical flexural regions (axilla, elbow, knee, ventral neck) has been identified. However, involvement of the plantar/palmar regions, face and mucosal sites has not been documented. Typically blood tests are normal and systemic symptoms are absent. Histological results vary but a predominance of superficial perivascular infiltrate with mononuclear cells has been reported. Occasional positive patch tests or stimulation of lymphocytes in the LTT may be suggestive for a type IV allergic reaction. Since the drug-related baboon syndrome shows clinically and histologically different findings to systemic contact dermatitis and acute generalized exanthematous pustulosis (AGEP), it should be seen as a separate entity. The mainstay for the diagnosis of systemic contact dermatitis requires contact sensitization and elicitation by a contact allergen. In contrast in the baboon syndrome and AGEP history for sensitization is often negative, although sometimes positive patch and LTT results can be found. AGEP, on the other hand, requires a distinct score including systemic signs and leucocytosis. In all three symptoms typically occur within 1 to 2 days. The term baboon syndrome does not reflect the whole range of symptoms and signs. And, although it is easily remembered, the comparison to an animal may be insulting. Therefore, we suggest that drug-induced baboon syndrome should be separated from the contact allergen-induced form, and should be recognized as a separate entity within the spectrum of drug eruptions. For this reason we propose the acronym SDRIFE (symmetrical drug related intertriginous flexural exanthema). This refers to the distinctive clinical pattern of this drug eruption without implicating any pathomechanism, which has not been clearly elucidated. The following criteria are proposed: 1) Exposure to a systemically administered drug, 2) Sharply demarcated erythema of the gluteal/perianal area and/or V-shaped erythema of the inguinal/perigenital area, 3) Involvement of at least one other intertriginous/flexural fold, 4) Bilaterally or symmetrically affected areas, and 5) Absence of systemic symptoms and signs.

    Notes: Cutaneous drug reactions occur with a frequency of 1–8% and can be higher for certain classes of drugs. They can range from mild morbilliform eruptions to more severe forms such as drug-hypersensitivity syndrome, toxic epidermal necrolysis or anaphylaxis. Acute generalized exanthematous pustulosis (AGEP) is considered to be a clinical reaction pattern, which is induced in over 90% of the cases by systemic drugs. It is a rare presentation of an adverse drug reaction most frequently triggered by anti-infectious drugs. A high proportion of these cases have been attributed to aminopenicillins and macrolides. We report a terbinafine-induced AGEP in a 68-year-old male confirmed by lymphocyte stimulation in vitro, and review the published cases induced by antimycotic drugs with special emphasis on terbinafine-triggered cases.

    Notes: Abstract This study was done to determine the effect of the direct ileal pouch-anal anastomosis upon pressure and sensory components of the anal canal and ileal pouch. These findings were related to postoperative continence. Thirty-three patients with ileal pouch-anal anastomosis (25 continent, eight with episodic minor incontinence) were studied 3±0.3 and 25±5 months after ileostomy takedown. The maximum resting pressure in the anal canal was significantly lower in patients with an imperfect result (35±5 mm Hg) than in continent patients (44±5 mm Hg) (P〈0.05). Postoperatively the maximum squeeze anal pressure was slightly greater in continent than in incontinent patients (99±8 mm Hg vs.87±7 mm Hg) (P〉0.05). The postoperative recto-(ileo-)anal inhibitory reflex was present in 27 percent. The linear correlation between strength of rectal (ileal) distension and depth resp. duration of internal sphincter relaxation as preoperatively observed disappeared postoperatively in every group of patients. Simultaneous measurements of pouch and anal pressure in patients with imperfect results revealed a reduced positive pouch anal pressure gradient compared to the continent group. This low pouch-anal pressure gradient is thought to be responsible for the increased incidence of soiling in some of our patients.
