    Notes: Eosinophil cationic protein (ECP) is a cationic protein secreted by eosinophils with toxic properties for the respiratory epithelium. Sputum-ECP levels have been shown to correlate inversely with airflow obstruction in asthma. In the present study we investigated whether ECP concentrations are different between asthmatic patients and patients with chronic bronchitis. Sputum-ECP concentrations from seven patients with bronchial asthma and seven patients with chronic bronchitis matched for FEV, were compared (FEV, 66.1 f 29.0% of predicted; FEV, Chronic Bronchitis: 65.2 33.3% of predicted; p = n.s.). Furthermore, sputum-ECP levels in 4 asthmatic patients with severe airflow obstruction and in 1 patient with chronic bronchitis were measured before and after initiation of a 7-day oral therapy with methylprednisolone 20 mg BID. Changes in sputum-ECP values were compared with changes in FEV, in these 5 patients. Sputum-ECP levels and pulmonary function were measured as previously described (Am Rev Respir Dis 1992: 145: 604). Sputum-ECP levels from asthmatics were significantly elevated compared with patients with chronic bronchitis: asthma: 893.4 f346.2 pg/l per g sputum; chronic bronchitis: 30.0 f 8.5 pg/l per g sputum (p = 0.002). The degree of airway obstruction correlated with the sputum-ECP levels in asthmatics (r = -0.76, p = 0.05). but not in the patients with chronic bronchitis (r = 0.24, p = n.s.). In a separate group of 4 patients with asthma, elevated sputum-ECP levels prior to initiation of systemic corticosteroid therapy were associated with a marked improvement in FEV, after treatment, though this was not observed in one patient with chronic bronchitis and low sputum-ECP levels. We conclude that airflow obstruction in bronchial asthma, but not chronic bronchitis, is associated with elevated sputum-ECP concentrations. Furthermore, preliminary data suggest that elevated sputum-ECP levels predict reversibility of airflow obstruction with systemic corticosteroid therapy. Hence, sputum-ECP levels might be helpful in the differential diagnosis and treatment strategies of bronchial asthma and chronic bronchitis, especially in those patients who lack significant peripheral blood- or sputum-eosinophilia.

    Notes: Background Leukotrienes have been implicated in the selective infiltration of eosinophils into the bronchial mucosa in asthma.Objective We studied whether eosinophil transmigration through cultured human umbilical vein endothelial cells (HUVECs) can be blocked by a specific cysteinyl LT1-receptor-antagonist.Methods Unstimulated and stimulated eosinophils from patients with asthma and normal controls were subjected to confluent human umbilical vein endothelial cell (HUVEC) monolayers separating the upper and lower chamber of Transwell™-culture plates. Unstimulated eosinophils or cells pre-incubated in the presence of the eosinophil activating cytokines GM-CSF or IL-13 were placed in the upper chambers while PAF, a potent chemoattractant factor for eosinophils, was added to the lower chamber. Migration of eosinophils was quantified by a β-glucuronidase assay.Results The assumption that eosinophils express CysLT1 (cysteinyl-leukotriene 1)-receptors was based on our demonstration of mRNA-expression for the CysLT-1-receptor by polymerase chain reaction on purified eosinophils. The chemotactic response to PAF was significantly reduced when eosinophils were pre-incubated with montelukast for 15 min. When eosinophils were pre-incubated with GM-CSF and/or IL-13, the migratory response to PAF was also significantly reduced by montelukast.Conclusion From these data we conclude that the specific cysteinyl LT1-receptor antagonist montelukast can inhibit PAF-induced eosinophil transmigration through cultured HUVEC monolayers.

    Description / Table of Contents: Zusammenfassung Bei 10 Patienten mit chronischem Asthma bronchiale und bei 10 lungengesunden Personen wurde die Wirkung von körperlicher Belastung und willkürlicher Hyperventilation auf die Bronchomotorik mit und ohne Dinatrium cromoglicicum untersucht. Die nach Belastung und nach Hyperventilation auftretende Bronchokonstriktion war unter Dinatrium-cromoglicicum-Medikation bei den Lungengesunden nicht mehr nachweisbar, bei den Asthmatikern signifikant geringer ausgeprägt. Die möglichen Angriffspunkte von Dinatrium cromoglicicum in den reflektorischen Mechanismen, die zur Veränderung des Bronchotonus führen, werden erörtert.
    Notes: Abstract The effect of physical exertion and intentional hyperventilation on the bronchial motoricity, with and without previous inhalation of SCG, was investigated in 10 patients with chronic bronchial asthma and in 10 persons with normal lungs. Bronchoconstriction induced by exertion and hyperventilation was significantly reduced in the asthmatics after previous inhalation of SCG, while no measurable constriction at all could be found in the persons with normal lungs after the same procedure. The possible sites of action of SCG, on the reflectory mechanisms which lead to a change in the bronchotonus, are discussed.

    Notes: Zusammenfassung Mittels eines den physiologischen Dimensionen des Thorax-Lungen-Systems angepaßten druckkonstanten Ganzkörperplethysmographen gelingt es, die Alveolardruck-Fluß-Volumen-Beziehungen als Funktion der Zeit bei allen Atemmanövern integral zu erfassen. Die Berechnung der gesamten gasdynamischen Atemleistung wird aufgrund „effektiver“ Widerstands- und Atemstromstärkedefinitionen anhand mono- und bialveolärer Modellvorstellungen erläutert. Patienten mit obstruktiver Lungenkrankheit sind während des Atemgrenzwertmanövers und des forcierten Vitalkapazitätsatmens in der Lage, mehr gasdynamische Leistung als Lungengesunde aufzubringen. Eine überschießende Mitarbeit beim forcierten Exspirationsstoß (Test von Tiffeneau) führt nicht nur bei Patienten mit obstruktiver Lungenkrankheit, sondern auch bei Lungengesunden zu Einschränkungen der effektiven, für ein gegebenes intrathorakales Lungenvolumen maximal erreichbaren, Atemstromstärken.

    Description / Table of Contents: Zusammenfassung Die ganzkörperplethysmographischen Messungen mit einem luftgefüllten BTPS-Rückatmungsbeutel werden durch den ansteigenden CO2-Teildruck und den sinkenden O2-Teildruck zeitlich limitiert. Durch die Einfürhrung eines CO2-Absorbers ins Rückatmungssystem gelingt es, die CO2-Konzentration nach 20–40 Atemzügen konstant auf ca. 3% zu halten. Der mit ca. 15 l reinem O2 gefüllte Rückatmungsbeutel verhindert nicht nur die bei Luftatmung auftretende Hypoxämie, sondern stabilisiert auch das Atemminuten-volumen und die Meßbedingungen.
    Notes: Abstract Measurements with the whole-body plethysmograph are often difficult because the increasing CO2 partial pressure and the decreasing O2 partial pressure in the rebreathing bag filled with air under BTPS conditions limit the time available for the investigation. The CO2 concentration in the rebreathing system can be held at a constant level of about 3% by means of a simple technical device for CO2 absorption. This concentration is reached 20–40 breaths after the connection of the subject with the rebreathing bag filled with about 15 l pure oxygen. This prevents hypoxemia and stabilizes the ventilation of the patients as well as the conditions for the measurements in the whole-body plethysmograph.

    Description / Table of Contents: Zusammenfassung In einer Doppelblindstudie wurde bei 11 Lungengesunden und bei 8 Patienten mit chronischer Bronchitis im cross over-Verfahren der Einfluß von Terbutalin auf das mukociliare Klärsystem der Lunge untersucht. Nach Inhalation eines radioaktiven Aerosols (99mTechnetium Schwefelkolloid) wurde das Depositionsmuster durch extern über der Brust aufgesetzte Detektoren bestimmt und die Verschwinderate über 200 min verfolgt. Dabei fand sich sowohl im Normalkollektiv als auch bei den Patienten mit chronischer Bronchitis eine signifikante Beschleunigung der Klärrate unter Terbutalin. Bei vergleichbaren Depositionsmustern und unter der Annahme, daß Terbutalin qualitativ und quantitativ das Bronchialsekret nicht wesentlich beeinflußt, wird daraus auf eine Stimulierung des Ciliarapparates geschlossen.
    Notes: Abstract In a double blind cross-over study, the effect of terbutaline (5 mg p.o.) on the mucociliary system of the lung was tested in 11 normal subjects and in 8 patients with chronic bronchitis. After inhalation of a radioactive aerosol (99mTechnetium-sulfur colloid), the deposition pattern and the clearance rate up to 200 min were studied. In both groups, a significant increase in the clearance rate was observed following terbutaline compared to the placebo. These findings were interpreted in favour of a positive effect of terbutaline on the ciliary apparatus.

    Notes: Abstract Evaluating lung volumes, the measured values are compared with predicted values. Determination with nomograms is time consuming and perhaps incorrect. For calculation in a desk-computer, equations with 1 or more variables are necessary. We determined therefore from the single values of the study of Amrein and Herzog, dependent on sex, age and height, regression-lines for following parameters: Vital capacity, forced expiratory volume, residual volume, functional residual capacity, resistance Our values are compared with those of the European Community for Coal and Steel.
