    Notes: The relationship between quadriceps muscle temperature (Tm) and sprint performance was evaluated during soccer matches in 25 competitive players. In one game, Tm was determined frequently (n=9). In another game, eight players performed low-intensity activities at half-time (re-warm-up, (RW), whereas another eight players recovered passively (CON). Tm was 36.0±0.2 °C at rest and increased (P〈0.05) to 39.4±0.2 °C before the game and remained unaltered during the first half. At half-time, Tm decreased (P〈0.05) to 37.4±0.2 °C, but increased (P〈0.05) to 39.2±°C during the second half. In CON and RW, Tm and core temperature (Tc) were similar before and after the first half, but 2.1±0.1 and 0.9±0.1 °C higher (P〈0.05), respectively, in RW prior to the second half. At the onset of the second half, the sprint performance was reduced (P〈0.05) by 2.4% in CON, but unchanged in RW. The decrease in Tm was correlated to the decrease in performance (r=0.60, P〈0.05, n=16). This study demonstrates that in soccer, the decline in Tm and Tc during half-time is associated with a lowered sprint capacity at the onset of the second half, whereas sprint performance is maintained when low-intensity activities preserve muscle temperature.

    Notes: Lipid class dynamics and the relationship between weight and length were analysed during early life stage (ELS) development in wild populations of shortbelly rockfish Sebastes jordani to investigate their utility in a condition or nutritional status assessment. Analyses from more than 3000 field-captured shortbelly rockfish indicated little variation in weight for a given length and provided limited application in the assessment of condition. The fractionation of total lipids into individual classes revealed trends in lipid metabolism throughout development. These trends were described best using a stage-specific regression model, since Sebastes show defined early life history transitions. Among developmental stages, triacylglycerols (TAG) and polar lipids (PL) were the dominant lipid classes and cholesterol (CHOL), sterol/wax esters, and nonesterified fatty acids were found in lower concentrations. In the preflexion stage, low concentrations and constant levels of TAG along with the combined influence of endogenous and exogenous sources of energy compromised the use of TAG as an index of nutritional status. However, during flexion through juvenile stages, TAG levels increased indicating an accumulation of energy reserves with development, thus providing a valid indicator of nutritional status in later stages. Large fluctuations in PL concentrations during development suggested complex metabolism which may be due to changes in cell volume and cell membrane proliferation with development as well as catabolism. CHOL declined in larval stages, then remained constant with increasing size through the juvenile stage, negating its use as a normalizer for variations of mass in a TAG/CHOL ratio prior to juvenile transformation. This study provides the first documentation of lipid dynamics during early life stages following parturition in a marine viviparous teleost.

    Notes: Ethische Prinzipien: Das Wohl des Patienten sollte im Vordergrund stehen (Prinzip des Nutzens), es sollte ihm kein Schaden zugefügt werden (Prinzip der Schadensvermeidung) und sein Recht auf Selbstbestimmug muß Berücksichtigung finden (Prinzip des Respekts vor der Autonomie). Jeder Patient sollte in gleicher Weise Zugang zur notfallmedizinischen Versorgung haben (Prinzip der Gerechtigkeit). Umsetzung der Prinzipien: Die Umsetzung des Prinzips des Nutzens in der präklinischen Notfallmedizin erscheint angesichts erfolgreicher Wiederbelebungsversuche offensichtlich. Zahlreiche Menschenleben konnten dank rechtzeitiger Hilfe gerettet werden. Auch das ethische Prinzip der Schadensvermeidung ist bei der Durchführung von Reanimationsmaßnahmen zu berücksichtigen. Mancher Patient verstirbt nach zunächst erfolgreicher Wiederherstellung der Kreislauffunktion Tage oder Wochen später auf der Intensivstation, ohne das Bewußtsein wiedererlangt zu haben. Andere können nur mit schweren neurologischen Einschränkungen aus der Klinik entlassen werden. Das Recht des Patienten, eine autonome Entscheidung zu treffen, also eine medizinische Maßnahme zu befürworten oder abzulehnen, bleibt grundsätzlich auch in einer Notfallsituation bestehen. Der Betroffene kann allerdings aufgrund der schnell eintretenden Bewußtlosigkeit dieses Recht nicht mehr aktiv wahrnehmen. Patienten, die sich im Endstadium einer chronischen Erkrankung befinden und Wiederbelebungsmaßnahmen ablehnen, können diesen Entschluß durch eine entsprechende Verfügung festhalten. Gerade in der Praxis der präklinischen Reanimation ist jedoch die Berücksichtigung einer solchen Verfügung aufgrund der äußeren Bedingungen und des Zeitdrucks stark eingeschränkt. Im Einzelfall kann ein solcher Hinweis die notärztliche Entscheidung, auf Wiederbelebungsmaßnahmen zu verzichten, erleichtern. Im Zweifel müssen immer Wiederbelebungsmaßnahmen begonnen werden. Unter stationären Bedingungen kann im weiteren Verlauf die Indikation zur Reanimation überprüft werden und ggf. nach sorgfältiger Abwägung ein sekundärer Therapieverzicht erfolgen.
