    Description / Table of Contents: Sommario Il fusso ematico regionale cerebrale (rCBF; metodo inalatorio con xenon-133) è stato valutato in due gruppi di pugilatori (24 dilettanti e 20 professionisti) ed in un gruppo di 10 judoka. I risultaoi sono stati confrontati con quelli ottenuti in un gruppo di volontari sani di pari età e sesso. Diciotto pugilatori (9 professionisti e 9 dilettanti) e tutti i judoka eseguirono anche un elettroencefalogramma (EEG). I valori medi di rCBF non differivano da quelli dei soggetti normali sia nei pugilatori dilettanti the nei judoka, mentre i valori dei pugili professionisti erano significativamente ridotti (p〈.001) in tutto il cervello ma soprattutto nelle regioni frontocentrali bilateralmente. La distribuzione dei valori globali di CBF (media delle 32 sonde) era simile nei controlli normali, nei judoka e nei pugilatori dilettanti, sebbene tra quest'ultimi il 12,5% presentava valori significativamente più bassi dei normali. I pugili professionisti presentavano nel 25% dei casi valori globali di CBF significativamente più bassi; negli altri casi il CBF globaae era più basso della media dei controlli, anche se in maniera non significativa. I valori globali di fusso non erano correlati nei due gruppi di pugilatori al numero dei combattimenti ufficiali. Ipoperfusioni regionali, localizzate soprattutto nelle regioni frontocentrali di entrambi gli emisferi, furono trovate nel 35% dei pugili professionistei e nel 29% dei dilettanti ma mai nei judoka. L'EEG è risultato normale nei judoka e nei pugili dilettanti, ma alterato in 3 pugili professionisti. Lo studio ha mostrato l'importanza dell'esplorazione neurofisiologica in atleti ingaggiati in sports in cui la violenza può essere causa di lesioni cerebrali anche importanti. Da questo punto di vista i pugilatori, specialmente i professionisti, risultano compromessi rispetto ai soggetti normali, a differenza dei judoka. Nei pugilatori, la mancata correlázione tra valori perfusionali e numero di incontri indica piuttosto la difficolta di tenere conto di variabili difficilmente quantificabili, quali la quantità e l'asprezza degli incontri fatti durante gli allenamenti.
    Notes: Abstract Regional cerebral blood flow (rCBF) was measured by 133-xenon inhalation in 24 amateur and 20 professional boxers, and in 10 judoka. Results were compared with those from age- and sex-matched healthy controls. Eighteen boxers (9 amateurs and 9 professionals) and all judoka also underwent electroencephalography (EEG). Mean rCBF values did not differ between either amateur boxers or judoka and controls, whereas in professional boxers rCBF was significantly (p〈.001) reduced in the whole brain, especially in the frontocentral regions. Healthy subjects, judoka, and amateur boxers showed a similar distribution of global CBF (gCBF, the mean of 32 probes) values, although 12.5% of amateurs had a significantly lower gCBF than controls. Among professional boxers, 25% showed a significantly low gCBF value; in the remaining 75%, gCBF was below the mean value of controls but did not reach statistical significance. Regional hypoperfusion, mainly in the frontocentral regions of both sides, was found in 35% of professional and in 29% of amateur boxers. A correlation between gCBF values and number of official matches was not found in boxers. EEG was normal in all judoka and amateur boxers, but it was abnormal in 3 professionals. This study shows the relevance of the neurophysiological assessment of athletes engaged in violent sports which can cause brain impairment. In fact, while professional boxers may show brain functional impairment in comparison to normal subjects, judoka do not. The lack of correlation between CBF values in boxers and the number of official matches points to the difficulty of taking into account variables, such as the number and the severity of matches during training.

    Notes: Summary An increase of regional cerebral blood flow (rCBF) has been shown to occur in man during spinal cord stimulation (SCS) by Hosobuchi (1986) and by Meglioet al. (1988) using the 133-Xenon wash-out technique. In this paper we report the effects of SCS on CBF as measured by two different techniques: 8 patients were studied with the 133-Xe method and 28 with the transcranial doppler sonography (TCD), in two cases both studies were performed. The aim of our study was to: 1-verify the effect of SCS on CBF, 2-compare observations made by two different methods, and 3-evaluate a possible correlation between the stimulated spinal segmental level and the modification of CBF. The results of our study confirm that SCS interacts with the mechanisms of regulation of CBF. The stimulation of different spinal cord levels in the same patient can produce different effects and such effects are reproducible. An increase of CBF is more likely to occur with the stimulation of the cervical spinal cord. In patients studied by both methods the sign of CBF changes induced by SCS was the same. Finally, in two patients the effect of SCS on CO2 autoregulation was studied with the TCD. The results of such a study, although preliminary, suggest that CO2 and SCS have a competitive effect upon the mechanisms of regulation of CBF.

    Notes: Abstract A new case of malignant atrophic papulosis (Köhlmeier-Degos disease) is reported. Vascular symptoms began at 17 months of age with cerebral ischaemia and progressive involvement of fingers and toes with torpid ulcers and apical necrotic amputations. At 6 years of age he developed chronic intestinal ischaemia with malabsorption and a new cerebral attack; in spite of anti-aggregant therapy the disease progressed and he died 7 months after diagnosis from a third cerebral ictus. Since the typical skin lesions of the disease were absent, the diagnosis was made on the basis of a pathological pattern of an occluded biopsed artery. The elder brother presents clinical and instrumental vascular involvement without cutaneous lesions and could be slightly affected.

    Notes: Summary In a double-blind study the analgesic activity of a new compound, diviminol 100 mg, was compared with that of codeine 60 mg, and a placebo given as single oral doses to three comparable groups of 30, 31 and 32 patients with post-partum pain. The two drugs were very significantly more active than the placebo on comparing both the hourly pain relief scores, and the frequency of satisfactory analgesia. There was no significant difference between the new drug and the reference coumpound. No undesirable side-effects were noted. These results in post-partum pain provide additional evidence that diviminol is a clinically effective analgesic agent.

    Description / Table of Contents: Résumé Le but de ce travail a été d'établir un modèle in vitro permettant d'étudier les fonctions normales et pathologiques de l'épithélium colique. Nous avons isolé des cryptes coliques par une digestion protéolytique douce et par une dissection mécanique de matériel biopsique prélevé au cours de colonoscopies. Les cryptes dépourvues de tissu conjonctif, placées en milieu de culture, se fixent rapidement sur le substrat et forment des colonies qui renferment plus de 95% de cellules épithéliales. L'histochimie et la caractérisation ultrastructurelle des colonies ont montré la présence de cellules d'absorbtion et de cellules secrétoires présentant un haut degré de différentiation. La prolifération active survient le plus souvent durant les premières 24 heures et diminue progressivement par la suite. Les cellules survivent et maintiennent les caractéristiques de différentiation durant au moins 3 jours en culture. Cette méthode peut être utilisée pour étudier les fonctions normales de l'épithélium colique. Elles peuvent aussi être utilisées pour investiger les facteurs nocifs et protecteurs dans des situations pathologiques telles que les maladies inflammatoires de l'intestin et les tumeurs colorectales.
    Notes: Abstract The purpose of this work was to set up an in vitro model for the study of normal and pathological functions of the colonic epithelium. We have isolated colonic crypts by mild proteolytic digestion and mechanical dissociation of human biopsy material obtained during colonoscopy. The crypts, free of connective tissue, when placed in culture rapidly attached to the substrate and formed colonies containing over 95% of epithelial cells. Histochemical and ultrastructural characterization of the colonies showed the presence of both absorptive and secretory cells, exhibiting a high degree of differentiation. Proliferative activity occurred mostly during the first 24 h and progressively declined thereafter. The cells survived and maintained differential characteristics for at least three days in culture. This method can be used to study normal functions of the colonic epithelium. It may also be employed to investigate both noxious and protective factors in pathological conditions such as inflammatory bowel disease and colorectal neoplasia.
