    Notes: Summary In 25 children with chronic disease of the maxillary sinus we did sinutomy with the transnosal way since 1976. A mucosal flap is cut after lateroposition of the lower concha, so that the lateral wall of the nose can be opened wide for removing of pathologic structures of the maxillary sinus under endoscopic control. It is important to take care of the not alterated mucosa. In our patients there could be seen a mucosal reaction 4 weeks after operation, but another 4 weeks later there were normal x-rays with ventilated maxillary sinuses. Therefore this method till now mainly practised in adults can be employed for children, too.

    Notes: Summary In 120 patients with vestibular neuritis the different neurotological results in various states of the disease are discussed. For the ENT-specialist in clinic and office it is very important to know that the vestibular neuritis in the beginning has another neurotological result than in later states. Often the patient comes to a neurotological diagnostic some weeks or months after having been treated for a long time assuming another diagnosis. But the vestibular test often shows only the state of central compensation or the state of remission. In the acute state the caloric reaction and the gaze nystagmus are important vestibular tests, later on the positional test and the vestibular-spinal reactions.

    Notes: Abstract Objective: The concentration-time profiles of specific metabolites of moxisylyte, an α-adrenoceptor blocking agent, in the plasma and urine from 18 healthy volunteers were investigated after intracavernous (IC) administrations at three dose levels (10, 20 and 30 mg). Results: Four metabolites, unconjugated desacetyl-moxisylyte (DAM), DAM glucuronide, and DAM and monodesmethylated DAM (MDAM) sulphates were found in plasma and urine. For all metabolites, t1/2 elimination was independent of the administered dose (1.19 h for unconjugated DAM; 1.51 h for DAM glucuronide; 1.51 h for DAM sulphate; and 2.17 h for MDAM sulphate). Cmax and AUC increased in direct proportion to dose, except for the inactive DAM glucuronide. Any the differences detected were small and equivalence of the three doses can be accepted. Conclusion: The pharmacokinetics of moxisylyte in humans following intracavernous administration were linear in the dose range 10 to 30 mg.

    Description / Table of Contents: Zusammenfassung Tierexperimentell bewährte sich die Fibrinklebung bei der Absicherung von Colon- und Tracheaanastomosen, der Deckung von Tracheadefekten mit Knorpeltransplantaten und der Wundverklebung an parenchymatösen Oberbauchorganen. Der gewebliche Abbau des Fibrinklebers entspricht dem körpereigenen Fibrins. Positive klinische Erfahrungen betreffen: Absicherung von Anastomosen am Verdauungstrakt, Fibrinpleurodese bei persistierendem oder rezidivierendem Spontanpneumothorax, Blutstillung und Prothesenabdichtung in der Herzchirurgie, plastische Deckung frontobasaler Liquorfisteln sowie Transplantatfixation im HNO-Rereich. Ein erhöhtes Hepatitisrisiko ließ sich nicht nachweisen.
    Notes: Summary In animal experiments fibrin glue proved useful in the protection of colonic and tracheal anastomoses, reconstruction of tracheal defects using cartelage grafts, and repair of hepatic, splenic, and renal wounds. The absorption of the glue in tissue is identical to that of physiological fibrin. Our positive clinical experience includes: Protection of digestive anastomoses, fibrin pleurodesis of persistent or recurrent spontaneous pneumothorax, topical haemostasis and sealing of dacron grafts in cardiac surgery, management of cerebrospinal fluid rhinorrhea, and fixation of transplants in ENT surgery. An increased risk of postoperative hepatitis has not been found.

    Notes: Summary During a six months lasting screening project 6866 persons complaining of hoarseness or other related symptoms of the head and neck were examined free of cost. Inspection of the mouth, endoscopy of the naso-hypopharynx and larynx with the Wolf-endoscope after v. Stuckrad, and palpation of the neck were included. The data from both the questionaires and the examination were evaluated by computer. Every sixth person required further diagnosis or treatment. Precanceroses were found in almost 3%, and up to now malignomas were found in 0.3% of the screened persons. Among theses 14 cases of cancer were 9 carcinomas of the larynx. This comperatively high percentage of precanceroses and cancer favors the establishment of pharyngo-laryngeal endoscopy in cancer prevention projects. In particular the endoscopic screening of patients with organ-related symptoms and of high risk groups is recommended.

    Notes: Summary In 40 cases of tympanomeatoplastic a special glue, consisting of fibrinogen and thrombin, was used to glue grafts of homologous cartilage into the defect of the posterior wall. In 39 of 40 cases the result was satisfactory. In 9 of 10 cases we had successful results in closing traumatic perforations of the tympanic membrane. In 12 cases recidiv perforations after tympanoplastics were clottid without any other manipulation. Since 1978 we used the new glue in 70 cases to fix bone transplants on to the stapes. The audiological results are 5–6 dB better as in usual techniques.
