    Notes: 70 odontogenic cysts with hyaline bodies (HB) were examined by light and scanning electron microscopy. The scanning electron microscopic studies were performed on dried tissue material, which had been previously examined by light microscopy and energy dispersive x-ray analysis of the histologic sections. The form and basic structure of both the early forms of HB as well as the HB Type I and II could be identified more closely. The early forms, the outer component of the HB Type II and the inner and outer component of the HB Type I consisted of a fine-grained substance, which presumably goes through varying “partial homogenization”, thus giving the HB an increased firmness and elasticity. The HB are a product of the epithelium of odontogenic cysts and have direct contact to the outer layer of the adjacent cyst epithelium via its intercellular bridges.

    Description / Table of Contents: Plasmocytic non-Hodgkin’s lymphoma is the most common tumor of bone and bone marrow, typically diagnosed by symptoms such as monoclonal paraproteinemia, proteinuria, anemia and hypercalcemia. In its progress, deposits of amyloids in almost all organs can be observed. However, plasmacytomas which are diagnosed by macroglossia of primarily unknown etiology are rare. This case report presents a 61-year-old woman who suffered from a persistent swelling of the tongue with painful ulcerations. A biopsy led to the diagnosis of primary systemic amyloidosis of the light-chain type, which subsequently proved to be a plasmacytoma with lambda light-chains stage II after Durie and Salmon. In the course of the disease the patient developed further deposits of amyloids in the whole gastro-enteric system. Macroglossia as a primary manifestation of plasmacytoma is rarely described in medical literature. However, reports on deposits of amyloid in the tongue in advanced stages of disease are well known.
    Notes: Das plasmozytische Non-Hodgkin-Lymphom ist der häufigste Tumor von Knochenmark und Knochen, der in der Regel über seine Kardinalsymptome monoklonale Paraproteinämie, Proteinurie, Anämie und Hyperkalzämie diagnostiziert wird. Amyloidablagerungen werden im weiteren Verlauf in fast allen Organen beobachtet. Plasmozytome, die über eine primär unklare Makroglossie diagnostiziert werden, stellen jedoch eine Seltenheit dar. In diesem Fallbericht wird eine 61jährige Patientin vorgestellt, die über eine persistierende Zungenschwellung mit schmerzhaften Ulzerationen klagte. Eine Probeexzision führte zur Diagnose einer primär systemischen Amyloidose vom Leichtkettentyp (AL) aufgrund eines Leichtkettenplasmozytoms vom Typ Ig-λ im Stadium II nach Durie und Salmon. Im weiteren Krankheitsverlauf entwickelte die Patientin zusätzlich Amyloidablagerungen im gesamten Gastrointestinaltrakt. Makroglossien als Frühsymptom eines Plasmozytoms werden in der zur Einsicht stehenden Literatur selten beschrieben, wogegen regelmäßig über Amyloidablagerungen im Bereich der Zunge bei länger bestehender Krankheit berichtet wird.

    Notes: Abstract Observations of explanted bronchial mucosa show that ciliary function is maintained for 7 days subsequent to explantation. This finding demonstrates that non-neural mechanisms exist which regulate ciliary function. Ultrastructural and immunohistochemical studies both for light and electron microscopy were performed on human bronchial biopsy material and lung resection specimens in order to recognize the morphological substrate of this regulatory mechanism. A complex system of cytokeratin filaments and microtubules radiate through the whole cytoplasm of ciliated cells with direct contact to the nucleus, cilia, microvilli, desmosomes and to the apical terminal adhesive complex. Between the basal bodies and the apical terminal adhesive complex microfilaments can be found. In the apical cytoplasm a dense filamentary network is seen in association with the adhesive complex. These morphological findings indicate that the cytoskeleton of the bronchial epithelium plays a key role in the co-ordination of ciliary function.

    Description / Table of Contents: Background. Systemic sclerosis (SSc) is a generalized disorder of the interstitial tissues and vasculature with distinct abnormalities in three systems, immune and autoimmune, vascular and microvascular, and mesenchymal extracellular matrix (ECM), that lead to exuberant fibrosis. The aim of this study was to compare the number of blood vessel profiles in the marginal gingiva between SSc patients and patients with periodontitis but without SSc by using biopsies. Methods. Marginal gingiva and gingival papilla were obtained from 13 scleroderma patients and 8 patients with periodontitis after routine tooth extraction and gingival curettage. On the histological sections, immunohistochemical investigations were performed using the avidin-biotin complex method (ABC) and the monoclonal antibody CD 31/Clone JC70A (platelet endothelial cell adhesion molecule-1). Blood vessels were identified by light microscopy (original magnification × 400) and counted within 0.3615 mm2. Medians of blood vessel profiles were compared by Mann-Whitney U-test. Results. There is no statistical difference between the median of blood vessel profiles in the marginal gingiva of SSc patients as compared to the median of blood vessel profiles in the marginal gingiva of patients with periodontitis (P = 0.665). We did not discover avascular areas in the subepithelial connective tissue. Discussion. The pathological changes in the microvasculature in the dermis of SSc patients are not transferable to the marginal periodont, as intraindividual histological examinations of dermis and oral mucosa in relation to the degree of the disease are not available yet.
    Notes: Hintergrund. Die systemische Sklerodermie ist eine relativ seltene, generalisierte Erkrankung des Bindegewebes. Im Krankheitsprozess spielen entzündliche und vaskuläre Phänomene eine wichtige Rolle. Methode. Anhand von Bioptaten der marginalen Gingiva wurde nach indizierten Zahnextraktionen oder Kürettagen der Vaskularisationsgrad im subepithelialen Bindegewebe der Gingiva von Sklerodermiepatienten mit demjenigen von Patienten mit einer Parodontitis verglichen. Die Mikrogefäßdichte (MVD) wurde immunhistochemisch mit Hilfe des monoklonalen Antikörpers CD31/Clone JC70A (platelet endothelial cell adhesion molecule-1, PECAM-1) dargestellt und lichtmikroskopisch quantitativ erfasst. Die statistische Auswertung der Mediane der Kapillaranschnitte erfolgte mit dem Mann-Whitney-U-Test. Ergebnisse. Die Mediane der Kapillaranschnitte bei den Sklerodermiepatienten (n = 13) und den Patienten mit Parodontitis (n = 8) erwiesen sich statistisch als nicht signifikant verschieden (p = 0,665). Im subepithelialen Bindegewebe der marginalen Gingiva wurden keine avaskulären Zonen gesehen. Diskussion. Die in der Dermis von Sklerodermiepatienten ablaufende Angiopathie ist nicht ohne weiteres auf das marginale Parodont übertragbar, da intraindividuelle histologische Untersuchungen von Dermis und Mundschleimhaut in Abhängigkeit vom Erkrankungsstadium derzeit nicht vorliegen. Das vorliegende Ergebnis rechtfertigt nicht, Probeexzisionen der Mundschleimhaut zu entnehmen, um das Krankheitsstadium einer Sklerodermie anzuzeigen oder eine prognostische Wertung vorzunehmen.

    Notes: Summary Infections with respiratory syncytial virus (RSV) are characterized by frequently occuring reinfections and are regarded to be responsible for bronchial hyperreactivity. In this report we describe a small-animal model suited to study RSV-induced pathogenesis and immune response. Guinea pigs are infected by inhalation of an RSV-aerosol. Lungs of infected animals show signs of a bronchiolitis at 7 days after the initial infection. Although neutralizing serum antibodies are synthesized viral proteins are still detectable at 6 weeks post infection. Therefore, the presence of neutralizing antibodies is obviously not sufficient for rapid clearance of persistent RSV-proteins from the lungs of infected guinea pigs.

    Notes: Abstract  Human (RSV) and bovine (BRSV) respiratory syncytial virus cause similar infections of the lower respiratory tract. Therefore, experimentally infected calves are suited to the study of RSV-induced chronic bronchiolitis. Colostrum-fed calves aged 17–24 days were successfully infected with BRSV. BRSV strain 375 was applied as an aerosol on 4 consecutive days. Clinical symptoms were already evident on the 1st day after infection. The calves were necropsied 12 weeks after the first infection. Focal severe chronic bronchiolitis with atelectasis and focal bronchiolitis obliterans were demonstrated. The bronchiolar lumina were filled with secretion. Transmission electron microscopy revealed an alteration of the ciliogenesis and partial loss of cilia. Immunhistochemically virus P protein could still be detected, mainly in the epithelial cells of the inflamed bronchioli.

    Description / Table of Contents: Treatment of degenerative gonarthritis is one of the main fields of arthroscopy. Different treatment possibilities are discussed considering recent literature, especially concerning dependence of indication on degree of chondromalacia, success and structural changes. Experimental stability testing shows increased translation in gonarthritis as a sign of instability. Partial resection of the posterior meniscal horns lead to considerable loss of stability, causing cartilage defects if no muscular compensation takes place. Finally, treatment methods for cartilage restoration are compared.
    Notes: Die Behandlung der Gonarthrose ist eines der Hauptarbeitsfelder der Arthroskopie. Die verschiedenen Verfahren werden unter Berücksichtigung der aktuellen Literatur abgewogen, insbesondere hinsichtlich ihrer vom Grad der Chondromalazie abhängigen Indikation, der Erfolgsaussichten und der strukturellen Gewebsveränderungen. Experimentelle Stabilitätstestungen zeigen die bei Gonarthrose vorliegende vermehrte Translation als Instabilitätszeichen. Meniskushinterhornteilresektionen erweisen sich als erhebliche Reduktion der sekundären Stabilisation, was ohne muskuläre Kompensation zu erheblichen Knorpeldefekten führt. Schließlich werden Verfahren der Knorpelrestauration verglichen.

    Description / Table of Contents: The case is reported of a 27-year-old patient who developed multiple recurrences of a carcinoma arising from richly stromal pleomorphic adenoma of the parotid gland over 13 years. Originally the tumor was classified as a benign pleomorphic adenoma because there were no cytological characteristics of malignancy. Later on the diagnosis had to be modified to that of a carcinoma arising from a pleomorphic adenoma because of the infiltrating growth pattern of the tumor. A multicentric local recurrence and metastases on the left side of the neck supported the correct diagnosis. This case demonstrates that, beside the classic cytological criteria of malignancy invasion and penetration of the capsule can be decisive for the diagnosis of malign transformed pleomorphic adenoma. Treatment and prognosis of salivary gland tumors assume a clear determination of their diguity. The unusual course in this case of a pleomorphic adenoma originally diagnosed as benign demonstrates the importance of regular follow-up.
    Notes: Es wird der Fall eines 27jährigen Patienten mit mehrfachen Rezidiven eines ursprünglich als pleomorphes Adenom der Glandula parotis diagnostizierten Karzinoms in einem stromareichen pleomorphen Adenom mit einer Laufzeit von 13 Jahren dargestellt. Da keine zytologischen Merkmale der Malignität vorlagen, wurde der Tumor primär als stromareiches pleomorphes Adenom klassifiziert. Aufgrund seines infiltrierenden Wachstumsmusters mußte jedoch schließlich die Diagnose eines Karzinoms im pleomorphen Adenom gestellt werden. Der weitere Verlauf mit der Entwicklung multipler Lokalrezidive sowie von Metastasen im Bereich der linken Halsseite bestätigte die Karzinomdiagnose. Der vorliegende Fall demonstriert, daß in der histologischen Diagnostik des Karzinoms im pleomorphen Adenom die Kapselinvasion und der Kapseldurchbruch als klassische Merkmale der Malignität, neben den zytologischen Kriterien, von entscheidender Bedeutung sind. Für die Therapie und Prognose von Speicheldrüsentumoren ist eine klare Abgrenzung der Dignität ausschlaggebend. Selbst bei primär als benigne eingestuften pleomorphen Adenomen untermauert dieser ungewöhnliche Krankheitsverlauf die Notwendigkeit einer regelmäßigen Nachsorge.

    Notes: Abstract Introduction: We describe a young woman with an unusual pancreatic tumor. Findings and discussion: Intraoperatively, a smoothly demarcated and encapsulated tumor was exposed. It was large (5 cm×4 cm) and of solid consistency, with a small stalk attached to the uncinate process. The tumor was partially surrounded by the pancreatic head. The macroscopic appearance suggested a benign tumor. Frozen sections revealed a benign pancreatic tumor, most likely of endocrine nature. Based on these findings, tumor enucleation was performed. The patient recovered rapidly from the intervention and was discharged from hospital after 2 weeks. One year after surgical treatment, the patient is without recurrence. The final diagnosis of the tumor was a solid pseudo-papillary tumor.

    Notes: Abstract Dermatofibrosarcoma protuberans (DFSP) previously called Morbus Darier is an intermediate grade sarcoma with a fairly high rate of recurrence, it occurs most commonly on the trunk and extremities but may also occur in the head and neck region. Treatment of choice is wide excision. A 69-year-old female patient with a 57-year history of dermatofibrosarcoma protuberans is presented. The lesion appeared on the right forehead and both eyelids at the age of 12 years. The patient required 64 surgeries mostly for the head and neck lesions.
