Abstract Segmental dilatation of the colon is an uncommon entity of unknown etiology. It may be caused by coproliths giving rise to chronic constipation, abdominal distension, tenderness, pain, and colonic obstruction. We report radiographic and clinical findings in an 85-year-old female geriatric patient with recurrent constipation and acute onset of pain in the lower abdomen. Radiography revealed a massively dilated segment of the proximal sigmoid colon containing a huge coprolith. The condition was successfully treated conservatively.
