    Notes: Two newborn infants with traumatic perforation of the pharyngo–oesophageal region are presented. This injury was induced by pharyngeal suction catheters and/or vigorous attempts at nasogastric or tracheal intubation during resuscitation of the newborn. The true nature of this condition remained unrecognized and the babies were thus referred with a tentative diagnosis of oesophageal atresia. The perforation itself could be treated successfully without surgery, despite a severe complication in one infant resulting from inadvertent use of barium sulphate contrast medium. Raising awareness of the possibility of this injury should help in avoiding this complication by gentle and skilful action during newborn resuscitation, particularly in the premature infant.

    Notes: Abstract Successful allogeneic bone marrow transplantation (BMT) for hematologic disorders may be complicated by graft-versus-host-disease (GVHD). Chronic GVHD is a systemic disease, involving, among other organs, the skin, mouth, liver, and esophagus. Esophageal involvement results in mucosal inflammation, leading to submucosal fibrosis and, occasionally, formation of webs and strictures. We investigated 25 allogeneic BMT recipients (17 with and eight without chronic GVHD). All patients had a videofluoroscopic study of the pharynx and esophagus to determine the radiographic abnormalities characteristic of chronic GVHD. Oropharyngeal abnormalities (poor bolus control, pharyngeal retention, or excessive mucous secretions) were found in five patients with and three patients without GVHD. Only one patient with GVHD had a pharyngo-esophageal stricture. There was no significant difference between the two groups with regard to pharyngo-esophageal radiographic abnormalities and esophageal symptoms. Radiographic evidence of esophageal motility disorder is not specific for GVHD involvement. In the absence of specific radiographic features, endoscopy is the most accurate method for the diagnosis of esophageal involvement by GVHD.

    Description / Table of Contents: Abstract Background. The pharynx, being part of the digestive as well as the respiratory system, is embedded in the complex spatial anatomy of the neck and, due to its function and location, represents a very sophisticated region. Diagnosis. Cross-sectional imaging with CT and MR imaging plays an indispensable complementary role to clinical work-up, as far as submucosal and deep cervical spaces are concerned. However, as dysphagia is a common symptom in clinical practice, interpretation of double contrast studies and videofluoroscopic investigations are also on frequent demand in the daily routine. Conclusion. Close interdisciplinary co-operation and the radiologist's familiarity with the anatomy of the region are required in order to use the diagnostic tools in an optimal fashion.
    Notes: Zusammenfassung Hintergrund. Der Pharynx repräsentiert, durch seine Funktion als Luft- und Speiseweg einerseits sowie durch seine Topographie in der komplexen Zervikalregion andererseits, ein äußerst differenziertes Organsystem. Bildgebungsverfahren. Neben den, in den letzten Jahren und Jahrzehnten einer rasanten Entwicklung unterworfenen, und in der Kopf-Hals-Radiologie unerlässlichen Schnittbildverfahren, spielen, gerade bei der Bildgebung des Pharynx, konventionelle Röntgenverfahren, welche eine dynamische Beurteilung der Funktion erlauben, weiterhin eine wichtige Rolle in der Diagnostik. Die Methoden der Bildgebung stellen somit heute, ergänzend zur klinischen Untersuchung, unumstritten das 2. Standbein in der Abklärung von Pathologien am Pharynx dar. Resümee. Dieses Zusammenwirken bietet dem Radiologen, entsprechend der Fragestellung nach funktionellen oder strukturellen Pathologien, ein vielfältiges Betätigungsfeld, erfordert gleichzeitig aber das grundlegende Verständnis der normalen Anatomie und Physiologie sowie die Kenntnis der spezifischen Vor- und Nachteile der zur Verfügung stehenden bildgebenden Verfahren.

    Description / Table of Contents: Zusammenfassung Primäres Ziel der bildgebenden Diagnostik des Larynx ist die Analyse der submukös gelegenen Strukturen, die der laryngoskopischen Beurteilung nicht zugänglich sind. Demzufolge gilt als allgemeine Regel, die bildgebende Diagnostik des Larynx unter Kenntnis der Befunde der endoskopischen Diagnostik vorzunehmen. Die topographisch-anatomischen Verhältnisse des Skeletts, der Binnenstrukturen und der Umgebung des Larynx werden synoptisch dargestellt werden.
    Notes: Summary The primary goal of diagnostic imaging of the larynx is analysis of submucosal structures that are not visible by clinical or endoscopic means. Therefore, image analysis of the larynx should only be performed under knowledge of the results of clinical diagnosis. The anatomic relationships of the laryngeal skeleton, the intrinsic structures, and the surrounding soft tissue are presented synoptically.

    Description / Table of Contents: Summary Esophageal carcinoma: CT and endosonography are complementary and the most important imaging modalities at present for staging. After endoscopic and histological diagnosis, CT of the thorax and the abdomen is used. With the proof of local infiltration of a neighbouring organ or in the presence of distant metastasis, palliative therapy can be started. If CT is not conclusive or no local infiltration or distant metastasis is proven, endosonography should be performed. Gastric carcinoma: At present endosonography shows the highest accuracy for diagnosis of the T stage. For differentiation between T3 and T4 tumors the accuracy of CT is not sufficient to predict resectability. The N stage can be determined at present by no modality with sufficient accuracy. Distant metastasis can be diagnosed by CT with high sensitivity and specificity. Small bowel carcinoma: These rare tumors are diagnosed with high accuracy by enteroclysis, whereby the diagnosis takes place at a late stage due to the nonspecific clinical findings.
    Notes: Zusammenfasung Ösophaguskarzinom: Die Computertomographie und die Endosonographie sind als komplementäre Modalitäten derzeit die wichtigsten bildgebenden Verfahren zur Stadieneinteilung des Ösophaguskarzinoms. Nach der endoskopischen und histologischen Diagnose wird primär die CT des Thorax und des Abdomens eingesetzt. Mit dem Nachweis der lokalen Infiltration eines benachbarten Organs oder von Fernmetastasen kann eine palliative Therapie initiiert werden. Wenn die Computertomographie inkonklusiv ist oder keine Umgebungsinfiltration bzw. Fernmetastasen nachweist, sollte eine Endosonographie durchgeführt werden. Magenkarzinom: Die Endosonographie hat derzeit für das T-Stadium die höchste Treffsicherheit. Für eine eindeutige Unterscheidung zwischen T3- und T4-Tumoren reicht die Treffsicherheit der Schnittbildverfahren nicht aus, um die Resektabilität vorherzusagen. Das N-Stadium kann derzeit von keiner Modalität mit ausreichender Treffsicherheit festgelegt werden. Fernmetastasen können mit hoher Sensitivität und Spezifität mit den zur Verfügung stehenden bildgebenden Modalitäten erfaßt werden. Dünndarmkarzinom: Diese seltenen Tumoren werden mit hoher Treffsicherheit mit dem Enteroklysma diagnostiziert, wobei die Diagnose aufgrund der unspezifischen Klinik in einem fortgeschrittenen Stadium erfolgt.

    Notes: Abstract Segmental dilatation of the colon is an uncommon entity of unknown etiology. It may be caused by coproliths giving rise to chronic constipation, abdominal distension, tenderness, pain, and colonic obstruction. We report radiographic and clinical findings in an 85-year-old female geriatric patient with recurrent constipation and acute onset of pain in the lower abdomen. Radiography revealed a massively dilated segment of the proximal sigmoid colon containing a huge coprolith. The condition was successfully treated conservatively.
