Summary 1. Bilirubin tolerance tests were performed in 8 premature infants, 4 of them with low levels, the other 4 infants with high levels of plasma bilirubin at the onset of the test. 2. It could be demonstrated that the drop of plasma bilirubin following loading is based on distribution processes. 3. The relative volumes of distribution were calculated. The rate of distribution is faster and the volume of distribution larger in cases of high bilirubin concentration of the plasma at the onset than in those with low bilirubin concentration, even though the volumes of distribution at the time 0 are practically equal and identical with the intravascular volume. 4. The possible explanations for these differences are discussed. 5. In prematures the estimation of bilirubin t 1/2 following bilirubin loading permits no conclusions concerning bilirubin elimination by the liver. 6. In prematures the calculation of the endogenous bilirubin formation with the aid of a bilirubin loading test is not permissible.

Zusammenfassung 1. An je vier Frühgeborenen mit hohem und niedrigem Ausgangsbilirubinspiegeln wurden Bilirubinbelastungen durchgeführt. 2. Es ließ sich nachweisen, daß dem Konzentrationsabfall des Bilirubins im Plasma ein Verteilungsvorgang zugrunde liegt. 3. Die relativen Verteilungsräume wurden berechnet. Die Verteilung ist bei hohem Ausgangsbilirubinspiegel schneller und die Verteilungsräume sind größer als bei niedrigem, obwohl die Verteilungsräume zur Zeit 0 praktisch gleich und vom Intravasalvolumen kaum verschieden sind. 4. Die möglichen Ursachen für diese Unterschiede werden diskutiert. 5. Aus einer Bestimmung von Bilirubinhalbwertszeiten lassen sich bei Frühgeborenen keine Aussagen über die hepatische Bilirubinelimination machen. 6. Eine Berechnung des endogenen Bilirubinzuflusses aus einer Bilirubinbelastung ist beim Frühgeborenen nicht statthaft.
