    Notes: Eight cases with striae distensae associated with obesity or prolonged corticosteroid intake were examined by light and scanning electro microscopy. Fine elastic fibers predominated in early lesions, while thick elastic fibers predominated in older lesions. In early lesions the periphery showed thick and tortuous elastic fibers, but in older lesions it was of normal appearance. It is suggested the elastic fibers in striae were newly synthesized and gradually increased and thickened with age.

    Notes: Aims : Apocrine carcinoma of the breast is typically, though not always, positive for gross cystic disease fluid protein-15 (GCDFP-15). In order to clarify the clinical significance of GCDFP-15 in apocrine carcinomas, GCDFP-15 expression was examined in apocrine carcinomas of different stages and compared with clinicopathological factors. Apocrine lesions reportedly exhibit an unusual immunohistochemical status, expressing androgen receptors (AR) instead of oestrogen receptors (ER), progesterone receptors (PR), or bcl-2. Their expression was also examined.Methods and results : Fifty-two apocrine carcinomas were examined immunohistochemically. Thirty-nine (75%) and 29 (56%) were positive for GCDFP-15 and AR, respectively. GCDFP-15 positivity was significantly lower in infiltrating carcinomas than intraductal carcinomas (P = 0.0111). In infiltrating carcinomas, GCDFP-15 positivity was significantly low in tumours ≥ 15 mm (P = 0.0005) and node-positive tumours (P = 0.0004). Similar phenomena were observed for AR. Rare cases were positive for ER (3.8%), PR (5.8%), and bcl-2 (1.9%).Conclusions : GCDFP-15 positivity is transient and should not be considered a definitive marker of apocrine carcinomas. Cases which have apocrine features but lack GCDFP-15 expression should rather be considered as advanced apocrine carcinomas. ER/PR/bcl-2 negativity will sometimes be helpful to confirm the diagnosis of apocrine carcinoma, because it is more consistent than GCDFP-15/AR positivity.

    Notes: Cutaneous scar tissue (atrophic and hypertrophic) and normal dermis were examined using scanning (SEM) and transmission electron microscopy (TEM) as well as light microscopy (LM). Although LM revealed only a few Weigert stain-positive elastic fibers in scar tissue, SEM and TEM disclosed the presence of a large number of fine elastic fibers which was not present in normal dermis. Atrophic scar tissue contained more Weigert stain-positive fibers and fewer fine elastic fibers than hypertrophic scar tissue. In addition, findings suggestive of newly synthesized elastic fibers were seen in some hypertrophic scar tissue. These results indicate that there is elastogenesis in scar tissue.

    Notes: Three patients arc reported, each with a reddish macule or plaque (7–10 mm) induced by trauma. The histology of these lesions showed hyperkeratosis, acanthosis and marked dilation of vessels in the papillary to deep dermis. The electron microscopic study revealed a layer of endothelial cells surrounding the dilated vessels, suggestive of telangiectasia. The relationship between this entity and solitary angiokcratoma is discussed.

    Notes: Aims:  The pathogenesis of breast carcinoma in very elderly women is of interest, because oestrogen levels are likely to be extremely low during the development of the disease. In an effort to understand the pathogenesis of breast carcinoma in these women, this study was undertaken to compare the histological patterns and hormone receptor status of breast carcinomas arising in very elderly and younger women.Methods and results:  Thirty-seven breast carcinomas from women over the age of 85 years at the time of their operation were examined histologically and compared with those from a large group of premenopausal women. The proportions of mucinous carcinoma and apocrine carcinoma were significantly greater in older women. The expression of steroid hormone receptors was studied immunohistochemically. Androgen receptor-positive carcinomas were significantly more frequent among older women, whereas progesterone receptor-positive carcinomas were significantly less frequent. There was no statistically significant difference in oestrogen receptor-α or -β expression between the tumours from both groups.Conclusion:  Breast carcinomas in women over the age of 85 years have a different morphological spectrum from carcinomas in younger age groups and may have different pathogenesis mechanisms that may be more dependent on androgen and androgen receptor interaction. Differences from the results of the other studies are discussed.

    Notes: Summary Three autopsy cases of pontine and extrapontine myelinolysis are reported; one, a malignant lymphoma in a man of 66 years, the other an alcoholic liver cirrhosis in a man of 54 years, and an esophageal cancer in a woman of 68 years who presented only with extrapontine myelinolysis, but lacked central pontine myelinolysis (CPM). The extrapontine lesions in these three cases revealed a characteristic and common localization; they occurred mostly (1) in the bundles of myelinated fibers in the gray matter, such as in the pons, basal ganglia, and thalamus; and (2) in the white matter surrounded by massive gray matter, such as the deeper layers of the cortex and subjacent white matter of the crowns and sides of the cerebral gyri, the white matter of the cerebellar folia and internal, external, and extreme capsules. Therefore, the third patient was classified as a subtype of pontine and extrapontine myelinolysis, which may be called the “extrapontine form” because of absence of CPM. Moreover, bilateral demyelination of the mamillary body was found in all cases, and laminar cortical astrocytosis and necrosis similar to Morel's cortical laminar sclerosis in two of them. From the clinical and pathologic findings, the significance of the changed osmolarity of the blood as a cause and the importance of some specificity of the tissue architecture in the pathogenesis are discussed.
