    Notes: Summary The treatment of injuries to the skull, the chest and/or the abdominal cavity has the priority in the handling of the multiply traumatized patient. In multiple fractures, especially with multiple fracture segments, operative treatment is advised. Suggestions are made as to the timing in the treatment of coexisting open and/or closed fractures and to the degree of stability which can be achieved by the various osteosynthetic procedures. Indications, contraindications and the requirements necessary for operative fracture treatment as well as its complications are discussed. The sequence of treatment is such that, if possible, all the injuries of one extremity are treated in one operative stage in order to obtain early motion stability. Specific possibilities of the operative fracture management in adults for cylindrical bones, save the foot and hand, are demonstrated in diagrams. Finally, data on our own material (540 fractures of long cylindrical bones) are analysed.

    Description / Table of Contents: Zusammenfassung Aufgrund der Ergebnisse moderner Genetik ist die Funktion knochenbildender Zellen eine direkte Folge der auf sie einwirkenden Reize oder Signale. Aus den Gesetzmäßigkeiten der cellulären Funktionen läßt sich die Entstehung der verschiedenen Pseudarthroseformen ableiten. Nach diesem Grundplan werden zunächst die einzelnen Stadien der normalen Knochenbruchheilung besprochen. Wird dieser normale Heilungsvorgang gestört, so wechseln die Zellen als Reaktion auf die Störung ihr Syntheseprogramm und bilden Faser- oder Knorpelgewebe. Erst nach Abbau dieses Ersatzgerüstes kann die endgültige knöcherne Verspannung der Bruchenden erfolgen. Die Entstehungsbedingungen für Faser- oder Knorpelgewebe werden ausführlich besprochen, weil sie die Entwicklung einer Pseudarthrose einleiten können. Dieser Vorgang wird unterstützt durch Faktoren, welche die Zellproliferation hemmen. Für die definitive Ausbildung einer Pseudarthrose sind Art und Ausmaß mechanischer Störfaktoren verantwortlich, welche die Zellen treffen. Sie werden an Hand praktischer Beispiele aus dem eigenen Krankengut ausführlich analysiert.
    Notes: Summary According to the data of modern genetics, the function of osteogenetic cells is a direct response to a stimulus or a signal, which influences them. The origin of the most varied types of pseudoarthrosis can be worked out from the principles of cellular function. Following this basis pattern, initially the separate stages of normal bone-fracture healing are discussed. If this normal healing process is disturbed, the cells change their synthesis-programme as a reaction to the disturbance and start to create fibrous or cartilage tissue. The final bony bridging of the fracture ends can only occur, when this provisional structure has been broken down. The circumstances, in which fibrous and cartilage tissue develop are extensively discussed, since they may be the precursors of a pseudoarthrosis. This process is supported by factors, which inhibit cell-proliferation. The nature and extent of mechanical disturbancefactors, which affect the cells, govern the definitive formation of the pseudoarthrosis. These are subjected to an extensive analysis, using examples from our group of patients.

General descriptions of injuries to the soft tissue are followed by a critical discussion of the possibilities of closed and open methods of internal fixation with some examples.

    Description / Table of Contents: Zusammenfassung Offene Sehnenverletzungen der Hand verdienen unsere besondere Beachtung, weil die Art der Erstversorgung über die spätere Funktion entscheidet. Der Erfahrene wird bei günstigen Wundverhältnissen möglichst alle ver letzten Strukturen primär versorgen. Sind dafür aber die Voraussetzungen nicht erfüllt, so führt ein primärer Sehnennahtversuch oft zur Wundinfektion mit ausgedehnter Vernarbung und irreparabler Funktionseinbuße. Wer daher bei einer frischen offenen Sehnenverletzung, insbesondere über der Beugeseite der Finger, sich auf die Wundtoilette mit dem Ziel der Primärversorgung beschränkt, um nach glatter Wundheilung 3–4 Wochen später die Wiederherstellungsoperation unter optimalen Bedingungen vornehmen zu lassen, geht das kleinere Risiko ein. Nach den Zoneneinteilungen von Verdan werden die Behandlungsmöglichkeiten verletzter Sehnen auf der Streck- und Beugeseite besprochen.
    Notes: Summary Open tendon injuries of the hand are claiming our most attention, first treatment deciding the later function. If wound conditions are favourable, the expert will try to treat all injured structures primarely. But if necessary suppositions are missed, the attempt of primary tendon suture often results in wound infection with large cicatrization and irreparable function lost. The minor risk in a recent open tendon lesion particularely on the palmar face of the fingers is to perform only primary wound toilet. So, after primary healing of the wound, 3–4 weeks later reconstruction will be performed under optimal conditions. After Verdan's classification in zones the possibilities of treatment of injuried tendons on dorsal and palmar face are discussed.
