    Notes: Special and claw meat products from the blue crab produced in the commercial processing plants in North Carolina had relatively poor shelf life. These products had high initial total anaerobe and psychrotroph counts. Many of the samples were positive for presumptive Vibrio parahaemolyticus, even though many samples did not have coliforms (in 0.2g sample). The presence of presumptive V. parahaemolyticus and high numbers of aerobic plate counts, total anaerobes and psycbrotrophs in these samples was probably from improper cooking of the raw crabs and post-heat-treatment contamination of the cooked crabs. The results suggested that coliforms may not be suitable to indicate proper plant sanitation and product contamination. Crab meat produced under sanitary conditions had lower numbers of total anaerobes and psychrotrophs; these products had relatively long shelf life. The results of these studies indicated that to produce products having longer shelf life the commercial plants will have to adopt proper sanitary practices. The main objective will be to prevent post-heat-treatment contamination to reduce the initial bacterial load in the products, especially with the anaerobes and psychrotrophs.

    Description / Table of Contents: Résumé. Cet article rapporte les détails techniques et les résultats cliniques de douze cas d’infection profonde de la hanche traités par révision chirurgicale en deux temps et utilisant un fantôme en ciment chargé de gentamycine et modeléà la main, mis en place temporairement dans l’intervalle entre la résection de la prothèse infectée et la réimplantation d’une nouvelle. Pendant ce traitement (4 mois) les patients furent pratiquement indolores, mobilisés et ayant bon contrôle du membre, et purent demeurer pendant les 2/3 du temps de traitement à domicile. Six des 12 fantômes échouèrent localement, ceci du à une luxation ou à une fracture du ciment, nécessitant une troisième intervention dans 3 cas. Une luxation du fantôme était due à un manque d’offset tête-col (P〈0,05) ou à un ancrage insuffisant dans le fût fémoral. Par contre aucune des prothèses ne s’infectèrent aprés la réimplantation. Sur la base des ces résultats nous proposons le concept du fantôme en ciment temporaire pour améliorer sur le point biologique aussi bien que mècanique le traitement des infections de hanche.
    Notes: Summary. We report the technical details and clinical results of twelve patients who had deep infections of implants in the hip joint and were treated by two-stage revision, using a gentamicin-loaded, hand-moulded cement spacer inserted for the period between resection and reimplantation arthroplasty. During management with the spacer, usually for 4 months, patients were almost free of pain and mobile with good leg control, spending 2/3 of the treatment period at home. Six of twelve spacers failed locally due to dislocation [5] or cement fracture [1], and more than two further episodes of surgery were required in 3 patients. Problems with dislocation of the spacer were significantly higher when the head to neck offset was lacking (P〈0.05) or when anchorage in the femoral shaft was poor. Nevertheless, infection after reimplantation arthroplasty did not occur by the time of follow-up (2.2 years). Based on these data, we consider that the use of the cement spacer is a promising approach to the treatment of complicated infections of the hip joint.

    Notes: Zusammenfassung Die bactericide Wirkung des Hautfettes wird durch ein Gemisch an der Oberfläche der Haut nachgewiesener freier Aminosäuren erheblich gehemmt. Die hemmende Wirkung ist in erster Linie bedingt durch die basischen, in geringerem Grade durch die neutralen Aminosäuren, während die sauren selber bactericide Eigenschaften besitzen und keine Hemmwirkung erkennen lassen. Wasserzusatz zu dem HF-AS-Gemisch hat nur geringen oder keinen Einfluß. Die einzelnen Aminosäuren einer Gruppe verhalten sich nicht gleich. Dies zeigt sich besonders bei den basischen Aminosäuren, wo z. B. Arginin und Histidin stark hemmen, während Ornithin ohne Einfluß ist.

    Notes: Zusammenfassend kann gesagt werden, daß die modernen immunologischen Untersuchungsmethoden sehr interessante Änderungen einiger Serum-Faktoren gezeigt haben, die einerseits, wie Erhöhung von IgG und IgE zum Ausfall der Hautteste, Beteiligung der Respirationstraktus und Schweregrade des Verlaufs in Beziehung stehen, anderseits von der Erkrankung des Respirationssystems unabhängig sind, wie Erhöhung des Coeruloplasmins, Erniedrigung des Haptoglobins, Erniedrigung des Komplementfaktors Beta 1-A (C′3), sowie des Alpha 1-Antitrypsins. Diese Veränderungen können einstweilen lediglich vermerkt, und in ihrer immunologischen Bedeutung noch nicht interpretiert werden. Möglicherweise sind sie Ausdruck einer polyfaktoriellen genetischen Störung. Die verschiedenen Faktoren des neuro-vegetativen-zirkulatorischen Systems, die sicher eine große Rolle in der Pathogenese der Atopie und der Neurodermitis spielen (Storck) sprechen nicht unbedingt genen eine immunpathologische Genese der Neurodermitis. Dank der quantitativen Bestimmung der IgE müssen jedoch die Argumente, die gegen die Rolle dieser Mediatoren jetzt immer noch aufgeführt werden, wie Vorkommen von Neurodermitis-ähnlichen Ausschlägen bei Agammaglobulinämie und beim Wiscot-Aldrich-Syndrom, fallen gelassen werden, da auch bei Patienten mit solchen immunologischen Defekten die Bildung von IgE möglich oder gar gesteigert ist. Für die beliebte Ausweichhypothese eines autoallergischen Mechanismus bei der konstitutionellen Neurodermitis liegen bis dahin keine schlüssigen Beweise vor.

    Notes: Summary The 24 h urinary excretion of 6β-hydroxycortisol and D-glucaric acid, the plasma half-lives and total clearances of aminopyrine, and serum gamma-glutamyl-transpeptidase activity have been measured in nineteen healthy male volunteers. The study was done double blind and was conducted as a test of induction of microsomal drug metabolizing enzymes during and after daily doses of 6 mg clemastine, 300 mg phenobarbital or a placebo. The urinary excretion of 6β-hydroxycortisol and D-glucaric acid was significantly increased in the phenobarbital group, the standard for induction. No changes were observed after treatment with clemastine or placebo. Phenobarbital also reduced the half life of aminopyrine, but it was not affected by clemastine or placebo. Gamma-glutamyl-transpeptidase activity increased only in the phenobarbital group. The elimination constant k2 of aminopyrine and the excretion of glucaric acid in the pre-medication period were correlated (p〈0.05). The results indicate that the tests were of diagnostic value in determination of microsomal enzyme induction by phenobarbital. Failure to observe similar changes after treatment with clemastine imply failure of induction of this activity under the experimental conditions.
