    Notes: The aim of the study was to characterize the acoustics of vowel articulation in maxillectomy patients. Digital acoustic analysis of five vowels, /a/, /e/, /i/, /o/ and /u/, was performed on 12 male maxillectomy patients and 12 normal male individuals. A simple set of acoustic descriptions called the first and second formant frequencies, F1 and F2, were employed and calculated based on linear predictive coding. The maxillectomy patients had a significantly lower F2 for all five vowels and a significantly higher F1 for only /i/ vowel. From the data plotted on an F1–F2 plane in each subject, we determined the F1 range and the F2 range, which are the differences between the minimum and the maximum frequencies among the five vowels. The maxillectomy patients had a significantly narrower F2 range than the normal controls. In contrast, there was no significant difference in the F1 range. These results suggest that the maxillectomy patients had difficulty in controlling F2 properly. In addition, the speech intelligibility (SI) test was performed to verify the results of this new frequency range method. A high correlation between the F2 range and the score of SI test was demonstrated, suggesting that the F2 range is effective in evaluating the speech ability of maxillectomy patients.

    Notes: summary  Rehabilitation of patients who have undergone bilateral maxillectomy is difficult because of extensive loss of bone and soft tissue. In this clinical report, prosthodontic rehabilitation of oral function in a bilateral maxillecitomy patient combined with a new fibular osteocutaneous flap, which was designed to have two oronasal slits for the retention of an obturator prosthesis, was described. A 58-year-old man with a maxillary alveolar carcinoma underwent bilateral maxillectomy. The defect was reconstructed using a vascularized fibular bone wrapped circumferentially with a peroneal flap, which was fixed with miniplates between the right malar prominence and cut edge of the left zygoma remaining two slits anterior and posterior to the graft. Two and half weeks after the surgery, a delayed surgical obturator was delivered and an obturator prosthesis was delivered 6 weeks after the surgery. This obturator prosthesis could be extended into the slits to engage the tissue undercuts, and was stable during use. Mastication, deglutition, articulation and the mid-facial profile of the patient were rehabilitated. After installation of the obturator prosthesis, relining of the prosthesis base was carried out alongside the healing process of the graft, and adjustment of occlusions and high-pressure spots was carried out. No clinical disorders were observed either in the grafted tissue or the obturator prosthesis with a 3-year prognosis. Newly designing a fibular osteocutaneous flap combined with tissue-borne obturator prosthesis is one successful approach to the restoration of oral function, and increases the patient's quality of life after bilateral maxillectomy.

    Notes: Abstract Accumulation of tetracycline (Tc) into Bacillus subtilis was studied by two methods, one involving a fluorescence assay and the other an absolute determination of accumulated drug. B. subtilis GSY908 harboring B. subtilis plasmid pNS1981 and the plasmidless host strain accumulated equivalent amounts of Tc. Prior exposure of the plasmid-harboring cells to subinhibitory concentration of Tc resulted in marked decrease in accumulation of the drug, indicating that pNS1981-determined Tc resistance is inducible. Of interest was the fact that the amount of Tc accumulated by the Tc-induced plasmid-harboring cells is increased somewhat by the addition of carboxylcyanide-m-chlorophenyl hydrazone (CCCP), an uncoupling reagent. This seems to show that energy-dependent accelerated Tc efflux is involved in decreased Tc accumulation.

    Notes: Abstract The in vitro activity of the new carbapenem SM-7338 was tested in comparison with imipenem, ceftazidime, cefotaxime, flomoxef, cefuzonam and cefmetazole against 2850 clinical bacterial isolates. SM-7338 showed good activity against a broad spectrum of grampositive and gram-negative bacteria. SM-7338 was very active against gram-negative bacteria, inhibiting allEnterobacteriaceae, except 25 % ofSerratia marcescens isolates, at a concentration of 0.78 mg/l. SM-7338 inhibited the majority ofPseudomonas spp. at concentrations of ⩽ 3.13 mg/l, its activity being twofold higher than that of imipenem. However, the activity of SM-7338 against gram-positive cocci was about one-fourth that of imipenem. Against anaerobes, SM-7338 also had the best activity of the β-lactams tested. The compound was inactive against methicillin-resistant staphylococci,Enterococcus faecium, Xanthomonas maltophilia andFlavobacterium spp., as were the other β-lactams. SM-7338 was quite stable in the presence of various types of β-lactamase, but was hydrolyzed byXanthomonas maltophilia β-lactamase, as was imipenem. This high degree of stability was responsible for the potent activity of SM-7388 against β-lactamase-producing species such asEnterobacter cloacae, Citrobacter freundii andProteus vulgaris. SM-7338 also showed bactericidal activity against clinical isolates at the MIC or at concentrations slightly above the MIC.

    Notes: Abstract The in vitro activity of meropenem, a carbapenem antibiotic, combined with eight other β-lactams against methicillin-resistantStaphylococcus aureus (MRSA) was tested. The MICs of these antibiotics alone ranged from 12.5 to 1,600 µg/ml for the 25 clinical isolates of MRSA studied. All combinations with meropenem exhibited marked synergy as determined by the checkerboard method. The MICs in combinations of meropenem with other β-lactams were reduced to 1/4 – 1/64 those of the antibiotics alone. No antagonism was observed for any of the combinations of meropenem with other β-lactams. Synergism between meropenem and cefpiramide was the highest among the combinations tested, the geometric mean of the fractional inhibitory concentration index for this combination being 0.237. This combination also demonstrated strong bactericidal activity, the MBCs decreasing to 1/16 – 1/64 of those for the agents alone. In terms of the fractional inhibitory concentration index, this was the most effective combination against MRSA highly resistant to meropenem alone with synergism for 85 % (81/95) of the strains. In addition, synergism of imipenem with cephalosporins against MRSA was demonstrated. The affinity of meropenem and cefpiramide for MRSA penicillin-binding protein (PBP) 2′ was very low, and the combination of both antibiotics showed an additive increase in affinity for this protein, but not a synergistic increase. Thus, the mechanism of synergism did not seem to be related to affinity for PBP2′. It is possible that there is another factor besides PBP2′ which increases the resistance of MRSA and that the combinations inhibit the unknown factor.
