    Notes: Abstract We describe herein a rare case of primary malignant melanoma of the rectum in an 85-year-old woman. The patient presented with intermittent rectal bleeding, and a colonoscopy revealed an ulcerated polypoid mass in the rectum, located 5 cm from the anal verge. The lesion was histologically characterized by solid growths of small round cells with scanty cytoplasm and prominent nucleoli. Although no melanin pigment was found in the tumor cells, they were immunohistochemically positive for HMB-45, a monoclonal antibody highly specific for malignant melanoma. Thus, HMB-45 proved very useful to establish a diagnosis of amelanotic malignant melanoma of the rectum.

    Notes: Abstract Considering the current improved resectability of advanced gallbladder cancers, differentiation from benign gallbladder lesions mimicking advanced cancer is now a significant problem to be overcome so that unnecessary extended operations can be avoided. During the 4 years from January 1989 to December 1992, we experienced 63 patients with elevated lesions of the gallbladder, including cancer. Among the 59 patients who underwent resection, preoperative imaging diagnostic procedures were suggestive of cancer in 27. In particular, the lesions of 14 patients appeared to be cancerous with hepatic infiltration. Frozen section histological examinations were performed on the specimens of 4 of these patients in whom the findings were inconsistent; all of them were shown non-cancerous. The final diagnoses in these 4 patients were: hyperplastic polyp with foreign body granuloma, xanthofibromatous granulation, localized type adenomyomatosis, and chronic cholecystitis. Resections were performed on the remaining 10 patients who had lesions suspected to be cancerous with infiltration to the liver. Nine lesions were confirmed as cancer by histological examination of the resected specimen. However, the lesion of the remaining patient, who underwent hepatopancreatoduodenectomy, proved after operation, to be a hamartoma with foreign body granuloma. Of the imaging diagnostic procedures, a combination of endoscopic or intraoperative ultrasonography and computerized tomography was most valuable for making the differential diagnosis between gallbladder cancer and benign gallbladder lesions mimicking cancer infiltrating the liver. However, frozen section histological examination should not be omitted before the extended operations are performed, even though en bloc resection is preferable because the procedures used to obtain the specimens have the potential to disseminate viable cancer cells.

    Notes: Abstract Two distinct epithelial lesions (dysplasia and adenoma) and one high-risk condition (anomalous pancreatico-biliary ductal junction; APBDJ) are currently recognized as premalignant stages of gallbladder carcinogenesis. In addition to clinicopathologic observations, recent molecular genetic studies have provided further insight into the pathogenesis and biological behavior of these precursor lesions. In this review, we concentrate on describing the histopathologic and clinicopathologic background and recent molecular genetic findings for each of these putative precursor lesions.

    Notes: Abstract The production of a tumor growth inhibitory factor (TGIF) was induced in human peripheral blood mononuclear cells (PBMC) by a streptococcal preparation, OK-432,in vitro. The antitumor effect of locally injecting PBMC treated with OK-432 into the tumor site was studied. PBMC were collected from patients with gastric cancer 5 to 12 days before their operation, and cultured with OK-432 for 24 hrin vitro. After the culture, the PBMC were washed thoroughly to eliminate the OK-432. The washed PBMC went on producing TGIF for more than 72 hrin vitro in the absence of OK-432. A small number of TGIF-producing PBMC, approximately 107 cells, were injected around the lesion under endoscopic observation. A remarkable antitumor effect was observed in 2 out of 10 cases of resectable gastric cancer. Histological examinations indicated that the antitumor effect is due to antitumor cytokines such as TGIF produced by PBMC rather than to the OK-432-activated PBMC themselves.

    Notes: Abstract. Phlegmonous colitis (PC) is an acute infectious entity caused by bacteria. In this study, we reviewed 8822 autopsy cases and found 13 cases of PC (0.15%). PC affected 2.43% of patients with hepatic cirrhosis or subacute liver atrophy, both of which were considered to be due to hepatitis viral infection. Before autopsy, none of the cases studied was suspected to involve PC, irrespective of the immediate cause of patient death. Thirteen autopsy cases showed some or all of the following pathohistologic characteristics: (1) involvement of the cecum (9 cases, 76.9%), (2) phlegmonous inflammatory changes and edema in the submucosa (100%), (3) bacterial infection (100%), (4) no microscopically detectable mucosal injuries (12 cases, 92.3%), and (5) acute serositis (peritonitis) (2 cases, 15.4%). These results suggest that PC is an unrecognized, but fatal complication of patients with some hepatic diseases and that PC has pathohistologic characteristics in common with previously reported spontaneous bacterial peritonitis in animal models. PC probably arises due to spontaneous infection in patients with hepatic cirrhosis.

    Notes: Abstract  Expression of c-Met, a gene for the hepatocyte growth factor/scatter factor (HGF/SF) receptor, is known to be associated with tumour development in several human carcinomas. The expression of c-Met was examined using immunohistochemistry in 82 cases of primary laryngeal carcinoma to evaluate the tissue distribution of c-Met and the clinicopathological significance of c-Met expression. In normal larynx, c-Met expression was observed only in some minor salivary glands. Positive reaction for c-Met in neoplastic epithelium was noted in 45 out of 82 (54.9%) cases. In 44 cases, structures adjacent to the carcinoma (noncancerous squamous epithelium, some stromal fibroblastic cells, and endothelial cells) showed positive reaction for c-Met. c-Met expression in cancerous epithelium was significantly correlated with lymph node status (P〈0.04) and proliferating activity expressed by the Ki-67 labelling index (P〈0.02). There was no correlation between c-Met expression and age, sex, histological type, T category, distant metastasis or clinical stage. The data suggest that overexpression of c-Met in laryngeal carcinomas represents a growth advantage for cancer cells, which may be conferred by the mitogenic effect of HGF/SF. Simultaneous c-Met expression in noncancerous components of the larynx may represent a paracrine modification of c-Met.

    Notes: Summary The present study was undertaken to investigate histological changes in aortocoronary saphenous vein grafts (SVGs) and the relationship between intimal thickening of the SVGs and the interval after grafting. The SVGs were divided into five groups according to the degree of intimal thickening and associated luminal narrowing: minimal thickening (0%–10% stenosis), slight thickening (11%–25% stenosis), slight-to-moderate thickening (26%–50% stenosis), moderate thickening (51%–75% stenosis), and severe thickening (76%–100% stenosis). SVGs showing minimal thickening had been implanted for 0–3 weeks, those with slight thickening for 2–13 weeks, those with slight-to-moderate thickening for 5–13 weeks, those with moderate thickening for 30–52 weeks, and those with severe thickening for 30–83 weeks. Thickening of the intima in SVGs (intimal hyperplasia) was time-dependent, and began as early as 2 weeks after the graft surgery. The change was diffuse and concentric, and observed from an aortic root to a coronary site. The major cell type involved in the intimal hyperplasia was the smooth muscle cell.

    Notes: We examined mortality from adult T-cell leukemia/lymphoma (ATL/ATLL) and other diseases alleged to be associated with human T-lymphotropic virus type I (HTLV-I) among anti-HTLV-I antibody-positive blood donots in Kyushu, Japan. During 1984–87, a total of 3,991 blood donors aged 40 years or over were followed from the date of donation to the date of death or the end of the study. Crude mortality rates from ATL (with 95 percent confidence intervals) were 68 per 100,000 (13–202) for males and 36 per 100,000 (3–132) for females. The rates were underestimated by approximately 50 percent because of self-selection and short observation periods. Neither death rates from other cancers not death rates from all cancers were elevated.
