    Notizen: Bearing the sensory nerve end organ, the skin establishes contact to our environment. So far, the analysis of the cutaneous nervous system was dependent on the use of tissue serial sections. Because such samples inherently allow visualization of only a small part of the mainly horizontally oriented nervous system of the skin, we searched for possibilities enabling a more comprehensive view. Here, we present a method based on the immunostaining of dermal sheet preparations for subsequent analysis by electron microscopy and light, or laser scanning microscopy. We used antibodies against PgP9.5 and NCAM/CD56, both showing a regular network of fibers covering the entire superficial dermis. The bulk of free ending nerve fibers ramified within 25 µm of the dermo–epidermal junction, whereas below that only larger nerve bundles were present. Along the course of nerve fibers, we observed NCAM/CD56+ protrusions with diameters ranging from 5 to 15 µm. We further characterized these protrusions demonstrating the ultrastructural features of terminal non-myelinating Schwann cells ensheathing nerve fibres. Depending on the body region, we detected between 140 and over 300 individual terminal Schwann cells/mm2 skin surface. In a double staining for NCAM/CD56 and vWF, we analyzed the topographical relationship of the nerve end organ to the blood vessels of the skin. In conclusion, this novel method allows for the first time a complex three-dimensional depiction of the cutaneous nervous system over several cm2. Additionally, terminal Schwann cells can be studied in detail in situ for the first time. Furthermore, application of this method may provide new impetus in the investigation of the cutaneous nerve end organ under physiological and pathological conditions.

    Notizen: Tumor microvessel density (TMVD) has been recognized as an important indicator for the metastatic risk in certain tumors. The purpose of this study was to analyse whether there is an association of TMVD in epithelial neoplasms of the skin with their clinical behavior. Paraffin sections of keratoacanthomas (KA, n=10), squamous cell carcinomas (SCC, n=9), nodular (nod-BCC, n=13), and sclerosing (scl-BCC, n=12) basal cell carcinomas were immunohistochemically stained for factor-VIII-related antigen and TMVD was determined. In all SCC, KA and nod-BCC, TMVD significantly exceeded perilesional skin microvessel density (PSMVD) (SCC:TMVD/PSMVD = 20.54:11.25, p 〈 0.0001; KA:TMVD/PSMVD = 20.90:12.17, p 〈 0.0001; nod-BCC:TMVD/PSMVD = 16.77:13.34, p=0.03). In contrast, no significant difference between TMVD and PSMVD was found in scl-BCC (15.44:12.86, p=0.22). TMVD was significantly higher in SCC and KA compared to nod-BCC (p=0.036 and 0.006, respectively). Our data demonstrate that SCC and KA are highly vascularized tumors. The fact that TMVD does not differ significantly between SCC and KA (p=0.80) suggests that MVD is not an indicator for the metastatic risk or aggressive growth behavior of epithelial skin tumors. The finding that MVD in both nod- and scl-BCC is significantly lower than in SCC and KA, might at least in part explain the slow growth of BCC.

    Notizen: We report what we believe to be a novel skin manifestation of dioxin intoxication. A 30-year-old woman with 2,3,7,8-tetrachlorodibenzo-p-dioxin levels of 144,000 pg g−1 blood fat presented with severe chloracne that affected the entire integument. She also exhibited acral granuloma annulare-like lesions and distal onycholysis and, at a later time point, showed signs of hypertrichosis, as well as brownish-grey hyperpigmentation of the face. In addition, she developed punctate keratoderma-like lesions on the palms and soles. These lesions were negative for human papillomavirus and histologically characterized by cone-shaped hyperkeratoses invaginating, but not penetrating, into the dermis. Squamous syringometaplasia of the eccrine glands was observed in the immediate vicinity of these lesions. Both clinically and histologically these alterations are essentially indistinguishable from what is described as keratosis punctata palmaris et plantaris (KPPP). Although a fortuitous coincidence of chloracne and KPPP cannot be formally excluded, the possibility exists that in our patient toxic levels of dioxin were causally involved in this disorder of keratinization.

    Notizen: CD40, a member of the nerve growth factor/tumour necrosis factor-receptor family, is expressed on endothelial cells in situ and in tissue culture. It is functionally involved in the regulation of the expression of adhesion molecules in these cells. In the current study we investigated the expression of CD40 on the endothelia of normal fetal and adult human tissues, as well as in the stroma of various neoplasms and in different inflammatory tissue reactions. By immunohistochemistry, endothelia were negative or only weakly positive for CD40 in all normal adult and most fetal organs studied. In contrast, endothelial cells of fetal and embryonic lungs, kidneys and larger vessels consistently expressed the CD40 antigen. Strong expression of CD40 on endothelial cells of blood vessels but not of lymphatic vessels was found in inflammatory reactions. In 65 tumours of vascular origin we found moderate to strong expression of CD40 in all of four juvenile proliferating capillary haemangiomas and 11 lobular capillary haemangiomas of adults. Senile and cavernous haemangiomas as well as lymphangiomas were negative or exhibited only weak CD40 reactivity. Weak to moderate CD40 expression was found in five of eight malignant neoplasms of endothelial origin. Our finding that the CD40 antigen is present on only a minority of normal endothelial cells, whereas strong staining can be detected on the majority of endothelial cells in inflammatory processes, complements previous reports which suggest that this surface receptor plays an important role in tissue inflammation. The heterogeneous pattern of expression in malignant endothelial neoplasms argues against a major role for CD40 in the pathogenesis of these tumours. Its consistent presence in juvenile and lobular capillary haemangiomas suggests rather the involvement of CD40 in the development and/or progression of these benign vascular proliferations.

    Notizen: Background After exposure, 2,3,7,8-tetrachlorodibenzo-p-dioxin (TCDD) is excreted via the faeces, breast milk and epidermal lipids. Objectives To determine to what extent TCDD is eliminated via the skin and to evaluate whether cutaneous elimination can be accelerated by the application of petrolatum. Methods In two patients severely intoxicated with TCDD, material obtained from the skin surface and, in one patient, cerumen and the content of epithelial cysts, was analysed for TCDD. Results The TCDD concentration in the initial blood sample taken was 144 000 pg g−1 blood fat in patient 1, and 26 000 pg g−1 blood fat in patient 2. Six months later, when the skin tests were performed, the blood TCDD levels had decreased to 80 900 and 16 100 pg g−1 blood fat, respectively. In the two samples of pooled cyst contents from patient 1, TCDD levels of 34 400 and 18 600 pg g−1 fat were found. A cerumen sample contained TCDD at 20 500 pg g−1 fat. In the material collected from the skin surface we observed a linear increase of the amount of TCDD measured per test field with time, indicating a continuous elimination of TCDD via the skin. The daily amount of TCDD eliminated via the skin was 1·51 pg cm−2 in patient 1 and 0·57 pg cm−2 in patient 2. Application of petrolatum led to a twofold increase in the amount of TCDD measured in patient 1, but had no significant effect in patient 2. Conclusions In our patients, elimination of TCDD via the skin, most probably through desquamating scales, represented 1–2% of the overall daily TCDD elimination rate, with regard to the body surface and when calculated on the basis of the half-life of TCDD at the time of the skin test. If a more typical overall elimination half-life of 7 years is used as the basis for the calculation, the skin would account for 9%(patient 1) and 15%(patient 2) of the overall elimination. Although we observed an increase in TCDD in material derived from the skin surface of up to 100% after application of petrolatum in patient 1, such an approach appears not to be a feasible means to increase elimination. Owing to the small amount of TCDD measured in skin-surface material, as well as in the cyst contents and cerumen obtained from one patient, contamination of the environment and other persons appears highly unlikely.

    Notizen: Background Alpha-hydroxy acids (AHAs) are widely used for the treatment of hyperkeratotic skin disorders and photodamaged skin. Objectives To investigate the effect of lactic acid (LA) on the secretion of cytokines by keratinocytes (KCs) of human reconstructed epidermis. Methods Creams containing 1·5%, 3% or 5% LA or vehicle controls were topically applied on to human epidermal equivalents (EEs). After 24 h, EEs were analysed for morphology and for the presence of apoptotic cells. Secretion of vascular endothelial growth factor (VEGF), angiogenin (ANG) and interleukin (IL)-8 was measured in the supernatants by enzyme-linked immunosorbent assay. Results LA led to a concentration-dependent increase in apoptotic cells as determined by cell morphology and terminal deoxynucleotidyl transferase-mediated deoxyuridine triphosphate nick end-labelling. VEGF secretion was increased 2·5- and 2·8-fold (P 〈 0·05) over vehicle control after treatment with 1·5% and 3% LA, respectively. No significant increase in VEGF secretion was detected with 5% LA. In contrast to VEGF, secretion of ANG was decreased by LA in a concentration-dependent manner (0·5-fold for 5% LA; P 〈 0·01). No significant changes in IL-8 secretion were found with any of the concentrations tested. Conclusions Our data demonstrate that the topical application of AHAs modulates the secretion of cytokines by KCs. Regulation of KC-derived growth factors and cytokines by AHAs might represent a mechanism contributing to their therapeutic effects in disorders such as photoageing.

    Notizen: Abstract  Prion diseases or transmissible spongiform encephalopathies have been shown to be communicated by oral ingestion of the infectious agent. However, the exact route of transmission is still unknown. In order to better understand the pathophysiology of these diseases, it is crucial to identify cell types of peripheral tissues in which the infectious agent may propagate. Since expression of cellular prion protein (PrPc) is a prerequisite for prion replication, we determined the expression of PrPc in the mucosa of the gastrointestinal tract using immunohistochemistry. Expression of PrPc was negative or weak in the neck region of the gastric mucosa and moderate to strong in crypts of both the small and the large bowel. PrPc was found to be upregulated in the mucosa of patients with Helicobacter pylori gastritis. In contrast, PrPc staining appeared to be downregulated in patients with inflammatory disorders of the large bowel and it remained moderate to strong in inflammatory disorders of the small bowel. Our results support the notion that epithelial cells of the gastrointestinal tract may represent a possible target for prion entry and replication.
