    Notes: Summary The regeneration of transected peripheral nerves of mice was studied using autoradiographical and electron microscopical techniques. In general, maximal proliferation occurred between the 5th and 7th day after dissection and stopped when the cells emigrating from the proximal and distal stumps of the nerve started to contact one another. Special attention was paid to the reaction of the connective tissue cells of the endo-, epi- and peri-neurium. The perineurial cells seemed to dedifferentiate between the 3rd and 5th day after the transection and then started to proliferate into the defect. Labelled perineurial cells were completely absent, when the minifascicles were fully developed in the neuroma. The epineurial fibroblasts started to proliferate during the 1st day. Even 6 weeks after transection the multiplication rate was about ten fold that of the controls. The results are discussed with special reference to clinical nerve repair.

    Notes: Summary Myelin degradation in Wallerian degeneration of rat femoral nerves has been studied with the electron microscope. In the initial stages, a decrease of myelin periodicity from 115 Å to 88 Å was noted, followed by the transformation of the myelin structure into uniformly layered lipid inclusions. 10–14 days after nerve section, most of the inclusions found represented unstructured lipid droplets or crystals. In the later stages of degeneration, numerous pleomorphic lamellar inclusions were found, some of them resembling the structure of π- and μ-granules. Lysosomal enzyme activity was found especially in pleomorphic inclusions during the late stages of myelin degradation. Normal myelin sheaths, as well as unstructured lipid droplets and crystals, were devoid of enzyme activity. The results are compared with alterations found in Wallerian degeneration of the central nervous system.

    Notes: We present a patient who developed skin lesions typical of disseminated superficial porokeratosis (DSP) while on immunosuppressive therapy for pemphigus foliaceus. Phototesting with artificial light sources did not have any effect on the DSP lesions. The literature describing occurrence of DSP on immunosuppression is reviewed and possible pathogenetic mechanisms are discussed.

    Notes: Five patients with typical bullous herpes gestationis and five patients with polymorphic eruption of pregnancy were studied. Shave biopsies were taken from both involved and uninvolved skin and examined by routine immunofluorescence. They were also examined by immunoelectron microscopy employing a multistep peroxidase-antiperoxidase technique which has been shown to be more sensitive than immunofluorescence. Despite the increased sensitivity of immunoelectron microscopy all cases of polymorphic eruption of pregnancy were completely negative. Although there is a considerable clinical and histopathological overlap between herpes gestationis and polymorphic eruption of pregnancy our findings indicate that these conditions are pathogenetically distinct and should be classified as separate disorders.

    Notes: We present an unusual case of linear focal elastosis occurring exclusively on the legs of a 13-year-old girl. An increase of elastic fibres (EF) was demonstrated histologically, and the number of EF in lesional and normal skin was quantified using a video measuring system. EF were found to be increased by about 100% in lesional skin compared with unaffected skin. EF were elongated, thinned and split at their ends with a paintbrush formation.     Notes: In a Study of twenty-five patients with herpes gestationis we found that 80% possessed the HLA antigen DR3, which confers increased immune responsiveness and a predisposition to ‘auto-immune disease’. In five patients the development of herpes gestationis coincided with a change in sexual partner, suggesting that the development of herpes gestationis may depend on exposure to an antigen derived from the father. This might share determinants with a component of the basement membrane zone of skin. Although anti-basement membrane zone antibodies are present in HG it is not clear whether they play a pathogenic role. The infrequency of neonatal involvement and the lack of correlation between immunofluorescence findings and clinical activity in our patients suggested that the antibodies might be a result of tissue damage rather than its cause. Two patients In our study were exceptional in that episodes of herpes gestationis were followed by normal pregnancies. In these patients the relationship of their DR antigens to those of the fetus may have been important in determining whether or not the pregnancy would be affected by herpes gestationis.

    Notes: We have compared the histopathological findings in twenty-five patients with polymorphic eruption of pregnancy and eighteen patients with herpes gestationis. The histopathology of polymorphic eruption of pregnancy varied with the clinical stage of the eruption. In the early lesions there was epidermal and upper dermal oedema with a perivascular lymphohistiocytic infiltrate. The fully developed lesions demonstrated vesicular spongiosis, and, in one case, suhepidermal vesicle formation. Eosinophils were frequently present in the perivascular infiltrate and in two cases there was eosinophilic spongiosis. In the resolving stage of the eruption there was acanthosis and foci of parakeratosis. These histopathological features were indistinguishable from those of the urticarial lesions of herpes gestationis. It was only in the presence of large bullac that the two disorders could he differentiated histopathologically. This finding emphasizes the importance of immunonuorescence studies in the investigation of urticarial eruptions occurring during pregnancy.

    Notes: We report a cutaneous infection by Aspergillus flavus in a tetraplegic but otherwise healthy young man. He presented with multiple erythematous to violaceous indurated papules and plaques which had progressed to central ulcers or black eschars. Lesional scrapings, biopsies and cultures demonstrated a fungus, Aspergillus flavus, as the aetiological agent. As the patient had to leave for his home country before diagnosis had been established, we cannot report on the results of therapy in this case.
