Zusammenfassung Die Untersuchung der Lungen von 48 Leichen, bei denen vor dem Tode eine künstliche Beatmung erfolgt war, ergab in wechselnder Ausprägung folgende charakteristische Veränderungen: Makroskopisch eine diffuse, fleischige Verdichtung bei vermehrtem Flüssigkeitsgehalt des Gewebes; mikroskopisch u.a. Atelektasen, „Alveolarzellkatarrh”, Blutungen, fibrinöses Exsudat, hyaline Membranen, Alveolarzellhyperplasien, mehrkernige Riesenzellen und verdickte, infiltrierte Septen. Die histologischen Befunde entsprechen zwar im einzelnen bestimmten pathogenetischen Phasen, eine Stadieneinteilung der Lungenveränderungen insgesamt gelang jedoch nicht. Daher war es nicht möglich, den verschiedenen Beatmungszeiten bestimmte Erkrankungsphasen zuzuordnen. Aber auch Schwere und Ausdehnung der Lungenveränderungen ohne Rücksicht auf bestimmte Stadien waren nicht mit Art und Dauer der Beatmung korreliert. Dieses rührt u.E. daher, daß die geschilderten Lungenveränderungen unspezifisch sind, d.h. nicht nur durch die künstliche Beatmung, sondern auch durch die Grundkrankheit bzw. die Begleiterkrankungen verursacht oder entsoheidend beeinflußt werden können. Daher darf die künstliche Beatmung nur dann als Ursache für die Lungenveränderungen angesehen werden, wenn andere Ursachen auszuschließen sind.

Summary We examined the lungs of 48 bodies, to whom artificial ventilation had been applied before death, and found the following characteristic changes: The whole lung tissue appeared heavy, solidified, fleshy, with an increased content of liquid. Histological slides showed among others atelectases, a severe desquamation of alveolar lining cells, alveolar hemorrhages, a fibrinous exudate, hyaline membranes, marked hyperplasia of respiratory epithelium, giant cells, and thickened, infiltrated alveolar walls. Whereas the particular histological findings may be regarded as the equivalent of certain pathogenetic phases, it is not possible to classify the pulmonary lesions as a whole, because practically all the above named changes exist at the same time in the same lung. Therefore a relationship between the duration of artificial ventilation and a certain stage of lung disease did not become evident. Moreover there was no correlation between the kind and duration of ventilation and the extent of pulmonary changes. This is in our opinion due to the fact, that they are not specific, i.e. they may not only be caused or decisively influenced by the artificial ventilation but also by the basic disease and its complications. Therefore the artificial ventilation can only be regarded as the cause of pulmonary lesions, if other causes can be eliminated.
