    Notes: This study was designed to analyse prostaglandins (PGs) in human nasal lavage- fluid using the combination of microcolumn high performance liquid chromatography and a He/Cd laser induced fluorescence detection system. Forty-seven patients with allergic rhinitis and 12 healthy volunteers were investigated. Four species of PG. i.e. PGD2, PGE2, PGF2x and 6-keto-PGF1, were detected in the nasal lavage fluid. Concentrations of PGD2; (1.33 ± 0.17 nmol/ml) and PGE2 (0.87 ± 0.11 nmol/ml) in nasal lavage fluid from patients with allergic rhinitis (the allergy group) were significantly increased compared with those of volunteers (the control group, 0.23 ± 0.16 nmol/ml, 0.29 ±0.19 nmol/ml, respectively). On the other hand, no significant differences were observed in concentrations of cither PGF2x, or 6-keto-PGF1x between the control group and the allergy group. Histamine concentration in nasal lavage fluid was significantly increased in the allergy group (53 ± 7.6 nmol/l) compared with the control group (3.4 ± 1.0 nmol/1). No significant correlation was observed between PGD2 and histamine concentration (r= 0.24), or between PGE2 and histamine concentration (r= 0.08) in nasal lavage fluid from patients with allergic rhinitis. Treatment with oxatomide, an anti-histamine and anti-allergic drug, significantly improved symptom scores, but did not alleviate them completely. Concentrations of each PG detected in nasal lavage fluid did not change significantly after oxatomide treatment. It is concluded that, not only histamine but also PGs, particularly PGD2 and PGE2, might be involved in the genesis of allergic rhinitis.

    Notes: Background: Intercellular adhesion molecule-1 (ICAM-1) plays a key role in the early stage of the signal cascade leading to cellular extravasation and the development of an inflammatory response. Recently, it has been reported that the soluble form of this adhesion molecule is present in human sera, possibly mediating biological actions.Objective: The purpose of this study was to investigate levels of soluble ICAM-1 (sICAM-1) and its receptors in patients with allergic rhinitis, and to discuss sICAM-1's biological function.Methods: The levels of sICAM-1 in sera and nasal epithelial lining fluids (ELF), the percentage of CD11a-positive lymphocytes in the peripheral blood, and scores of subjective symptoms from 14 patients with pollinosis (allergic group) were measured from pre- to post-season, results were compared with those from 10 non-allergic subjects (control group).Results: The levels of sICAM-1 in sera and ELF were upregulated, and CD11a-positive lymphocytes were downregulatcd during the in-season in the allergic group. In addition, levels of sICAM-1 in sera from the allergic group remained high during the post-season, when levels of other parameters (symptoms, blood eosinophil counts, sICAM-1 in ELF and CD11a-positive lymphocytes) had roughly returned to the initial pre-season levels.Conclusions: We demonstrate systemic and local upregulation of sICAM-1 and systemic downregulation of LFA-1 positive lymphocytes in patients with seasonal allergic rhinitis under natural allergen provocation, suggesting that sICAM-1 plays a role in regulating seasonal allergic inflammation.

    Notes: We investigated whether the amount of antigen has an effect on the systemic and local levels of soluble ICAM-1 (sICAM-1) in patients with pollinosis, and assessed its biologic significance. The levels of subjective symptoms and SICAM-1 in sera and nasal epithelial lining fluids (ELF) from 14 subjects with pollinosis (allergic group) and eight healthy subjects (control group) were measured from pre- to postseason in 1993 (total pollen count: 10854/cm2) and 1994 (total pollen count: 415/cm2)), and the results were compared with each other among the four groups. The levels of subjective symptoms and slCAM-1 in ELF from the allergic group significantly increased during the season in both 1993 and 1994. However, there was a significant difference (P〈0.01) between the levels of those in 1993 and those in 1994 during the season. The levels of sICAM-1 in sera from the allergic group were significantly upregulated during the seasons and postseasons only in 1993, and there was a significant difference (P〈0.05) between the levels in 1993 and those in 1994 during the postseason. We conclude that amount of pollen has an influence on the local and systemic levels of sICAM-1, as well as the scores of subjective symptoms, in patients with seasonal allergic rhinitis.

    Notes: Summary We performed a 10-year comparative study on 95 cases (98 ears) of mumps deafness and 97 cases (97 ears) of profound sudden deafness and found the following results. The age of onset of deafness was less than 9 years in the majority of patients with mumps deafness, while it was frequently 30–50 years in those patients with sudden deafness. Among these latter patients, no cases occurred under 9 years of age. The incidence of tinnitus and vestibular symptoms was more frequent in the patients with sudden deafness, but no significant difference was noted in cases over 10 years of age. When determining prognosis, no cases of hearing improvement were encountered in mumps deafness, while about 70% of sudden deafness showed improvement.

    Notes: Summary We used a guinea pig model to investigate the effect of round window membrane rupture on endolymphatic and perilymphatic pressures under conditions known to increase these pressures: anoxia, hypercapnia, increased intracranial pressure, and occlusion of the vein of the cochlear aqueduct. When the round window membrane was not ruptured, increases in endolymphatic pressure paralleled the perilymphatic pressure following exposure to each of the experimental conditions. After the round window membrane had been ruptured, however, no increases in perilymphatic pressure or endolymphatic pressure were seen. These results suggest that endolymphatic pressure is dependent on perilymphatic pressure, even when the round window membrane is ruptured.

    Notes: Summary The effects of sudden occlusion of the vein of the cochlear aqueduct (VCAQ) on the cochlear blood flow, endocochlear potential (EP), endolymphatic and perilymphatic fluid pressures (PE and PP) were studied in the guinea pig. Cochlear blood flow showed a sudden decrease, and EP began to drop within 90s, ranging from 50 to 70 mV in 5 of 11 animals studied, recovering to normal levels when the animals were placed on a continuous inhalation of carbogen. The PE and PP increased simultaneously (max. PE = 3.4 ± 1.4 mm Hg; max. PP = 2.5 ± 1.0 mm Hg) and returned to their initial values after 5 min. The EP was sustained within the normal range, even when there was an apparent decrease in cochlear blood flow (61.4 ± 8.4%). We believe that variations in EP following VCAQ occlusion were due to anatomical differences in collateral venous communications among the animals studied. Carbogen inhalation produced uniform recovery patterns, indicating that individual collateral vessel responses were eliminated.

    Notes: Summary Although sudden deafness has many causes, it is often a sensorineural hearing loss with an abrupt onset and unknown origin. The majority of these latter cases are unilateral in their occurrences, while bilateral simultaneous involvement is rarely seen. During the past 13 years, we have treated 997 patients with sudden deafness. All patients were seen at our university hospital within 2 weeks after the onset of their hearing losses and were observed until the hearing level was fixed. Among them, 10 patients were found to have bilateral simultaneous onsets. The average age and the incidence of vestibular symptoms were similar to those with unilateral deafness. The hearing recovery was much better in the ear with the lesser deafness. No improvement was observed on the side with no sound perception. Those patients in whom common cold and fever seemed to trigger the onset of deafness were also noted to have high viral antibody titers, and were more commonly seen than were those cases with unilateral deafness.

    Notes: Summary For an accurate histological evaluation of cochlear blood flow, it is essential to fix the cochlear vessels while maintaining their physiological state. In the present study, we administered 10% CO2 to guinea pigs and then used phase-contrast microscopy to determine how two different methods of fixation influenced the cochlear vasculature. The first method of fixation employed perilymphatic perfusion in vivo, while the second one involved fixation after decapitation. Decapitation caused significant changes in the vessels of the stria vascularis, including constriction and sludging. In contrast, no sludging occurred in the perilymphatic perfusion method and erythrocyte morphology was preserved. However, dilatation of the strial blood vessels occurred after the inhalation of 10% CO2 even in the decapitation method. The results demonstrate that particular attention must be paid to the fixation method used, especially when evaluating the blood flow of the stria vascularis.

    Notes: Summary The vein of the cochlear aqueduct (VCAQ) is the principal drainage vein of the cochlea in the guinea pig. Morphological observations of the VCAQ and its adjacent structures were made by studying serial sections of the cochlea. We detected the presence of two collateral vessels from the mucoperiosteal veins of the middle ear which communicated with the VCAQ. Following acute occlusion of the VCAQ, marked dilatations of these vessels were observed in corrosion cast preparations. Our findings suggest that these vessels act as collateral veins following acute venous congestion of the inner ear.

    Notes: Abstract Otosclerosis is a disease of the otic capsule that is caused by abnormal resorption and redeposition of bony tissue. Sixty-two unrelated Japanese patients exhibiting clinical otosclerosis were typed for HLA-A, -B,-C antigens. Twenty-one of the patients were also typed for DR antigens. The frequency of HLA-Aw33 was significantly higher in otosclerosis patients than in the control group (24.2% vs 9.5%). This finding suggests that the presence of HLA-Aw33 antigens may be related to an increased susceptibility to otosclerosis or to its clinical outcome.
