Résumé Les auteurs rapportent le cas d'un homme de 47 ans présentant pendant une année des symptômes de tumeur épidurale de D5-7, excisée partiellement lors de l'intervention. L'examen histologique réalisé par trois morphologistes, était en faveur d'une tumeur maligne, mais sans qu'une classification précise puisse être faite. Le patient fut irradié de façon conventionnelle et son état s'améliora en quelques mois. Cependant, neuf mois après l'intervention et l'irradiation, les symptômes réapparurent. Une nouvelle intervention démontra une cicatrisation et l'état clinique fut attribué à une radionécrose tardive par des spécialistes. Une action à justice fut engagée, mais le malade n'obtint pas gain de cause. Par la suite pourtant, les compagnies d'assurances proposèrent un compromis. L'autopsie, pratiquée treize ans plus tard, montra une strangulation de la moelle épinière par le tissu cicatriciel, responsable de troubles. Aucun signe de radionécrose médullaire tardive ne fut observé.

Zusammenfassung Bericht über den Fall eines 47 Jahre alten Mannes mit einem epiduralen Tumor in Th5-7 mit einer Symptomatologie von einjähriger Dauer. Der Tumor wurde durch Laminektomie teilweise entfernt und durch drei Morphologen als maligne bezeichnet. Eine präzise Klassifikation konnte nicht erfolgen. Es wurde eine Röntgenbestrahlung mit konventionellen Methoden und Dosen durchgeführt und der Patient erholte sich sehr wesentlich. Nach einem „klinischen” Recidiv der Querschnittsläsion wurde erneut operiert und nur Narbenbildung gefunden. Das Gutachten der Spezialisten lautete auf eine „Röntgen-Spätnekrose” und es wurde dann ein gerichtliches Verfahren auf Schadenersatz eingeleitet. Zunächst wurde dem Antrag stattgegeben, später einigten sich die Versicherungsgesellschaften. — 13 Jahre später ließ sich durch Autopsie zeigen, daß nur eine Vernarbung des epiduralen Gewebes und dadurch eine Strangulation des Rückenmarks stattgefunden hatte. Zeichen einer spinalen Röntgen-Spätnekrose konnten nicht gefunden werden.

Summary A 47 year old man with symptoms for one year had an epidural tumor at Th5-7 which was removed partially by operation. The tissue was studied histologically by 3 morphologists who were of the opinion that it was malignant, but they were not able to classify it precisely. Radiation, in conventional dose and manner, was administered and marked symptomatic improvement followed in a few months. However, 9 months after operation and radiation, there was a recurrence of symptoms. Reoperation revealed only scarring and the clinical condition was attributed by the consulting specialists to “delayed radionecrosis”. Legal action followed and the prosecution first lost the case, but afterwards the insurance companies made an out-of-court “compromise”. Postmortem examination 13 years later revealed that strangulation of the spinal cord, due to scar tissue, caused the disability. No signs of “delayed spinal radionecrosis” were found.
