    Notes: Abstract  The geochemical and mineralogical characteristics of bulk samples and various size fractions of sulfide-bearing fine-grained sediments in the Petalax area, western Finland, were studied with ICP-AES (aqua regia digestion and "total" digestion) and X-ray diffraction. The sediments, which are dominated by particles 〈0.06 mm (clay and silt size), are composed of phyllosilicates (micas, kaolinite, chlorite, vermiculite), quartz, feldspars, amphiboles, organic matter, and secondary S minerals. The chemical analyses show that Na, Ca, and Sr are enriched in the silt and sand fractions, that Zr is enriched in the silt fraction, and that the concentrations of all other studied metals (Al, Ba, Co, Cr, Cu, Fe, K, La, Mg, Mn, Nb, Ni, Sc, Th, Ti, V, Y, and Zn) increase from the sand to the clay fraction. Sodium, Ca, and Sr occur largely in poorly dissolved feldspars, which explains the distribution of these metals in the sediments. The increase in the concentrations of most metals in the clay fraction is interpreted to be the result mainly of an increase in phyllosilicates in this size fraction. A geochemical comparison between the sulfide-bearing fine-grained sediments and glacial till shows that the former are enriched in several potentially toxic metals. It is therefore argued that the hydrological and ecological problems associated with the sulfide-bearing sediments are related not only to the production of acidity in oxidized layers, but also to mobilization and dispersion of toxic metals. Strategies to minimize damage of freshwater systems in areas covered with sulfide-bearing fine-grained sediments are suggested.

    Notes: A double-blind, randomised. 4-week clinical trial with parallel group design in 57 patients with gingivitis was conducted for studying the antibacterial efficacy and safety of a delmopinol HCl aqueous solution 2 mg/ml (0.2% w-'v). which was used for unsupervised mouth-rinsing and compared with placebo and chlorhexidine digluconate 2 mg/ml (0.2% w/v, Hibitane Dental. 1CI Pharmaceuticals, UK). The plaque index and plaque wet weight were used to measure plaque formation, and gingival fluid flow and bleeding on probing to measure gingivitis. According to the reduction from baseline, chlorhexidine showed a significantly better effect on plaque formation than the placebo after 4 weeks treatment for both plaque measurements. Delmopinol exhibited significantly lower plaque index scores than placebo. The difference between chlorhexidine and delmopinol was not statistically significant for any of the plaque measurements. For gingivitis, no statistically significant differences were obtained between the effects of delmopinol. chlorhexidine and placebo. A transient anaesthetic sensation in the oral mucosa was experienced more clearly by the patients in the delmopinol group than by those using chlorhexidine or placebo rinses. Rinsing with chlorhexidine resulted in more staining of the teeth and tongue than did delmopinol and placebo. The placebo solution lasted better than the 2 active solutions. The results showed that rinsing with either delmopinol HC1 aqueous solution 2 mg/ ml or chlorhexidine digluconate 2 mg/ml 2× daily for 60 s as a supplement to normal oral hygiene, following an initial professional tooth cleaning, leads to a lower plaque formation than rinsing with placebo. In order to obtain conclusive results beyond the initial etTect of the professional tooth cleaning and the placebo effect and lo achieve enough power in the statistical analysis of gingivitis reduction, trials of longer duration and with a larger number of patients are required. This study showed good tolerance and acceptability of mouth-rinsing with the detmopinol HC1 aqueous solution 2 mg/ml.

    Notes: Abstract. Delmopinol has been considered as a potential agent for the chemical control of plaque. The aims of these studies were to measure the effects of a 0.2% delmopinol hydrochloride mouthrinse on (1) plaque reformation and (2) salivary bacterial counts. Comparisons were made with a 0.2% chlorhexidine rinse and a placebo rinse. A group of 12 male volunteers took part in the plaque study which was of a double blind, randomised, 3 cell, cross-over design. From a zero plaque baseline subjects rinsed, 2 × a day, under supervision, for 1 min with 10-ml volumes of the allocated rinse. After 4 days, during which no other form of oral hygiene was performed, plaque was scored by area and index. Plaque results were significantly lower with chlorhexidine and delmopinol compared with control, and with chlorhexidine compared to delmopinol. Side-effects with delmopinol were transient tingling and numbness of the tongue in some subjects. A 2nd group of 12 male volunteers received single, 1-min rinses of the 3 formulations. Salivary bacterial counts were determined immediately before and up to 420 min after rinsing. Compared to the control rinse, chlorhexidine significantly reduced bacterial counts of 420 min. Delmopinol produced a small reduction in bacterial counts which was only significantly different from control at one time point. Delmopinol deserves further evaluation as a chemical plaque inhibitor, particularly when used as an adjunct to normal toothcleaning.

    Notes: This review of laser Doppler flowmetry (LDF) focuses on indications and limitations in general and the author’s experience of LDF in the assessment of tendon perfusion in particular. LDF is easy to use, minimally invasive and well suited for the study of relative changes in perfusion during rapid events. Problems specifically related to LDF include motion artifacts, a non-flow dependent output of unknown significance and uncertainty regarding the spatial resolution. The human Achilles tendon has a lower perfusion at the insertion but otherwise an even distribution of blood flow. Male gender, advancing age and mechanical loading of the tendon are associated with diminished tendon blood flow. The lesion in chronic degenerative Achilles tendinopathy is associated with hyperemia of uncertain origin.

    Notes: Abstract  Objective. To compare information gained by ultrasonography and magnetic resonance imaging (MRI) in chronic achilles tendinopathy with regard to the nature and severity of the lesion. Design. Imaging of both achilles tendons with ultrasonography and MRI was performed prior to unilateral surgery. Operative findings and histological biopsies together served as a reference. Patients. Twenty-seven patients (22 men, 5 women; mean age 44 years; 21 athletes) suffering from chronic achilles tendinopathy participated in the study. Eighteen patients had unilateral and 9 had bilateral symptoms. Results and conclusions. Surgical findings included 4 partial ruptures, 21 degenerative lesions and 2 macroscopically normal cases. Microscopy revealed tendinosis (degeneration) in all tendon biopsies, including cases with a partial rupture, but only slight changes in the paratendinous tissues (paratenon). Ultrasonography was positive in 21 of 26 and MRI in 26 of 27 cases. Severe intratendinous abnormalities and a sagittal tendon diameter 〉10 mm suggested a partial rupture. In tendons with a false negative result histopathological changes were mild and a tendency towards a better clinical outcome was noted in the sonographic cases. Assessment of the paratenon was unreliable with both methods. Ultrasonography and MRI give similar information and may have their greatest potential as prognostic instruments.
