    Notes: Abstract Objective: Pregnanolone is a 3α-hydroxylated-5β-reduced metabolite of the female sex steroid hormone progesterone. The compound is currently being evaluated for anaesthetic purposes. Previous studies have indicated a differential physiological response across the menstrual cycle and a different response in patients with premenstrual syndrome (PMS). This study was undertaken to determine whether hormonal changes during the menstrual cycle influence pregnanolone pharmacokinetics and to compare PMS diagnosis-related differences in pregnanolone pharmacokinetics. Methods: Seven patients with premenstrual syndrome and seven female controls were given three increasing doses of pregnanolone in the follicular and luteal phase of the menstrual cycle. Results: Mean pregnanolone elimination half-life varied between 28.4 min and 31.8 min and clearance between 59.6 ml · min−1 · kg−1 and 64.0 ml · min−1 · kg−1, depending on diagnostic group and cycle phase. No significant differences in pregnanolone pharmacokinetic properties were found between PMS patients and controls in either phase of the menstrual cycle. Furthermore, no differences in pharmacokinetic variables were detected between cycle phases. Conclusion: Pregnanolone pharmacokinetics do not differ between follicular and luteal phase of the menstrual cycle, nor between PMS patients and control subjects.

    Notes: Glucocorticoids and neurosteroids, such as allopregnanolone and tetrahydrodeoxycorticosterone, are released during stress. A non-genomic effect of glucocorticoids has been established but is not yet fully understood. We have studied the effect of glucocorticoid metabolites on the γ-aminobutyric acid (GABA) system. In these experiments we studied the effects of the glucocorticoid metabolites allotetrahydrocortisol, tetrahydrocortisol, allotetrahydrocortisone and tetrahydrocortisone in rat cortical microsacs. Our results showed that both these cortisol and cortisone metabolites reduce GABA-mediated chloride ion uptake. This reduction was not observed in the presence of allopregnanolone but allotetrahydrocortisol interacts with allopregnanolone, enhancing the allopregnanolone-stimulated potentiation of GABA-mediated chloride ion uptake. This enhanced effect was completely blocked by the addition of 30 µm of the 3β-isomer of allopregnanolone, isoallopregnanolone. Our findings show that steroids released during stress interact with each other and GABA in the GABA system.

    Notes: Equivalent doses of oestradiol-17β were given orally and percutaneously to eight post-menopausal women and three men. The resultant serum levels of unconjugated immunoreactive oestrogens and total oestrone were followed during 48 hours after hormone administration. A single percutaneous dose of 4 mg oestradiol-17β resulted in increased serum levels of oestradiol-17β in all individuals. Compared to oral administration the increase in serum oestrogen levels was quite slow and they were maintained for 48 hours. In contrast to the rapid and very marked increase in serum total oestrone following oral administration of oestradiol-17β, only a moderate increase was obtained after percutaneous administration and there was a tendency to a biphasic pattern. Topical administration of oestradiol-17β might have physiological advantages as the steroid will reach the peripheral circulation without passing the entero-hepatic circulation.

    Notes: Summary Comparison is made between numbers of fits and estrogen, progesterone plasma values during the menstrual cycle of women with epilepsy. All six cycles in women with ovulation had a significant positive correlation between estrogen/progesterone ratio and scores of fits. Three periods without ovulation all showed a significant correlation to estrogen variations.
