    Notes: Abstract To investigate the role of injecting cultured fetal-bladder tissue into the region of the vesicoureteric orifice (VUO) to correct surgically produced vesicoureteric reflux (VUR), 12 Coopworth ewe lambs were studied. Four weeks after incising the intravesical segment of ureter, VUR was demonstrated by micturating cystourethrography. Bladder tissue was obtained from a fetal Coopworth lamb at 10 weeks' gestation, cultured in RPMI 1640, and injected into the region of the VUO of 1 ureter of each lamb using an open surgical approach. The lambs were killed between 1 and 6 months after the injection. Smooth-muscle cells from the cultured fetal bladder tissue were identified by the monoclonal antibodies HHF-35 for muscle alpha-actin and D33 for muscle desmin, and by electron microscopy. One lamb died of a gastrointestinal infection at 8 weeks of age. Of the remaining 11 animals, the injection of fetal-bladder tissue corrected the reflux in 7, while it was reduced in degree in 3 and persisted unchanged in 1. The reflux on the contralateral control side was also corrected in 6 ureters and improved in 2. Using histochemical techniques, grafted fetal-bladder tissue could not be differentiated from host tissue in the region of the VUO. Histopathological studies failed to show any injected tissue in distant organs. This study has shown that surgically-induced VUR in lambs was corrected or improved by the injection of cultured fetal-bladder tissue into the submucosa adjacent to the VUO.

    Notes: Abstract The efficacy of preventing neonatal urinary infection in infants by diagnosing hydronephrosis in the fetus on obstetrical ultrasonography was studied. 426 infants had uroradiologic evaluation between 1984 and June 1991 because they had hydronephrosis detected in utero. Thirteen with posterior urethral valves were excluded. Of the remaining 413,13 (3.1%) presented with urinary infection in the first 6 months of life. Ten of the 13 were boys and 7 were not circumcised. Eleven of the 13 infants less than 2 months old were formula-fed. The causes of hydronephrosis were reflux alone in 6, ureteropelvic junction obstruction in 6 (with coexisting ipsilateral reflux in 4), and primary megaureter in 1. Ultrasonography alone was insufficient to exclude reflux. Amoxicillin-resistant bacteria were the causative organisms in all 10 for whom bacteriology data was available. Four categories of management failure were identified: 1) failure of communication of the prenatal findings, 2) antibiotics not prescribed, 3) antibiotics prescribed but not administered, and 4) infection in spite of continuous antibiotic prophylaxis. Uncircumcised formula-fed male infants with reflux seemed to be at special risk for infection.

    Description / Table of Contents: Zusammenfassung Im Folgenden wird eine Übersicht über publizierte Studien zur Behandlung von Harnwegsinfektionen mit einer Antibiotika-Einmaldosis gegeben. Unkomplizierte Harnwegsinfektionen bei Frauen und Kindern, verursacht durchEscherichia coli, sprachen auf eine Einmaldosis verschiedener antimikrobieller Substanzen ebensogut an wie auf eine Behandlung mit mehreren Dosen. Zur Zeit werden Trimethoprim und Co-trimoxazol am häufigsten für die Einmaltherapie eingesetzt. Das Nichtansprechen auf eine Einmaltherapie dürfte ein einfacher Hinweis darauf sein, daß weitere diagnostische Maßnahmen zur Abklärung der Harnwegsinfektion oder eine intensivere Therapie nötig sind. Derzeit ist die Einmaltherapie das Behandlungsverfahren der Wahl bei unkomplizierter Harnwegsinfektion.
    Notes: Summary The published studies of the use of single dose antimicrobial therapy for the treatment of urinary tract infection have been reviewed. In women and children a single dose of any of several antimicrobial agents was as effective as a course of treatment for uncomplicated urinary tract infections caused byEscherichia coli. Trimethoprim or co-trimoxazole are currently the preferred agents for single dose therapy. Failure of single dose therapy may prove to be a simple guide as to the need for further urinary tract investigation or more intensive therapy. Single dose antimicrobial therapy is now the treatment of choice for uncomplicated urinary tract infections.
