    Notes: Objective To provide a valid estimate of singleton neonatal mortality based on birthweight and gestational age at delivery.Design Record linkage of maternity data and neonatal mortality data.Setting Scotland, UK.Population All singleton preterm deliveries from 24 to 36 weeks inclusive between 1985 and 1994.Main outcome measure Neonatal death.Results There were 625,646 liveborn singleton deliveries over the study period, of which 33,912 were preterm (5.4%). The overall neonatal mortality in the preterm group was 41/1000 and the data have been presented by both gestational age and birthweight. The neonatal mortality rate fell with advancing gestation from 795/1000 live births at 24 weeks to 9/1000 live births at 36 weeks and was higher at the extremes of birthweight for a given gestational age. There was a significant increase in the proportion of babies delivered iatrogenically over the study period (χ test for trend P 〈 0.001).Conclusion This is the largest recent series to consider neonatal mortality using both birthweight and gestational age. These figures will be of use in obstetric management when elective preterm delivery is considered, and for providing prognostic guidance following preterm delivery.

    Notes: Abstract– Four quantitative (area-standardized electrofishing, trap nets, small-mesh [1.3 and 2-cm bar mesh] gill nets, large-mesh [2.5 and 5-cm bar mesh] gill nets) and two qualitative (non-standardized electrofishing, dip netting) sampling techniques were concurrently used to capture larval to adult fish in forested wetland habitats on seasonally inundated floodplains of two Alabama (USA) streams. Standardized area electrofishing appeared to be the best sampling technique for collecting and quantifying species because of high catch rate, broad species and size coverage, and rapid sampling. Electrofishing accounted for most fish (47 and 57% of pooled catch), almost all taxa, and a broad range of fish sizes. Large-mesh gill nets captured few fish (2% and 8%), a distinct subset of the species present, and the largest fish. Light traps were the most efficient and taxa-comprehensive sampling technique for larval fish. Light traps captured the vast majority of the pooled larval fish catch, including 6 of 7 families recorded at the sites. Larval fish dip netting captured larval fish of most families known to be present but in low numbers. For surveys of species composition and rapid bioassessments, nonstandardized electrofishing and larval fish dip netting appear adequate. For intensive research studies, the combination of area electrofishing, large-mesh (and in some cases small-mesh) gill nets, and light traps provide thorough and comprehensive data on wetland fish assemblages.

    Notes: Summary We consider the variational solution of a particular class of second order differential equations and show that expansions in terms of Chebychev and a range of ultraspherical polynomials lead to operator matrices that are asymptotically diagonal, and that hence their convergence properties can be completely characterised using a previously developed analysis. For a given class of weight functions bounds are given on the convergence of the coefficients and of the weighted mean square error, in terms of the analyticity properties of the coefficients in the differential equation. These bounds are used to discuss the optimum choice of weight function for such a calculation.

    Notes: Abstract Growth hormone (GH) insufficiency was demonstrated in a patient with methylmalonic aciduria. GH administration at 14–21U/m2 per week accelerated linear growth, stimulated lipolysis, and produced clinical improvement and reduced urinary methylmalonate excretion. The clinical and metabolic benefits were lost as the dose of GH was increased to 28U/m2 per week. Conclusion The use of GH in other patients with methylmalonic aciduria (and other disorders of intermediary metabolism demonstrating similar clinical and metabolic features) warrants further study.

    Notes: Abstract Over an 18-month period serial observations of plasma tyrosine, methionine and urinary tyrosine metabolites were made and compared with urinary succinylacetone excretion in an infant with tyrosinaemia type 1 treated by diet alone. Despite broadly similar profiles there were significant temporal and quantitative differences between each of these metabolic parameters. Only when plasma tyrosine was kept in the low-normal range by strict phenylalanine restriction (10–15 mg phenylalanine/kg body weight) was detectable succinylacetone consistently eliminated from the urine. Urinary succinylacetone is the only measure of metabolite accumulation immediately proximal to the enzyme defect and its routine measurement will allow more effective control of dietary treatment.

    Notes: Abstract Sodium dodecyl sulphate-polyacrylamide gel electrophoresis of whole cell protein, immunoblotting with normal human serum and restriction endonuclease analysis usingTaq I enzyme were applied to 38 clinically significant isolates ofMoraxella (Branhamella) catarrhalis obtained during a suspected outbreak of nosocomial infection. Each of 18 strains had individual profiles by at least two of the three methods (unique strains). The remaining 20 strains were assigned to five groups (A-E) on the basis of similarity by at least two of the three methods. Isolates within groups A, D and E were homologous by all three methods. Immunoblot groups B and C had two distinct whole cell protein profiles (B1 and B2) but indistinguishable restriction endonuclease profiles (group B/C). This emphasizes the need to use more than one technique in characterizing strains from suspected outbreaks of nosocomial infection. Grouped strains were more likely to originate from the same hospital ward than unique strains and were associated with a significantly longer median time from patient admission to strain isolation (14 versus 3.5 days, p〈0.005). Furthermore, the β-lactamase activity was homologous within the groups. The results suggest that nosocomial infection involving several distinctMoraxella catarrhalis strains persisted over a period of months, involving at least 20 patients on three different wards. Such infection is probably common in wards harbouring suitably predisposed patients. The mode of transmission remains to be elucidated, but the above three techniques possess sufficient reproducibility and discriminatory ability to constitute suitable investigative tools.
