    Notes: Abstract: We compared the thiamine and thiamine phosphate contents in the frontal, temporal, parietal, and occipital cortex of six patients with frontal lobe degeneration of the non-Alzheimer's type (FNAD) or frontotemporal dementia with five age-, postmortem delay-, and agonal status-matched control subjects. Our results reveal a 40–50% decrease in thiamine diphosphate (TDP) in the cortex of FNAD patients, whereas thiamine monophosphate was increased 49–119%. TDP synthesizing and hydrolyzing enzymes were unaffected. The activity of citrate synthase, a mitochondrial marker enzyme, was decreased in the frontal cortex of patients with FNAD, but no correlation with TDP content was found. These results suggest that decreased contents of TDP, which is essentially mitochondrial, is a specific feature of FNAD. As TDP is an essential cofactor for oxidative metabolism and neurotransmitter synthesis, and because low thiamine status (compared with other species) is a constant feature in humans, a nearly 50% decrease in cortical TDP content may contribute significantly to the clinical symptoms observed in FNAD. This study also provides a basis for a trial of thiamine, to improve the cognitive status of the patients.

    Notes: Critically ill patients are usually in a catabolic state and may require total parenteral nutrition; this often includes lipid emulsions. Any adverse effects of constituents on pulmonary function, white cell function or the haemocoagulation system could have disastrous consequences in such patients. We have investigated the effects of a new intravenous lipid preparation containing medium chain triglyceriks, which, in severely ill malnourished patients are theoretically a preferable energy source to conventional drug chain triglycerides. In a pilot study 17 critically ill patients whose lungs were artificially ventilated were given this lipid emulsion; no adverse eflects were observed. Arterial oxygen and carbon dioxide tensions, ratio of inspired oxygen fractional concentration to arterial oxygen tension. platelet and white cell counts all remained constant and the complement system was not activated.

    Notes: Summary The diameters and densities of capillaries and arterioles in the hippocampal cortex of normal subjects and patients with Alzheimer's dementia were measured in thick celloidin sections stained for alkaline phosphatase. Microvascular diameters in general are affected more by age than by the presence of dementia of the Alzheimer type. The diameter of both capillaries and arterioles increases significantly with age. The density of capillaries decreases whereas that of the arterioles increases significantly. The capillary changes suggest that a reduced exchange potential accompanies ageing. In brains of people with Alzheimer's disease the overall capillary diameters and densities do not differ from those of age-matched controls. Regional changes may, however, be important: those hippocampal zones showing the greatest severity of or increment in nerve cell lesions do correspond to those having the highest levels of or increase in capillary density and the greatest decrease in diameter, suggesting a direct association between neuronal susceptibility to Alzheimer changes and degree of regional blood supply. Capillary surface areas, volumes, and area/capillary volume ratios support the possibility of this relationship. Neurofibrillary tangles and granulovacuolar degeneration do not correlate equally with the degree of capillary “irrigation” tangles are more closely related to these morphological vascular parameters.

    Notes: Summary The number of pyramidal neurones in the hippocampal cortex was determined in serially sectioned mesial temporal lobe from brains of 18 mentally normal people; as well as those of 8 demented patients with pathologically confirmed Alzheimer's disease. Normal ageing was accompanied by a gradual loss of neurones, whereas dements' brains showed a much more severe decrease, exceeding that of controls at any age. A high degree of negative exponential correlation was found between the density of neurones/mm3 of cortex and both the number of neurones with neurofibrillary degeneration and the number with granulovacuolar degeneration. The functional significance of the latter changes is thus probably greater than previously assumed, given the diminished population of surviving neurones in which these alterations appear. Both tangles and granulovacuoles demonstrated a stronger propensity for occurring in the posterior half of the hippocampus in demented patients' brains. This would not have been predicted from the relative distribution of neuronal loss in the two halves. The posterior portion of the hippocampus may be considerably more susceptible to the degenerative nerve cell changes prominent in dementia of the Alzheimer type.

    Notes: Summary Topographic analysis was performed of the distribution of Alzheimer's neurofibrillary tangles and the granulovacuolar degeneration of Simchowicz in the hippocampal cortex of patients with Alzheimer's dementia and mentally normal aged controls. A semiautomated scanning stage microscope was linked potentiometrically to an XY pen recorder in order to plot cytoarchitectonic “scattergrams” from the sequentially screened hippocampal formations. The density of both lesions per cubic mm of pyramidal cortex was quantified by measuring the area of each of six “zones”, using a digitizer and programmable calculator. In elderly normal brains as well as those of Alzheimer's disease, the statistically most representative ranking order of predilection forneurofibrillary tangles (in decreasing severity) was: entorhinal cortex 〉 subiculum 〉 H1 〉 end-plate 〉 presubiculum 〉 H2. Forgranulovacuolar degeneration the best rank order was: subiculum 〉 H1 〉 H2 〉 end-plate 〉 entorhinal cortex 〉 presubiculum. The notable similarities of both such orders of predilection to the well-recognized “selective vulnerability” of certain hippocampal neurones in clinical conditions of hypoxia, ischemia and epilepsy suggest some common, focally accentuated cytotoxic mechanism may underlie all these regional predispositions.

    Notes: Abstract The metabolic effects of an intravenous lipid emulsion containing medium chain triglycerides (MCTs) were studied in eleven critically ill, ventilated patients receiving parenteral nutrition. The effects were compared in a cross-over study with those of a conventional emulsion containing only long chain triglycerides (LCTs). The lipid was well tolerated, but there were differences in the metabolic effects with a significantly greater increase in the plasma concentrations of glycerol and ketones during MCT/LCT infusion compared to LCT. The plasma concentration of non esterified fatty acids was also higher. This fell rapidly post infusion. Since non esterified fatty acids and ketones are readily available energy sources for tissues lipid emulsions containing MCT may prove valuable for catabolic, critically ill patients.

    Notes: Abstract Objectives The study investigated the use of an intravenous lipid emulsion containing medium chain tryglycerides (MCTs) in critically ill patients, and compared the effects with those of a conventional long chain triglyceride (LCT) preparation. Design Patients received a parenteral nutrition regime including either 500 ml 20% Lipofundin MCT/LCT (1/1) per day, or 500 ml 20% Lipofundin S (LCT) infused over 8 h each evening. Setting The patients were receiving treatment, including assisted ventilation, in the Intensive Care Unit of a large teaching hospital. All patients on this unit for at least 3 days and who were likely to receive parenteral nutrition for at least a week were considered, unless they had severe renal or liver disease, or trauma/major surgery in the previous 3 days. Because ICU patients are a heterogenous group, subjects were randomised within clinical groups to receive either lipid. There were 24 patients entered into the study and the data on 20 matched patients is reported. Measurements and results Blood specimens were collected pre-TPN, daily at 0800 and after 5 h lipid infusion on days 1 and 6. Urine collections (24 h) were also performed. There were no apparent adverse effects due to the new MCT/LCT emulsion. Plasma ketone and glycerol concentrations were higher during MCT/LCT infusion, but 8 h post infusion plasma levels of ketones, triglycerides, non-esterified fatty acids and glucose were similar. Urinary carnitine excretion was high in all patients and was not significantly different between the groups. Nitrogen balance was less negative in patients receiving MCT/LCT on days 6 and 9. Conclusion MCTs are rapidly hydrolysed and oxidised to fatty acids and ketones which can be readily utilised. This study indicates that intravenous lipid emulsion containing MCT are safe in critically ill patients and may have advantages over LCT. The number and range of patients studied was, however, small and larger studies are needed.

    Notes: Summary Fifty-two patients with Fredrickson Type IIb or Type IV hyperlipidaemia, in whom diet had not achieved satisfactory lipid levels, completed a double blind randomised study of acipimox versus placebo. The patients were given acipimox, 250 mg three times daily or placebo for a three month period, and plasma lipids and glucose were monitored. The patients receiving acipimox showed a fall in the mean concentration of plasma triglyceride compared to placebo (0.74 mmol/l) and this was most marked in patients whose initial plasma triglyceride levels were greater than 3 mmol/l (1.0 mmol/l, confidence limits 0.18, 1.82). Acipimox was well tolerated, and could be a useful addition to the drugs available for the treatment of patients with hypertriglyceridaemia.
