    Notes: Summary The purpose of this study was to demonstrate that the laparoscopic Stoppa procedure by the transabdomino-preperitoneal route is possible with results similar to those of an open procedure, and that there are two learning curves: one for the posterior route described by Stoppa, and the second for the laparoscopic route. After describing their laparoscopic inguinal hernia repair experience, the authors report their technique, used since 1991 and discuss its indications and limitations. After a series of 800 patients and 920 operations, they consider the laparoscopic procedure now to be their standard method in inguinal hernia repair. The laparoscopic route implies minimal access but not minimal surgery. Finally, to shorten the learning curve, the authors suggest that beginners assist at 20 operations and operate on 20 to 30 patients assisted by a surgeon experienced in laparoscopic inguinal hernia repair.

    Description / Table of Contents: Abstract At attempt has been made by the authors to automate the aspiration of blood in cardiac surgery under extra-corporal circulation. The presence of blood at the end of tubing can be detected by means of a system based upon the blood's property of conductivity. The motor of the aspiration pump is halted when there is no blood. The motor is triggered when blood appears. Another detector has been designed to prevent the suction effect when the extremity of the tube is applied against a wall. This suction is accompanied by depression inside the tube which persists even if the pump is stopped. This depression is picked up by a vacuometer. An electronic system detects a limit level of negative pressure and stops the pump motor. To oppose the persistence of this suction blood arrives through a calibrated aperture at the level of the aspiration tubing from a reservoir in which there is at mospheric pressure; this enables the cannula to detach itself from the tissue. The pump will start again when the pressure in the tube is equal to atmospheric pressure and if there is blood at the end of the cannula. These two detectors have been linked in such a way as to form an automatic aspiration unit.
    Abstract: Zusammenfassung Die Autoren versuchen, die Aspiration von Blut bei Herzoperationen durch extra, korporellen Kreislauf zu automatisieren. Die Anwesenheit von Blut am Rohrende läßt sich mittels eines Systems, das auf der Leitfähigkeit des Blutes basiert, feststellen. Der Motor der Saugpumpe stellt sich ab, wenn kein Blut vorhanden ist und wieder an, sobald Blut erscheint. Ein weiterer Detektor wurde entwickelt, um den Saugeffekt auszuschalten, wenn das Rohrende an einer Wand angliegt. Dieses Saugen wird von einem Druckabfall im Rohr begleitet, selbst wenn die Pumpe abgestellt wird. Dieser Druckabfall wird mittels eines Saugmenters festgestellt. Eine elektronischeVorrichtung stellt einen Grenzwert für den negativen Druck fest und schaltet den Pumpenmotor aus. Um dem anhaltenden Saugen entgegenzuwirken, kommt Blut durch eine geeichte Öffnung in Höhe der Saugleitung aus einem Behälter, in dem Atmosphärendruck herrscht; hierdurch wird erreicht, daß sich die Kanüle vom Gewebe löst. Die Pumpe arbeitet wieder, sobald der Druck im Rohr gleich dem Atmosphärendruck ist, und falls sich Blut an der Absaugöffnung befindet. Diese beiden Detektoren wurden so verbunden, daß sie zusammen eine automatische Sauganlage darstellen.
    Notes: Sommaire Les auteurs ont tenté d'automatiser les aspirations du sang en chirurgie cardiaque sous circulation extra-corporelle: un système basé sur la propriété de conductivité sanguine permet de détecter la présence de sang en bout de tubulure. En l'absence de sang, le moteur de la pompe d'aspiration est arrété. Lorsque du sang apparait la mise en route est déclenchée. Un autre détecteur a été réalisé pour empêcher l'effet de succion, lorsque l'extrémité de la tubulure est appliquée contre une paroi. Cette succion s'accompagne d'une dépression à l'intérieur du tuyau, qui persiste même si la pompe s'arrête. Cette dépression est captée par un vacuomètre. Un système electronique détecte un niveau limite de pression négative et arrête le moteur de la pompe. Pour s'opposer à la persistance de cette succion, un orifice calibré situé au niveau de la tubulure d'aspiration permet l'arrivée de sang provenant d'un réservoir où régne la pression atmosphérique, ce qui permet à la canule de se décoller du tissu. La pompe se remettra en marche lorsque la pression dans la tubulure sera égale à la pression atmosphérique et si du sang est présent à l'extrémité de la canule. Ces deux détecteurs ont été associés de façon à former un bloc d'aspiration automatique.
