Neurological manifestations of the thoaracic outlet syndrome are characterized by pain and paresthesia in the ulnar forearm and hand and by atrophic paresis of the hand muscles. Symptoms are caused by compression of the lower part of the brachial plexus by a cervical rib ligament. The condition is rare. Diagnosis is based on clinical neurological examination, imaging and electromyography. The most frequent neurological differential diagnoses are carpal tunnel syndrome, lower plexus lesion of other etiology and C-8 nerve root irritation. With a sufficiently established diagnosis and a manifest neurological deficit surgical intervention should be considered to prevent progression.

Bei der neurologischen Form des Thoracic-outlet-Syndroms (TOS) treten Reizerscheinungen in Form von Schmerzen oder Parästhesien im Bereich des ulnaren Unterarms und der ulnaren Handpartie sowie atrophisierende Lähmungen der Handmuskulatur auf. Ursächlich ist eine Kompression der unteren Plexusanteile durch ein Halsrippenband. Das Krankheitsbild ist selten. Diagnostisch hilfreich ist neben der klinisch-neurologischen Untersuchung und der bildgebenden Diagnostik die Elektromyographie. Die häufigsten neurologischen Differenzialdiagnosen des TOS sind das Karpaltunnelsyndrom, die untere Plexusläsion anderer Genese und das Wurzelkompressionssyndrom C8. Bei sicherer Diagnose und bestehenden neurologischen Ausfällen sollte eine operative Therapie erwogen werden, um die Progredienz der Paresen zu verhindern.
