    Notes: Summary A case is reported of a 21 month old boy from whom a 357 g ependymona was removed. Postoperatively, radiation therapy (Co60) was administered, and the central dosage was estimated to be 4,350 r given over 46 days. Seven and a half years later craniotomy was again performed because of a supposed recurrence. However, a meningeal sarcoma weighing 370 g was found. Tumour induction by radiotherapy is discussed on the basis of published reports, and the need of estimating tumour dose in relationship to the age of the patient is stressed.

    Notes: Summary Seven cases of so-called melanosis of the dentate nucleus are reported, 6 of them females and all but one over 49 years of age. The neuromelanin-like pigment seemed to be derived from lipofuscin in protoplasmic astrocytes and in 4 of the cases it was present in cerebral grey matter, mainly cortical, in addition to the cerebellum. The intensity of pigmentation and granule size in the different sites suggested that it could have developed sequentially, first in the dentate nucleus, then in the cerebellar granular layer and ultimately in the cerebral grey matter from occipital cortex forward. the melanosis appeared not to give rise to clinically recognisable functional effects and there was no other consistently associated disease process.

    Notes: Abstract The importance of the method of administration of acid glucose infusions for the venous inflammatory response has been investigated in two series of experimental trials. 60 ml of 5% glucose solution was administered into rabbit-ear veins in three ways: 1) continuously over 5 hours (slow infusion), 2) continuously over 1 hour (rapid infusion), 3) discontinuously during 2 x 30 minutes with an interval of 4 hours (discontinuous infusion). Microscopical examination of the veins revealed that the inflammatory changes were less pronounced after rapid and discontinuous infusions than after slow infusions.

    Notes: Abstract The clinical experience that a fat emulsion (Intralipid®) protects the vein against infusion thrombophlebitis caused by an amino acid solution (Aminosol®) was controlled by an experimental method based on a quantitative histological analysis of the inflammatory changes observed in the veins of rabbit ears after infusions. In a controlled blind study, sequential analysis failed to reveal any difference in the histological changes which developed after infusion of Aminosol alone and Aminosol plus Intralipid. In a second series of experiments in which Intralipid was compared with 0.9% NaCl it was demonstrated that the fat emulsion alone gave rise to considerable inflammatory changes in the veins.

    Notes: Abstract An experimental method is presented, which compares the tendency of different infusions to cause thrombophlebitis. It is based on a quantitative histological analysis of the inflammatory changes in the veins of rabbit ears after infusions under standardized conditions. By means of this method the inflammatory changes in the veins have been shown to be significantly less pronounced when the pH of glucose solutions is altered from 3.0 to 3.6. This pH change has been prescribed in the 1971 corrections to Pharmacopoea Nordica 1963. By complete neutralization of 5% glucose a further reduction of the damage to the veins has been obtained. For this purpose phosphate buffer is recommended.

    Notes: Abstract On the basis of clinical experience it has been claimed that glucose results in a higher incidence of infusion thrombophlebitis than fructose, and that glucose solutions in concentrations higher than 5% are extremely irritant to the venous endothelium. The validity of these two assertions has been investigated by an experimental method based on a quantitative histological study of rabbit-ear veins into which infusions were given under standardized conditions. In two series of experiments, it was shown (1) that fructose solutions caused significantly more inflammatory changes in the veins than did glucose solutions, and (2) that glucose in 10% solutions caused significantly more injury to the veins than 5% solutions, although the difference in the average inflammatory changes was only slight.

    Notes: Summary The use of morphometry and modern stereology in malignancy grading of brain tumors is only poorly investigated. The aim of this study was to present these quantitative methods. A retrospective feasibility study of 46 patients with supratentorial brain tumors was carried out to demonstrate the practical technique. The continuous variables were correlated with the subjective, qualitative WHO classification of brain tumors, and the prognostic value of the parameters was assessed. Well differentiated astrocytomas (n = 14) had smaller estimates of the volume-weighted mean nuclear volume and mean nuclear profile area, than those of anaplastic astrocytomas (n = 13)(2p = 3.1 · 10−3 and 2p = 4.8 · 10−3, respectively). No differences were seen between the latter type of tumor and glioblastomas (n = 19). The nuclear index was of the same magnitude in all three tumor types, whereas the mitotic index was significantly increased in glioblastomas (2p = 0.01). Three-dimensional, shape-independent estimates of macroscopical tumor volume were not different in anaplastic astrocytomas and glioblastomas (2p = 0.39). Histological type of tumor and mitotic index were of significant prognostic value (2p = 8.2 · 10−6 and 2p ≈ 0.05, respectively). Age above the median and short duration of symptoms were significantly associated with short survival (2p = 0.01). Further investigations of larger series of patients are needed to define the clinical usefulness of these objective, reproducible, and quantitative techniques in the prognostic evaluation of primary brain tumors.

    Notes: Summary Two cases of fibrous dysplasia of the skull are reported. Both patients were young women with acromegaly and were treated with radiotherapy. Progressive pareses of cranial nerves, pain, and a malignant course of the disease were characteristic in both patients, and the diagnosis of osteogenous sarcoma proved in one of them by histological examination. The clinical picture of fibrous dysplasia of the skull and the role of radiotherapy with the risk of development of malignancy is discussed.
