Zusammenfassung Wegen einer schweren Schluckstbrung and röntgenologischen Veränderungen, die für eine Achalasie mit grote§qum Megaoesophagus sprachen, wurde bei einem 43jährigen Patienten auswdrts transabdominal eine Hellersche Myotomie und Fundoplikatio durchgeführt. Gleiche Beschwerden and massive Blutungen führten zur transthoracalen Re-Operation, bei der ein gestieltes Riesenneurofibrom gefunden wurde. Der vom Pharynx ausgehende Stiel wurde getrennt vom Hals aus versorgt. Der äußerst seltene Befund ware bei der Erstoperation nicht übersehen worden, wenn das heute übliche Standardverfahren der transthoracalen Myotomie angewendet worden ware.

Summary A 43-year-old man underwent transabdominal esophagomyotomy and fundoplication in another hospital for severe dysphagia and radiological signs of achalasia with grotesque megaesophagus. Persistence of the symptoms and massive gastrointestinal bleeding led to further examination. Thoracotomy revealed a huge pedunculated tumor completely filling the wide esophageal lumen. The collar pedicle in the pharynx had to be resected by a separate approach. A neurofibroma of this size is extremely rare, and it would not have been overlooked at the time of the first operation had the procedure standard in achalasia, i. e. transthoracic myotomy, been followed.
