    Notes: Abstract LetF be a formally real function field (i.e., −1 is not a sum of squares inF) of transcendence degreen over ℝ. LetA be a finitely generated ℝ-subalgebra ofF whose field of quotients isF. The real holomorphy ringH(F|A) ofF overA is the intersection of all valuation rings ofF which containA and have formally real residue field. The ringH(F|A) has been extensively studied and applied by Becker, Schülting and others. It is known to be a Prüfer domain of Krull dimension not exceedingn and from this it can be shown that every finitely generated ideal ofH(F|A) can be generated byn+1 elements. Here, assuming thatA is regular (i.e., every localization with respect to a prime ideal is regular local), we give necessary and sufficient topological conditions in order for every finitely generated ideal ofH(F|A) to admitn generators. We also provide a geometric description ofH(F|A).

    Notes: A dynamical generalization of the instantaneous normal mode (INM) theory of liquid state dynamics is presented. Due to anharmonicities the eigenvalues and eigenvectors of the Hessian matrix change with time. Therefore, regular INM theory gives a description of molecular dynamics valid only for short times. Starting out from the classical equations of motion the velocity correlation function is expressed in terms of a series of propagation matrices. These are calculated by diagonalizing the Hessian matrix at configurations equidistant in time along a short piece of trajectory. Correlation functions calculated by this normal mode propagation (NMP) for a representative selection of atomic systems agree quantitatively with results from molecular-dynamics simulation. © 1998 American Institute of Physics.

    Notes: Aim : This study aimed to investigate the effectiveness of a one-week triple therapy with esomeprazole, clarithromycin and metronidazole for eradication of Helicobacter pylori infection in the absence of antimicrobial resistance.Methods : Patients testing positive for H. pylori susceptible to metronidazole and clarithromycin (E-test) were randomized to receive a one-week regimen with either esomeprazole 2 × 20 mg or omeprazole 2 × 20 mg in combination with clarithromycin 2 × 250 mg and metronidazole 2 × 400 mg. Follow-up endoscopy with histology and culture and/or rapid urease test was performed 4–8 weeks after the end of treatment.Results : Eighty patients were randomized. Helicobacter pylori infection was cured in 38/39 patients of the esomeprazole group and 31/33 patients of the omeprazole group (per protocol 97.4% (95% confidence interval [CI], 86.2–99.9), 93.7% (95% CI, 79.2–99.2), P = 0.59); intention-to-treat 90.4% (95% CI: 77.4–97.3), 81.6% (95% CI: 65.7–92.3), respectively. No major side effects occurred. Minor side effects occurred in eight (20%) and six (23%) patients during esomeprazole and omeprazole therapy, respectively. Post-treatment susceptibility testing revealed resistance to both metronidazole and clarithromycin in two of the three patients who failed.Conclusion : We conclude that esomeprazole, clarithromycin and metronidazole as one-week triple therapy is effective for eradication of H. pylori in the absence of antimicrobial resistance.

    Description / Table of Contents: This retrospective study shows the results in 30 patients of revision surgery on their reruptured ACL. The follow-up time was 30 months on average (range 10 – 54 months). The follow-up examination included subjective (Lysholm score, Tegner activity level rating score), clinical and radiological criteria and also instrumental laxity testing with the KT-1000 arthrometer. The overall evaluation was done according to the IKDC score. The follow up showed subjective and objective results almost equal to those after primary surgery on ACL. The IKDC score revealed certain abnormal and highly abnormal results, which demonstrated that revision ACL surgery gives poorer results than primary surgery. Nevertheless, we recommend reconstruction of a torn ACL even in a revision case to restore the knee function and prevent further menisceal and chondral damage and degeneration.
    Notes: 30 Patienten, deren bereits einmal rekonstruiertes vorderes Kreuzband (VKB) traumatisch rerupturiert war, wurden bei uns erneut operiert und in einem Zeitraum von durchschnittlich 30 (10 – 54) Monaten nach operativer Versorgung dieser Reruptur nachuntersucht. Die Nachuntersuchung umfaßte neben subjektiven Kriterien (Lysholm-Score, Activity-Score nach Tegner, Bewertung seitens der Patienten) die klinische, instrumentelle (KT-1000-Arthrometer) und radiologische Beurteilung der operierten Kniegelenke, sowie die Evaluation im IKDC-Score. Die Ergebnisse entsprachen sowohl nach subjektiven als auch nach objektiven Maßstäben in einigen Bereichen den Resultaten nach nur einmaligen VKB-Rekonstruktionen und rechtfertigen somit unter Berücksichtigung gewisser Kriterien zur Indikation die erneute operative Versorgung nach traumatischer Reruptur des VKB. Die Einstufung der Resultate im IKDC-Score läßt erkennen, daß die Sekundärrekonstruktion eines bereits operativ versorgten VKB nur sehr selten eine vollständige Wiederherstellung der Funktion und Belastbarkeit des Kniegelenks erzielen kann.

    Notes: Abstract The object of this paper is to study continuous vector bundles, over real algebraic varieties, admitting an algebraic structure. For large classes of real varieties, we obtain explicit information concerning the Grothendieck group of algebraic vector bundles. We show that in many cases this group is small compared to the corresponding group of continuous vector bundles. These results are used elsewhere to study the geometry of real algebraic varieties.
