    Notes: Summary The pharmacology, toxicology and kinetics of a new diuretic indapamide, have been studied in six normal volunteers following a single oral dose of 40 mg. Pronounced diuresis was found, commencing three hours after ingestion, with a peak urinary flow at four to six hours, and continuing for a total of thirty-six hours. A fall in systolic standing blood pressure occurred twenty four hours after ingestion, coincident with the period of maximum dehydration. Free water clearance rose, accompanied by increased urinary losses of Na+, K+ and Cl− and alkalinisation of the urine comparable to the actions of benzothiadiazines. Total urinary losses of Ca2+, Mg2+ and PO 4 3− rose in spite of a fall in urinary concentrations of these ions. The Ca2+ effect compares with the acute ionic effects of other diuretics. No renal, hepatic or haematological toxic effect was demonstrated. The blood sugar level was not disturbed. Serum uric acid rose to abnormal levels although the change did not reach statistical significance. — A thin layer chromatographic method, with a sensitivity limit of 0.1 µg/ml., has been developed for the assay of indapamide in urine. The urinary excretion rates of the volunteers measured over forty-eight hours indicate that the drug is rapidly absorbed with a peak excretion, 2.9±1.3 µg/min occurring three hours after ingestion. The drug is eliminated bi-phasically with an initial short rapid elimination followed by a slower exponential decline with a mean elimination half-life of 10.3 ± 3.9 h. The mean urinary excretion of unchanged indapamide over forty-eight hours was 4.4±1.4% of the administered dose. — It is concluded that indapamide is an effective long-acting diuretic with comparable action to the benzothiadiazine diuretics, but without an effect on blood sugar level in single doses in normal subjects. In contrast with other diuretics, indapamide appears to be extensively metabolised in man, and its longer duration of action to be related to a longer elimination half-life.

    Notes: Abstract New radar images obtained from the Arecibo Observatory (resolution 1.5–4.0 km) for portions of the southern hemisphere of Venus show that: the upland of Phoebe Regio contains the southern extension of Devana Chasma, a rift zone extending 4200 km south from Theia Mons and interpreted as a zone of extension; Alpha Regio, the only large region of tessera within the imaged area, is similar to tessera mapped elsewhere on the planet and covers a smaller percentage of the surface than that observed in the northern high latitudes; the upland made of Ushas, Innini and Hathor Montes consists of three distinct volcanic constructs; Themis Regio is mapped as an ovoid chain of radar-bright arcuate single and double ring structures, edifices and bright lineaments. This area is interpreted as a region of mantle upwelling and on the basis of apparent split and separated features, a zone of localized faulting and extension. Linear zones of deformation in Lavinia Planitia are characterized by lineament belts that are often locally elevated, are similar to ridge belts mapped in the northern high latitudes and are interpreted to be characterized mainly by compression; radar-bright lava complexes within Lavinia Planitia are unique to this part of the planet and are interpreted to represent areas of eruption of high volumes of extremely fluid lava; the upland of Lada Terra is bound to the north by a linear deformation zone interpreted as extensional, is characterized by large ovoids and coronae, is interpreted to be associated with an area of mantle upwelling, and is in contrast to the northern high latitude highland of Ishtar Terra.     Notes: Summary The distribution and excretion of radioactivity from [14C]-fotemustine was examined in mice with melanomas at different stages of development to determine whether the disease state substantially alters the disposition of the drug and its metabolites. Normal BDF1 mice and mice that had been subcutaneously grafted with B16 melanoma either 1, 3, 7, 14 or 21 days previously were used. The animals were killed at either 5 min, or at 3, 24 or 96 h after receiving an intravenous dose of [14C]-fotemustine (20 mg/kg) and were examined either by whole-body autoradiography or by liquid scintillation counting of excreta and tissues of interest. The majority of the [14C]-fotemustine dose was excreted in the urine, with similar amounts being measured in both non-tumourous animals (61.6%±13.1%) and tumourous mice grafted 14 days previously (67.2%±5.7%). Small amounts of radioactivity, again similar in both non-tumourous and tumourous mice, were recovered in the faeces (5.4%±5.6% and 3.6%±1.8%, respectively) and as carbon dioxide (7%±3.5% and 6.4%±1%, respectively), with minimal amounts being expired as chloroethanol (〈1%). When mice were examined 5 min after dosing, there was extensive tissue distribution accounting for 75%±10% of the dose. The highest concentrations determined by both whole-body autoradiography and liquid scintillation counting were measured in the excretory organs, with 33 and 28 μgEq/g being found in the liver and kidney, respectively. High levels were also seen in the lung and plasma (19.8 and 19.5 μgEq/g, respectively). Analysis of variance indicated that groups of tissues, such as the excretory organs, blood and plasma or the pigmented tissues, showed distinct but inconsistent patterns. Only tumours at 14 and 21 days of development were suitable for examination, and these showed levels of 12.1 μgEq/g; however, the tumourto-plasma ratio increased from between approx. 0.5 and 0.6 at 5 min to approx. 2 at 96 h after dosing, suggesting retention within the melanoma, whereas the ratio for the femur remained at approx. 1. Whole-body autoradiography showed that the distribution in the tumour was not uniform, but rather was concentrated in the peripheral area (presumably viable cells) as opposed to the central necrotic region. Thus, the high and sustained concentration of radioactivity found in the active cells of the melanoma may provide an explanation for the high efficacy of the drug.

    Notes: Abstract Deterred by the complexity of the mathematics, pharmacologists and clinical pharmacologists have only recently appreciated the usefulness of pharmacokinetics in drug development. Now unfortunately, although the vernacular of the science is known, often the meaning behind the words is lost. It is often assumed that drug levels are linearly related to drug action. Frequently they are not. This review shows, with reference to psychotropic drugs, how, in simple terms, it is possible to relate pharmacokinetics with pharmacodynamics, and how such relationships may provide a greater insight into drug activity and enhance drug development. Assuming that an equilibrium exists between the drug in plasma levels, and at the site of action, the same Michaelis-Menten equations used to relate effect to drug receptor binding can be used for drug level-dynamic interactions. A number of these relationships have been published and are discussed in terms of their derivation and their limitations. The graphical and computerised methods to create completeE max curves are described and how the parameters of maximal effect, potency, variability and slope can be measured. When the drug is not in equilibrium with its site of action, hysteresis occurs and drug levels are out of phase with activity. Anticlockwise hysteresis, that is, activity increasing with time for a given drug level, can be caused by uptake into an active site, active metabolites, cascade activity, and sensitisation whilst clockwise hysteresis, in which activity decreases with time, can be caused by tolerance, active antagonistic metabolites, learning effects and feedback regulation. Attempts to relate simultaneously kinetics and dynamics by Link models can be difficult and not always necessary. It is assumed in therapeutic drug monitoring that individuals will show the same response for a given drug level. On the contrary, differences in individual subject sensitivity to drugs measured by kinetic-dynamic relationships may provide a greater understanding of the disease itself.

    Notes: Abstract Recent high resolution, high incidence angle Arecibo radar images of southern Ishtar Terra and flanking plains of Guinevere and Sedna on Venus reveal details of topographic features resolved by Pioneer Venus. The high incidence angles of Arecibo images favor the detection of surface roughness-related features, and complement recently obtained low incidence angle Venera 15/16 images in which changes in surface topographic slope are well portrayed. Four provinces have been defined on the basis of radar characteristics in Arecibo images and topography. Volcanism and tectonism are the dominant processes in the mapped area, which has an average age of about 0.5–1.0 billion years (Ivanov et al., 1986). These processes vary in relative significance in the mapped provinces and it is likely that geologic activity has occurred simultaneously in all four provinces. On the basis of stratigraphic evidence, however, a general sequence is proposed which represents the major activity in each area. The low predominantly volcanic plains of Guinevere and Sedna Planitiae are the relatively oldest terrain. A major region of complex tectonic deformation, the Southern Ishtar Transition Zone, postdates much of the low plains and delineates the steep-sloped flanks of Ishtar Terra. Lakshmi Planum is characterized by a distinctive volcanic style (large low edifices, calderas, flanking plains) and at least in part postdates the Southern Ishtar Transition Zone. Relatively recent plains-style volcanism occurs locally in Sedna Planitia and embays the Southern Ishtar Transition Zone. Compressional deformation appears to dominate the mountains of the Ishtar plateau, but the nature of the tectonic deformation in the Southern Ishtar Transition Zone is very complex and likely represents a combination of extension, compression and strikeslip deformation. Arecibo data reveal additional coronae in the lowlands, suggesting that corona formation is an even more widespread process than indicated by the Venera data.
