    Notes: Summary In order to rule out age-related differences in the sinus node (SN) function, the effects of different substances were tested in SN isolated preparations from adult (4–5-month-old) and old (24–25-month-old) rats. No difference was seen in the in vivo heart rate between adult and old rats, whereas sinus node rate (SNR) was significantly lower in the old rather than in the adult preparations. The effects of acetylcholine (10−9–10−5 M) were similar between the two groups of preparations. The response to adenosine (10−9–10−4 M) was significantly higher in the adult than in the old preparations (p〈0.05). No significant difference was noted in the response to isoproterenol (10−10–10−6 M). High calcium (5.4–8.1 mM) caused an increase of the SNR that was significantly greater in the old than in the adult preparations (p〈0.005). In conclusion, our results show an age-related decrease in the isolated SNR that does not seem to be mediated by different responses to neuromediators but is probably due to an alteration of the intrinsic activity of the SN cells.

    Notes: Summary Pharmacokinetics and pharmacodynamics of verapamil were studied in 11 elderly subjects (age=79.67±4.74 years) and in 11 middle-aged subjects (age=45±11.37 years) following intravenous (IV), single oral, and long-term oral administration. Plasma verapamil concentrations were determined using high-pressure liquid chromatography (HPLC). Twenty-four hour dynamic Holter electrocardiographic (ECG) recordings were employed to study heart rate (HR) and P-R interval. No difference in plasma half-life, distribution volume, body clearance, and area under the curve (AUC) was observed between the two groups after IV and oral verapamil administration. Blood pressure (BP) and HR were significantly reduced after verapamil IV administration in the elderly group only (p〈0.05, p〈0.01, respectively). After single and long-term oral administration, variable HR and BP responses were observed in both groups. The P-R prolongation following both IV and single oral doses exhibited a delay with respect to the peak plasma concentration, inducing a definite hysteresis loop. The slope of P-R variations (using a linear pharmacodynamic model) was greater in the elderly both after IV and single oral verapamil administration, but statistical significance was obtained only after the single oral dose (p〈0.05). In the elderly group, after long-term oral administration, there was a significant prolongation of the P-R interval (p〈0.0001) with respect to the corresponding time point of the 24-hour predrug period. Such variations in pharmacodynamic parameters in the elderly did not, however, cause any clinical problem. In conclusion, verapamil seems to be well tolerated in the elderly as well as in younger patients at similar dosages. However, its use in the elderly requires careful clinical evaluation.

    Notes: Abstract A retrospective study of the autopsy rate of the clinico-pathological correlations was made in a group of inpatients. The trend of autopsy rate was observed in all impatients died in a university hospital during a nine year period (from 1975 to 1983). The agreement between the clinical and pathological death diagnosis was retrospectively controlled in 294 consecutive patients died and submitted to autopsy in the same hospital during a 6 month period (from January to June 1983). The results show that: 1) In the oldest (〉 60 yrs) patients, the autopsy rate trend was significantly reduced from 1975 to 1983, whereas the hospital admissions and the mortality rate increased. 2) In the youngest subjects (〈 60 yrs), the autopsy rate trend, the hospital admissions and the mortality rate did not significantly change from 1975 to 1983. 3) In all the years considered, the autopsy rate was significantly reduced in the oldest class (p 〉 0.01). 4) The agreement between the clinical and pathological diagnosis was observed in 83% of cases in the 〈 60 years class and in 63°% of cases in the 〉 60 yrs class (p 〉 0.001). 5) The association of main disease with other diseases was significantly more frequent in the 〉 60 yrs class with respect to the 〈 60 yrs class (p 〈 0.01). It has been concluded that the autopsy, expecially in the elderly, is absolutely necessary for a better quality control of the clinical diagnosis and of the medical care.
