    Description / Table of Contents: Zusammenfassung 81 von 86 Patienten, die nach der Methode von Brunner in den Jahren 9955 bis 1966 an einer Trichterbrust operiert worden sind, konnten nachuntersucht werden. Alter, Konstitution, morphologische Begrenzung des Trichters, Thoraxkonfiguration einschließlich Wirbelsäule und begleitende Fehlbildungen bestimmen zu einem Teil den Operationserfolg und müssen bereits bei der Indikationsstellung oder Beratung des Patienten Berücksichtigung finden, zumal eine vitale Indikation fast nie besteht. Für die Operationsindikation sind psychologische und kosmetische Gesichtspunkte vielfach ausschlaggebend. Im Kindesalter liegt in der Operation allenfalls eine Prophylaxe angesichts möglicher kardiospiratorischer Insuffizienzerscheinungen nach Abschluß des Wachstumsalters. In jedem Fall verlangt die Zuordnung subjektiver Beschwerden zu klinischen Daten Kritik. Urteilt man nach dem kosmetischen Gesamtergebnis, so kann zur Dokumentation und Objektivierung in erster Linie nur das seitliche Röntgenbild herangezogen werden. Das Elektrokardiogramm und die Ergospirometrie ergeben in der Regel keine pathognomonischen Befunde. Demnach besteht auch keine verläβliche Korrelation zwischen den gemessenen physikalischen Größen und Ausmaß oder Vollständigkeit der operativen Korrektur der Thoraxdeformität. In der Gruppe der Jugendlichen und Erwachsenen war das kosmetische Spätresultat in über 80% ideal oder gut. Kinder stellten prozentual das Hauptkontingent unzureichender Langzeitergebnisse. Insgesamt 27 % unserer Patienten zeigten im Gegensatz zum unmittelbaren Operationserfolg ein schlechtes Dauerergebnis. Bei Jugendlichen und Erwachsenen mit einer Trichterbrust als isolierte Fehlbildung bei ansonsten relativ normaler Thoraxkonfiguration sind nach der Brunnerschen Methode fast ausnahmslos gute Korrekturergebnisse zu erwarten. Zurückhaltung ist bei kombinierter Flach- und Trichterbrust geboten. Diese Konstitutionsanomalie bringt für den Operateur meistens ein enttäuschendes Ergebnis. Im Hinblick auf das Frühresultat und dem oft unbefriedigenden Dauererfolg bei Kindern und Asthenikern besonders mit Flach- und Trichterbrust ist zu überlegen, ob die so überzeugende Methode von Brunner in diesen Fällen nicht durch eine temporäre innere Schienung (Interimsprothese) ergänzt werden sollte. Einschränkend ist zu sagen, daß von allen Methoden mit innerer Schienung vergleichbare Nachuntersuchungen zur Beurteilung der Spätresultate noch nicht vorliegen.
    Notes: Summary Pectus excavatum is a congenital thoracic deformity which in severe cases may result in disturbances of cardiorespiratory physiology. The radiographs of the chest in this condition indicate cardiac displacement (70%). The electrocardiographic findings are the result of cardiac rotation and thoracic deformity and do not reveal any evidence of intraventricular conduction defect or myocardial damage. Generally on cardiac catherzations in funnel chest individuals we have not seen abnormal values. Only one Patient had an abnormal end-diastolic pressure in the right ventricle. The marked restrictions of the intrathoracic space in the cardiac region had no profound influence on venous return and did not impair alveolar gas exchange. There are no complaints if the deformation is but slight. In some cases symptoms arising in affected individuals were a direct outcome of the physical inactivity of mental anxiety resulting from unwarrated invalidism. In the view of the fact that the deformity is congenital, and unpredictably progressive operation should be undertaken. Funnel chest can be corrected satisfactorily by different operation with good physiologic and cosmetic results. The best cosmetic results by the method of Brunner are obtained when the operation is done after the age of 14 years. Because of the dissatisfaction with Brunner's technic in children the operative produce should not done so early.

    Notes: Summary The organization of the nucleus of the optic tract was investigated with light and electron microscopy in combination with Golgi impregnation. In Golgi material, neurons ranged in size from 10 to 25 μm with three to seven principal dendrites extending predominantly parallel to the fibres of the optic tract, irrespective of their location within the nucleus. In some areas dendrites extended into the neuropil of the adjacent dorsal terminal nucleus of the accessory optic system and the posterior pretectal nucleus. Occasionally spines and appendages were observed. The fine structure of the nuclei, perikarya and the dendritic arborization did not allow a well-defined distinction between interneurons and projection neurons. The synaptic organization of the nucleus of the optic tract showed great resemblance to the neuropil of the lateral geniculate nucleus and the superior colliculus. Similar types of presynaptic terminals were noticed: (i) R-terminals were either large and scalloped or small and regular in outline with spherical vesicles and electron-lucent mitochondria, and showed asymmetric contact zones; (ii) F-terminals with flattened vesicles, opaque mitochondria and symmetric contact zones; (iii) RLD-terminals with spherical vesicles and electron-dense mitochondria and asymmetric contact zones; (iv) P-terminals with pleomorphic vesicles and electron-lucent or opaque mitochondria and asymmetric synaptic thickenings. These different types of terminal were found isolated in the neuropil or in clusters of synapses. The most striking differences between the nucleus of the optic tract and the lateral geniculate nucleus were the relative scarcity of F-terminals in the clusters, the paucity of triadic arrangements and the relatively small size of the R-terminals. The differences in ultrastructure may be related to retinal W-type ganglion cells, which form the main retinal input to the nucleus of the optic tract and could also be related to the physiologically identified direction-selective units within the nucleus of the optic tract.
