    Notes: Abstract. Paediatric Crohn's disease can be a serious and complex condition which is not always easy to diagnose if it presents in an atypical manner. Although the pathological processes are the same in adults and children, the clinical presentation and disease distribution can be different in children. The impact of the disease can also be much more significant in a growing and developing child than in an adult. Numerous investigations are available to establish the diagnosis, define the complications and to demonstrate the extra-intestinal manifestations. Close co-operation is required between the clinician and radiologist when investigating a child suspected of having Crohn's disease or its symptoms to minimise the radiation dose and the psychological and physical trauma. We review the clinical and radiological aspects of Crohn's disease and discuss the imaging modalities available in diagnosing Crohn's disease and its complications and suggest an investigation pathway as used in our institution.

    Notes: Abstract The aim of the study was to examine the relationship between anastomotic leaks and the development of symptomatic anastomotic strictures after primary repair of oesophageal atresia (OA) with or without tracheo-oesophageal fistula (TOF) and the prognostic value of the anastomotic appearance on postoperative oesophagography. The records and X-ray films of 49 patients with OA with or without TOF admitted between January 1990 and June 1995 were reviewed retrospectively. Of these, 37 had a primary repair (34 immediate and 3 delayed) and a postoperative contrast swallow was done between day 5 and day 30 (median day 7). Radiological leaks were documented. In addition, the maximum transverse diameters of the upper pouch and anastomotic region and the length of the anastomotic narrow segment were measured; the ratio of upper-pouch diameter to that of the anastomosis was calculated (anastomotic index, AI). The need for dilatation of symptomatic anastomotic strictures and for repeat dilatation as a separate episode were documented. Of the 37 patients, 4 developed an anastomotic leak (11%), 3 radiological and 1 clinical. A symptomatic stricture occurred in 55% of patients. There was no correlation between a leak and later development of a symptomatic stricture, although the small number of patients with leaks precluded definite conclusions. No radiological leak progressed to a clinical leak. There was no correlation between the AI or the length of the anastomotic narrow segment and the need for dilatation, the need for repeat dilatation, or the number of dilatations. In this study, the presence of a leak, the AI, and the length of the narrow segment on early postoperative contrast swallow did not correlate with the later development of a symptomatic stricture. A radiological leak was of no clinical significance. Since major leaks are apparent clinically, an early routine contrast study did not influence management and is not necessary.

    Notes: Abstract We first introduced endoscopy-guided balloon dilatation (EGBD) as a new method of nonoperative treatment for infantile hypertrophic pyloric stenosis (IHPS) in 1988. Up to now, we have treated 12 patients with this technique. The system used included an Olympus PQ 20 GIF endoscope, a balloon catheter with built-in-guide-wire, a pressure gauge, and a home-made inflationary device. The procedure was performed with the patients under general anaesthesia by an experienced paediatric endoscopist. Visualisation of the narrow pylorus was facilitated by a 180° clockwise rotation of the endoscope in the antrum. A 15-mm diameter balloon catheter was introduced after lubrication with silicone spray and inflated to 45 psi for 5–10 min. Of 12 patients, 11 had EGBD as primary treatment; of these, 4 responded completely and surgery was avoided, but 7 had an incomplete response requiring surgery. The remaining patient had EGBD as secondary treatment for an incomplete Ramstedt pyloromyotomy and responded satisfactorily. The longest follow-up after successful balloon dilatation was 22 months. Complete disappearance of the pyloric tumour was demonstrated by ultrasound examination 3 months after the procedure. Our experience confirms that balloon dilatation of IHPS is feasible, but at present its results are not consistent enough for recommendation of general use.

    Notes: Abstract Fourteen hydrocephalic children with functioning lateral ventricular shunts developed posterior fossa problems: posterior fossa cyst (7 patients); isolated fourth ventricle (6 patients), and concomitant dilatation of third and fourth ventricles (1 patient). Posterior fossa symptoms were present in only 2 children; 10 had nonspecific symptoms and 2 were asymptomatic. Computed tomography was the most useful investigation for diagnosis and management. Decompression of progressing posterior fossa lesions was attempted in 5 patients: cystoperitoneal shunt (3), marsupialization (1), and aspiration (1). All 5 patients had initial symptomatic relief but symptoms recurred subsequently; 2 eventually died of severe ventriculitis. The remaining 9 did not require posterior fossa surgery.

    Notes: Abstract The radiological features of three cases of pulmonary actinomycosis in children are presented. Two of the cases had the classical roentgen features of chronic consolidation with overlying rib periostitis. The third had non specific consolidation but demonstrated extension through the diaphragm. Two of our cases were mentally subnormal children in institutional care. Their pulmonary pathology is almost certainly attributal to poor dental hygiene, a constant problem in this group of children. Hence, although pulmonary actinomycosis is rare in the general populus, we suggest it might be suspected more often in the subnormal group when chronic consolidation is present.

    Notes: Abstract Chest wall mesenchymoma is a rare benign tumour arising from the ribs and occuring in infants, being usually present from birth. The few previously documented cases have been of solitary tumours. A 6-week-old infant presented with two separate lesions demonstrating specific radiological signs, and on isotope scanning with Technetium MDP a high uptake of the agent was demonstrated.
