    Notes: Abstract As an alternative to computationally expensive N-body simulations for gravitional clustering, the Zel'dovich approximation (ZA) was studied in 3D, 2D and 1D. Plots of the density contrast were compared against linear theory and the exact solution. The ZA was found to perform very well in the linear regime, better than linear theory, and to give a good approximation well into the non-linear regime.

    Description / Table of Contents: Résumé Le reflux gastro-oesophagien est un trouble fréquent qui relève du traitment chirurgical dans de nombreux cas. Jusqu'à une date relativement récente, les complications du reflux relevait de la seule chirurgie. Au cours de ces 5 dernières années, des traitements endoscopiques ont été rapportés par plusieurs équipes médicales. Ces techniques semblent ouvrir une voie nouvelle dans le traitement du reflux, même si elles ne sont que des méthodes encore bien souvent expérimentales. Puisque les mécanismes anti-reflux physiologiques concernent essentiellement les muscles du cardia et du sphincter inférieur de l'oesophage, il n'est pas surprenant que les traitements endoscopiques se soient portés sur ces deux régions. Les premiers essais endoscopiques de contrôle du reflux ont concerné le sphincter inférieur de l'oesophage. Des essais plus récents ont porté sur les muscles de l'estomac proximal, consistant en une scarification au niveau de l'extrémité inférieure de l'oesophage afin de réduire la quantité ou la fréquence du reflux. Aucune de ces approches thérapeutiques n'a été jusqu'à présent rapportée chez l'homme. Puisque toutes ces approches thérapeutiques endoscopiques ne sont pas sans danger, nous recommandons d'adopter un protocole rigoureux en cas d'application chez l'homme. Un exemple de protocole est décrit dans cet article. Les auteurs proposent de fournir les méthodes d'analyse pour toute série de patient bénéficiant d'un traitement endoscopique et les résultats.
    Abstract: Resumen La enfermedad por reflujo gastrointestinal es una entidad común que en algunos casos requiere tratamiento quirúrgico. Hasta hace poco el tratamiento quirúrgico era la única alternativa para las complícaciones graves del reflujo. En el curso de los últimos aňos varios centros han informado el tratamiento endoscópico para el reflujo. Estos novedosos aproches anuncian el comienzo de una nueva era en la terapia de las complicaciones del reflujo; sin embargo, en la actualidad son puramente experimentales. Puesto que los mecanismo fisiológicos antirreflujo involucran a los músculos del cardías proximal así como al esfínter esofágico inferior, no es de sorprenderse que los tratamientos endoscópicos hayan sido orientados hacia estas dos áreas. Sin embargo, los primeros intentos de control endoscópico de reflujo fueron dirigidos al esfínter esofágico inferior. Los esfuerzos más recientes han estado dirigidos al estómago proximal, fundamentados orientados por el concepto de que la escarificación por debajo del extremo inferior del esófago (esclerosis por inyección endoscópica para crear una zona de fibrosis) reduce el volumen y la frecuencia del refujo. Ninguno de estos últimos enfoques ha sido informado en humanos. Teniendo en cuenta que todas las modalidades endoscópicas son potencialmente peligrosas, nosotros recomenadmos la adopción de un protocolo riguroso para su aplicación humana. Un posible protocolo para este propósito es decrito en el presente artículo; los autores proveerán análisis de datos de cualquier cohorte de pacientes sometidos a terapia endoscópica y los resultados de todos los tratamientos endoscópicos a investigadores serios. La posibilidad de métodos endoscópicos para el tratamiento de reflujo gastroesofágico representa una novedosa idea que merece valoración completa y cautelosa.
    Notes: Abstract Gastro-esophageal reflux disease is a common disorder which sometimes requires surgical treatment. Until recently, surgical treatment was the only alternative for severe complications of reflux. During the past 5 years endoscopic treatments for reflux have been reported from several medical centers. These new approaches may herald a new era for the treatment of reflux complications. However, at this time they are purely experimental approaches. Since physiologic antireflux mechanisms involve the muscles of the proximal gastric cardia as well as those of the lower esophageal sphincter, it is not surprising that endoscopic treatments have been directed at these two areas. However, the first endoscopic attempts at reflux control were directed at the lower esophagus sphincter. The most recent efforts at endoscopic control have been aimed at the proximal stomach, based on the concept that scarification beneath the lower end of the esophagus reduces the amount or frequency of reflux. Neither of these latter approaches has been reported in human patients. Since all endoscopic approaches are potentially dangerous, we recommend the adoption of a rigorous protocol for all human application. A possible protocol for this purpose is described herein; the authors will provide data analysis for any cohort of patients receiving endoscopic therapy, and will provide the results of all endoscopic treatments to any serious investigators.

    Notes: Summary Gastroesophageal reflux disease remains a disorder of unknown etiology associated with abnormal function of the lower esophageal sphincter (LES) and other physiological co-factors of the pathologic reflux. Effective operations for reflux are designed to reinforce the anti-reflux barrier and alter the tendency towards abnormal reflux. We have postulated that the most important component of these procedures is the prevention of distraction of the lowermost components of the LES at the onset of a potential reflux episode. Distraction of the LES causes shortening of the effective sphincter mechanism and can initiate experimental reflux events. In this study we used endoscopic sclerosis of the submucosal space at the cardia as a means of reducing distraction of the cardia in the hope that this would reduce abnormal reflux events. Canine gastroesophageal reflux was induced by intravenous atropine and monitored by continuous esophageal pH monitoring. Sclerosis of the cardia prevented gastroesophageal reflux, without measurable effect on the LES pressure or length. Endoscopic sclerosis of the cardia may be a useful technique in the control of human gastroesophageal reflux.
