    Notes: Summary Five patients with biopsy-proven craniofacial fibrous dysplasia underwent MRI with T1- and T2-weighted sequences and a gadolinium-enhanced T1-weighted spin-echo sequence. Low to intermediate signal intensity was usually seen in the largest part of the lesion on both spin-echo sequences, but smaller regions of hyperintensity on T1- and T2-weighted images and intermediate signal intensity throughout a lesion on T1-weighted images were also seen. All lesions enhanced but only two became iso- or hyperintense compared to fat. High clinical and pathological activity in three cases correlated with high signal intensity on both spin-echo sequences and with strong enhancement in two of the three. The presence of large veins or sinusoids on pathological examination did not correlate with the enhancement pattern.

    Notes: Summary Two patients with ruptured intracranial dermoids, examined with both CT and MRI are reported. Clinical presentation was transient cerebral ischemia in one patient and acute meningeal signs in the other. CT scan showed typical fat density of the tumor and the subarachnoid space. On MRI both the tumor and the subarachnoid fat, were strongly hyperintense on T1-weighted images.

    Notes: Abstract The sensitivity of different MRI sequences in the detection of inner ear malformations in patients presenting with sensorineural hearing loss (SNHL) and/or vertigo was evaluated. We studied 650 patients presenting with SNHL and/or vertigo, clinically not suspected of having inner ear malformations. The sensitivity of T1-weighted, Gd-enhanced T1-weighted and (when available) T2-weighted spin-echo images, and three-dimensional Fourier transformation-constructive interference in steady state (3DFT-CISS) gradient-echo images, to unexpected malformations was assessed. Inner ear malformations were found in 15 (2.3%) of these patients. Enlargement of the endolymphatic duct and sac was the most frequent malformation, found in 11 patients. The 3DFT-CISS images showed all lesions; the other sequences were less sensitive and the pathology was missed, partially or only retrospectively seen in 11 of the 15 patients. Therefore, in addition to the routine unenhanced and Gd-enhanced T1-weighted and T2-weighted images, thin gradient-echo (3DFT-CISS) images are necessary to detect all clinically unexpected inner ear malformations in patients presenting with vertigo and/or SNHL.

    Description / Table of Contents: Zusammenfassung Neuere CT- und MR-Untersuchungsmethoden erlauben eine detaillierte anatomische Darstellung des Innenohres. Die CT ist unverändert die beste Bildgebungstechnik zur Darstellung von Frakturen, angeborenen Mißbildungen und Otodystrophien im Innenohr. In den letzten Jahren hat sich die MRT im Nachweis pathologischer Veränderungen von innerem Gehörgang, membranösem und knöchernem Labyrinth und zur Charakterisierung von Schläfenbeinspitzenläsionen als diagnostisch äußerst wertvoll erwiesen. Bei Patienten mit Frakturen oder kongenitalen Mißbildungen erlaubt die MRT den Nachweis von labyrinthären Kontusionen bzw. Aplasie des vestibulocochlearen Nervs. Die Diagnose einer Labyrinthitis sowie von intralabyrinthären Tumoren wurde erstmals bildgebend mittels MRT möglich. Es muß aber betont werden, daß MRT und CT meist komplementäre Methoden sind. Dies trifft insbesondere bei Patienten mit gemischtem Hörverlust, kongenitalen Mißbildungen und Schläfenbeinapexläsionen zu.
    Notes: Summary New computed tomography (CT) and magnetic resonance (MR) techniques allow more detailed anatomic studies of the inner ear. CT is still the best technique to study patients with fractures, congenital malformations and otodystrophies involving the inner ear. During recent years MR imaging has emerged as an excellent method to detect pathology in the internal auditory canal, membranous labyrinth and bony labyrinth and to characterize petrous apex lesions. MR has even proved its value in patients with fractures and congenital malformations making the diagnosis of, for instance, labyrinthine concussion and absence of the vestibulocochlear nerve possible. The diagnosis of acute/chronic labyrinthitis and intralabyrinthine tumors has also became possible. However, MR and CT are often complementary, as is the case in patients with mixed hearing loss, congenital malformations and petrous apex lesions.

    Description / Table of Contents: Zusammenfassung Cefadroxil, ein neues, halbsynthetisches Breitspektrum-cephalosporin wurde 144 Patienten mit akuten (64,6%) und chronischen (35,4%) Harnwegsinfektionen verabreicht. Eine hervorragende Korrelation bestand zwischen klinischem Ansprechen (geheilt+gebessert=82,6%) und der Elimination der verantwortlichen Erreger (88,9%). Das Medikament wurde sehr gut vertragen, auch nach Langzeitanwendung (bis zu 23 Tagen): Nebenwirkungen traten nur bei sechs Fällen auf (4,2%), zwei davon waren Exantheme (1,4%). In einem der beteiligten Laboratorien wurde festgestellt, daß 22 (35%) der vor der Behandlung isolierten Stämme gegen Ampicillin oder seine Derivate resistent waren: alle diese 22 Stämme zeigten sich gegenüber Cefadroxil sensibel.
    Notes: Summary Cefadroxil, a new semi-synthetic broad-spectrum cephalosporin, was administered to 144 patients with acute (64.6%) and chronic (35.4%) urinary tract infections. An excellent correlation was found between clinical response (cured+improved=82.6%) and bacteriological eradication of the causative organism (88.9%). The drug was very well tolerated, even after prolonged administration (up to 23 days): side-effects only appeared in 6 cases (=4.2%), two of which were rashes (=1.4%). In one of the participating laboratories, 22 (=35%) of the pre-treatment isolated strains were found to be resistant to ampicillin or derivatives: all of these 22 strains proved to be sensitive towards cefadroxil.

    Notes: Abstract We report four patients with internuclear ophthalmoplegia and torsional nystagmus, ipsiversive to the side of the medial longitudinal fasciculus (MLF) lesion. The MLF lesion could be responsible for an inactivation of the ispilateral interstitial nucleus of Cajal inducing a contraversive slow phase. The presence of a corrective ipsiversive quick phase implicates an intact rostral interstitial nucleus of the MLF.

    Notes: Abstract Although gadolinium-enhanced magnetic resonance imaging (Gd-MRI) has been used to indicate the presence of a subclinical labyrinthitis in patients with idiopathic sudden sensorineural hearing loss (ISSHL), its sensitivity in daily clinical practice is unknown. We describe Gd-MRI findings in 27 ISSHL patients taking part in a prospective multicenter clinical trial. MRI findings were related to the severity of the hearing loss, vestibular involvement and the time interval between the occurrence of ISSHL and imaging. Pathological enhancement of the cochlea indicating a labyrinthitis was found in one patient with ISSHL. In 26 cases, no pathological enhancement could be established. Study results indicate that the present sensitivity of Gd-MRI is low for detecting subclinical viral labyrinthitis in patients with ISSHL. To improve the sensitivity of MRI in ISSHL, we recommend that imaging is performed as early as possible, preferably before treatment is started.
