    Notes: Evidence for direct learning curve for technique on change of cervical sampling device The recommended sampling device within the NHS Cervical Screening Programme is the Aylesbury spatula. A local decision was taken to decrease brush usage (either alone or in combination) by 50% from an initial level of 41.8%, with an initial inadequate smear rate of 9.93%. This was managed by controlling smear taking equipment through provision of smear taking kits. The monthly inadequate rate unexpectedly rose to 17.8% before dropping back to previous levels. Brush usage overall fell to 35.2%, with a corresponding increase in spatula use. The possible reasons for this are discussed. The facts suggest that these changes were directly linked, and that there is a learning curve with change of sampling device and that a rise in the inadequate rate should be expected under these circumstances.

    Notes: Abstract L-dopa, the major treatment for Parkinson's disease (PD), depletes S-adenosyl-L-methionine (SAM). Since SAM causes PD-like symptoms in rodents, the decreased efficacy of chronic L-dopa administered to PD patients may result from a rebound increase in SAM via methionine adenosyl transferase (MAT), which produces SAM from methionine and ATP. This was tested by administering intraperitoneally saline, or L-dopa to mice and assaying for brain MAT activity. As compared to controls, L-dopa (100 mg/kg) treatments of 1 and 2 times per day for 4 days did not significantly increase MAT activity. However, treatments of 1 and 2 times per day for 4 and 8 days did significantly increase the activity of MAT by 21.38% and 28.37%, respectively. These results show that short interval, chronic L-dopa treatments significantly increases MAT activity, which increases the production of SAM. SAM may physiologically antagonize the effects of L-dopa and biochemically decrease the concentrations of L-dopa and dopamine. Thus, an increase in MAT may be related to the decreased efficacy of chronic L-dopa therapy in PD.

    Notes: Abstract Polychlorobiphenyl (PCB, Aroclor® 1242) loadings of 2.12, 21.5 and 212Μg.d−1 were added to three model planktonic ecosystems. Each loading consisted of a clay and water mixture in which 92% of the PCB was adsorbed to the clay particles. After addition, partitioning between soluble and particulate pools in the water and sedimentation of PCB was examined. Particulate PCB concentrations were positively related to changes in particulate organic carbon (POC) indicating a transfer of PCB from clay to the algal community. The percent of the total PCB concentration measured as soluble was inversely related to POC. The proportion of PCB added to each column that was sedimented, not recovered or remained in the water column was similar at all loading rates. Approximately one-half of the PCB added was not accounted for in sedimentation or the water and was assumed lost by volatilization. High algal biomass and sedimentation of organic matter reduced this loss.

    Notes: Abstract The reasons for urinary incontinence can in most cases be determined by careful history taking, physical examination, laboratory, X-ray and endoscopic examination. To establish the diagnosis in a minority of patients with incontinence additional investigations are required. If the incontinence be the result of a deranged nervous control, abnormal bladder muscle behaviour or a sphincteric disorder, pressure-flow measurements may be necessary. Urethral pressure measurements are of value in elucidating the cause of some forms of incontinence, which are described in this paper.
