    Notes: This study examined the effectiveness of health promotion counseling for overweight adolescent nursing students. The Chinese version of an established health promotion counseling booklet developed by the author of this study was used in the counseling. The study employed a one-group pre- and post-quasi-experimental research design from September 1997 to June 1999. A total of 166 (17%) first-year, junior college nursing students were found to be overweight according to the results of a health examination at the school health center at a junior college in the Taipei Metropolitan Area. Of these 166 students, 58 were randomly selected and 49 of these had complete participation records for the 2-year study. All subjects received 8 hours of whole group and 12 hours of small group health promotion counseling over a 1-year period. The results show that health promotion counseling enabled subjects to adopt healthier lifestyles and most of the physiological variables (WLI, HDL, SBP, TC) were significantly improved. This research outcome supports the value of health promotion counseling in school health centers, which incorporates material to help overweight adolescent students live a healthier life.

    Notes: This study examined the effectiveness of health promotion counseling of the family caregivers from Taipei Metropolitan Area. An established Chinese version of the health-promoting scale was developed by the author of this study (which was modified and authorized by Walker, Sechrist, & Pender, 1987). A two-group quasi-experimental research design was employed. From October 1996 to July 1997, 84 family caregivers were selected and assigned to one of the two groups. Members of the treatment group received health promotion counseling from four trained professional home care nurses during home visiting. The members of the control group were given no counseling but instead received traditional care. Pre-assessment was carried out after subjects were selected and post evaluation was done 6 months later. The findings showed that the counseling enabled members of the treatment group to adopt healthier lifestyles. The research outcome supports the value of home-care nurse counseling of family caregivers.

    Notes: Skin wound healing requires temporally and spatially coordinated motility of multiple skin cell types. The limited success of current skin wound care is due to lack of understanding about how the healing processes begin, progress, and complete. One of the most dramatic environmental changes in an acute wound is the transition from plasma to serum. The cells in the wound come into contact with serum, instead of a filtrate of plasma, for the first time. We report here that this transition differentially regulates the migration of various skin cell types. Plasma promotes dermal fibroblast and endothelia cell, but not keratinocyte and melanocyte, motility. In contrast, serum selectively promotes keratinocyte motility and inhibits fibroblast and endothelial cell motility. The key regulators are the increased transforming growth factor-beta (TGF-beta) in serum and the distinct TGF-beta receptor (I, II and III) profiles in different cell types. TGF-beta selectively inhibits fibroblast and endothelial cell, but not keratinocyte and melanocyte, migration. Neutralization of TGF-beta function in serum eliminates the differential effects between serum and plasma. Manipulations of the TGF-beta receptor profiles in the cells, especially TGF-beta receptor II and III, convert TGF-beta-insensitive cell types to sensitive cell types and vice versa, and their responses to plasma and serum. This is the first study that reveals a molecular mechanism for coordinating multiple skin cell-type motility in wounds.

    Notes: Abstract  Objective : To explore health-related behaviors among adolescent mothers living in the rural area of Taoyuan, Taiwan. Design: A cross-sectional descriptive design and nonrandom survey method were used. Sample: The sample consisted of 37 adolescent mothers, identified by public health nurses. Measurements: Standardized interview and Adolescent Health Promotion (AHP) questionnaire. Results: Findings revealed a pattern of economic disadvantage. Nearly half of the participants still lived with their biological parents. Two-thirds needed economic support from their parents (generally coming from their biological mother). Thirty-five percent of participants reported never using contraceptives, two-thirds had never had a Pap smear, and 44% did not breast-feed their infants. Nearly 60% of the children were cared for by the biological mothers of the participants. Adolescent mothers with high school education, and who were employed, married, and received parental economic support had better health-related behaviors than adolescent mothers without these characteristics. Conclusions: Although the study sample has geographic limitations, future international studies with similar populations of adolescent mothers in rural settings will help public health nurses understand adolescent mothers’ stressors and needs which in turn affect their health-related behaviors. Intervention strategies are needed to encourage behaviors to keep this population healthy.

    Notes: Abstract  The purposes of this study were to (a) develop a teaching course on health promotion for nursing students in Taiwan, (b) evaluate the effects of this teaching course, and (c) understand students’ appraisals of its effectiveness in helping them to change unhealthy behavior. A sample of 65 randomly selected female nursing students took an 18-week course developed by the investigators, which included 30 h of classroom lectures and 4 weeks of written reports by students chronicling the changes in their behavior. Health promotion questionnaires administered before and after the course and content analysis of the students’ reports were used to evaluate the effects of the course. Students’ questionnaire scores after course completion indicated significantly increased intent to adopt healthy lifestyles. Content analysis of students’ reports on their personal behavior-changing experiences showed that they accepted the potential value of curriculum aspects such as “experiencing the struggle, suffering, and even abandonment of the process,”“experiencing the benefits of change,”“increasing self-confidence,” and “empathizing with how difficult it is for clients to change behavior.” These results support the value of teaching courses on health promotion to nursing students. The authors recommend including such a course as part of a regular nursing education.

    Notes: Abstract  The purpose of this study was to assess the psychometric properties of a newly devised instrument, the Adolescent Health Promotion scale (AHP), a 40-item Likert-type self-report instrument used to detect unhealthy lifestyles in adolescents. Content validity was considered to be supported based on the findings of previous studies and the observations of a panel of 14 content experts. This study examined the construct validity and reliability of the instrument. The psychometric properties of the AHP, including item analysis, factor analysis, and reliability measures, were assessed based on the responses of 1,128 Taiwanese adolescents. Kaiser-Meyer-Olkin (KMO) measures and Bartlett's sphericity test showed that the samples met the criteria for factor analysis. Factor analysis yielded a six-factor instrument that explained 51.14% of the variance in the 40 items. The six factors were social support, life appreciation, health responsibility, nutritional behaviors, exercise behaviors, and stress management. The Cronbach alpha reliability coefficient for the total scale was 0.932, and alpha coefficients for the subscales ranged from 0.75 to 0.88. The results of this study indicate that the AHP has good construct validity and reliability in Taiwanese and that its use by school health nurses to assess adolescent health promotion programs is warranted.

    Notes: The physiological importance of connexin-26 (Cx26) gap junctions in regulating auditory function is indicated by the finding that autosomal recessive DFNB1 deafness is associated with mutations of the Cx26 gene. To investigate the pathogenic role of Cx26 mutation in recessive hearing loss, four putative DFNB1 Cx26 mutants (V84L, V95M, R127H, and R143W) were stably expressed in N2A cells, a communication-deficient cell line. In N2A cells expressing (R127H) Cx26 gap junctions, macroscopic junctional conductance and ability of transferring neurobiotin between transfected cells were greatly reduced. Despite the formation of defective junctional channels, immunoreactivity of (R127H) Cx26 was mainly localized in the cell membrane and prominent in the region of cell–cell contact. Mutant (V84L), (V95M), or (R143W) Cx26 protein formed gap junctions with a junctional conductance similar to that of wild-type Cx26 junctional channels. (V84L), (V95M), or (R143W) Cx26 gap junctions also permitted neurobiotin transfer between pairs of transfected N2A cells. The present study suggests that (R127H) mutation associated with hereditary sensorineural deafness results in the formation of defective Cx26 gap junctions, which may lead to the malfunction of cochlear gap junctions and hearing loss. Further studies are required to determine the exact mechanism by which mutant (V84L), (V95M), and (R143W) Cx26 proteins, which are capable of forming functional homotypic junctional channels in N2A cells, cause the cochlear dysfunction and sensorineural deafness.

    Notes: Abstract : Caspase-3 enzyme activity is induced, and cell death follows, when cerebellar granule neurons (CGNs) from 8-day-old rats are transferred from an extracellular concentration of 25 mM K+ (25 mM [K+]e) to 5 mM [K+]e. Death of these neurons is diminished by an inhibitor of caspase-3 but not by an inhibitor of caspase-1. Actinomycin D and cycloheximide inhibit induction of caspase-3 and prevent death. Experiments in which CGN intracellular Ca2+ concentration ([Ca2+]i) was manipulated by either changing [K+]e or adding a voltage-gated Ca2+ channel antagonist or a Ca2+ ionophore to the medium showed that caspase-3 mRNA rises 2.5-fold when [Ca2+]i is diminished from 300 to 150 nM, with a corresponding rise in peak caspase enzyme activity. Whereas the caspase-3 mRNA level does not rise further with a still greater diminution in [Ca2+]i, peak caspase enzyme activity continues to increase, reaching sevenfold induction when [Ca2+]i is reduced to 55 nM. In CGNs in which [Ca2+]i is set at 55 nM by incubation in 5 mM [K+]e, treatment with forskolin or dibutyryl 3′,5′-cyclic adenosine-5′-monophosphate delays caspase-3 induction and diminishes death but does not alter [Ca2+]i. We conclude that, in immature CGNs, both caspase-3 transcription and the subsequent processing of caspase-3 are induced by a fall in [Ca2+]i. Elevating cyclic AMP content delays caspase-3 induction by a mechanism that does not require an increase in [Ca2+]i.
