    Notes: Aims to describe current issues in health and safety training in the UK, focusing on the training requirements associated with general workplace risks. Rasmussen's skill-, rule-, and knowledge-based errors are briefly described and training's role in promoting safe behaviour and in reducing human error are discussed. The weaknesses of using accident statistics as the sole measure of health and safety performance are highlighted. Reference is made to the legislative requirements, especially harmonised European health and safety law. The meaning of "competence" in the context of health and safety law is given. Contemporary views of training's role as a risk control measure and topical issues relating to training for hazardous occupations are described. The article concludes by arguing that risk assessment is set to become an integral part of the trainer's planning process.

    Notes: Examines the subject of safety training and attempts to demonstrate that it has particular challenges, especially in relation to the provision of realistic training. A review of the research literature in both industrial training and occupational health and safety reveals a wealth of material on the subject. However, the authors expected to find - and did not - material concerning the subject of "training safely" and of "risk assessment for training". Proceeds to show that these are issues that are of practical concern both to practitioners and to safety regulators. A small study involving 34 safety inspectors seems to confirm this. It is argued that "training safely" is an area where the research lags behind practical developments. Concludes by proposing a programme of future research to meet this need.

    Notes: Describes how, in August 1989, the Engineering Industry TrainingBoard (EITB) were commissioned by the Training Agency to evaluate thequality of open learning materials supplied to the industry. Inparallel, the EITB entered into a collaborative research agreement withAston University. A range of 67 products, in eight subject areas andfrom 39 suppliers, were evaluated. The majority (48) were eithertext-based (32) or video-led (16). The remainder (19) were audio,computer-based training (CBT) and interactive video (IV) packages.Details three main findings emerging from the research. First, thatprice alone is not a predictor of quality. Second, good quality productsexist across the entire price range including some at less than£50. Third, text-based materials form the majority of the goodquality products (24 out of 34), followed by video-led (5) andinteractive video (4). Results from the small sample of audio products(5) suggest that this may not be an effective medium for open learning.

    Notes: Cell proliferation, matrix accumulation and cell infiltration are characteristic features of progressive glomerulosclerosis and tubulointerstitial fibrosis. Platelet-derived growth factor (PDGF), a cytokine which has proliferative, prosclerotic and chemokine properties, has been shown to be upregulated in the rat remnant kidney model. Inhibition of the renin–angiotensin system by angiotensin-converting enzyme (ACE) inhibitors has a beneficial effect on renal function and morphology, but the effect of ACE inhibition on PDGF gene expression and PDGF-mediated cellular proliferation in subtotal nephrectomy has not been studied in detail. Twelve rats were subtotally nephrectomized (STNx) and received either the ACE inhibitor perindopril or a placebo for 12 weeks. Five sham-operated rats served as controls. Subtotal nephrectomy was associated with hypertension, proteinuria, elevated plasma creatinine and increased kidney weight. After 12 weeks, PDGF B-chain mRNA was significantly upregulated in the glomeruli and tubulointerstitium of subtotally nephrectomized rats. ACE inhibition attenuated PDGF mRNA expression in association with a reduction in tubular and glomerular proliferation, as assessed by staining for proliferating cell nuclear antigen. In the context of the known in vitro and in vivo effects of PDGF, it is postulated that the renoprotective action of ACE inhibitors may be partially related to PDGF-mediated antiproliferative mechanisms.

    Notes: Summary: The accumulation of extracellular matrix (ECM) within the glomerulus is the ultrastructural hallmark of diabetic nephropathy and is directly linked to the declining renal function. the pathogenesis of these ECM changes in diabetes is not well understood but is likely to involve interaction between cells, growth factors, structural proteins and cell receptors for these molecules. Recent advances in cellular and molecular biology have highlighted the complex nature of the molecular interactions which may lead to glomerulosclerosis in diabetes.

Renal microvascular disease as assessed by urinary albumin excretion and glomerular ultra-structure was evaluated in a model in which streptozotocin diabetes was induced in spontaneously hypertensive rats (SHR).3. Diabetes was associated with increases in urinary albumin excretion, and hypertension resulted in a further increase in albuminuria.4. Various antihypertensive regimens were administered to diabetic SHR, with the angiotensin-converting enzyme inhibitor perindopril and triple therapy (hydralazine, reserpine and hydrochlorothiazide) being more effective than the calcium antagonist (lacidipine) in retarding the increase in albuminuria in diabetic SHR.5. Antihypertensive therapy appears to ameliorate the development of diabetic renal disease.

    Notes: Background:  The β- d-endoglycosidase, heparanase, is emerging as an important contributor to the pathogenesis of proteinuria. The purpose of the present study therefore was to examine the role of heparanase in a model of accelerated anti-glomerular basement disease (anti-GBM).Methods:  Accelerated anti-GBM disease was induced and animals sacrificed at day 10 to establish heparanase expression using immunohistochemistry and western blot analysis. In addition, cortex was isolated from normal and diseased glomeruli to determine if mRNA levels altered with disease. A previously validated anti-heparanase antibody associated with proteinuria reduction, in a model of membranous nephropathy, was administered prior to disease induction to establish its impact on protein excretion in this model.Results:  At day 10 of anti-GBM disease, an increase in glomerular heparanase was shown using immunohistochemistry. Sequential staining studies revealed that this increase was associated with glomerular endothelial, epithelial cells and invading ED-1-positive inflammatory cells. RT-PCR revealed an insignificant 1.2-fold induction of mRNA at day 10 of disease. Western blot analysis of kidney cortex confirmed that the active 58-kDa heparanase species was restricted to diseased kidney at day 10. The inactive 65-kDa precursor, however, was found only in cortex derived from normal kidney. Proteinuria at day 10 of disease was significantly reduced, in the absence of altered rat anti-sheep antibody titres, after administration of a validated polyclonal anti-heparanase antibody (P 〈 0.05). Furthermore, sheep IgG deposition was not altered by administration of the anti-heparanase antibody.Conclusion:  These data suggest that heparanase contributes to the pathogenesis of proteinuria in a model of anti-GBM disease.

The equilibrium dissociation constant of ligand-angiotensin-converting enzyme interaction was unchanged.5. Increased vascular angiotensin-converting enzyme concentration may contribute to vascular hypertrophy and diabetic vasculopathy by increased local synthesis of angiotensin II.
