    Notes: Objective Fetomaternal mismatch for human platelet antigen (HPA)-1a accounts for approximately 85% of cases of neonatal alloimmune thrombocytopenia. The purpose of the study was to determine the prevalence of the HPA-1a negative platelet phenotype in a cohort of pregnant women in Ireland, the rate of alloimmunisation to HPA-1a in HPA-1 mismatched pregnancies and the associated incidence of neonatal alloimmune thrombocytopenia.Design A prospective case–control study.Setting The antenatal clinics of a large maternity teaching hospital.Population or sample Pregnant women, regardless of parity, presenting at the antenatal clinics.Methods An enzyme-linked immunosorbent assay (ELISA) designed for simultaneous HPA-1a typing and antibody detection was used. Further analysis for HPA-1a alloantibodies was performed using commercial ELISA's (GTI PakPlus and Pak1) and the monoclonal antibody immobilisation of platelet antigens assay. Confirmation of serological typing was by the polymerase chain reaction technique using sequence-specific primers (PCR-SSP).Main outcome measures The presence of the HPA-1a negative phenotype and its association with the development of maternal anti-HPA-1a and infant thrombocytopenia.Results Eighty-four of 4090 consecutive women enrolled in the study tested positive for HPA-1a in the screening ELISA. Confirmatory genotyping was performed on 67 women (representing 80% of the cohort), and 54 women (representing 3272 non-selected pregnancies), were homozygous for the HPA-1b allele (1.7%). Three of 34 (9%) women who delivered HPA-1a positive babies had detectable anti-HPA-1a and all three babies had neonatal alloimmune thrombocytopenia, for an overall incidence of 1:1100 non-selected pregnancies.Conclusions The observed prevalence of 1.7% for the HPA-1a negative platelet phenotype is as expected from studies in other countries. While we have demonstrated the practicability of antenatal HPA-1a screening, further research is warranted to investigate maternal parameters predictive of severe fetal thrombocytopenia in HPA-1a alloimmunised pregnancies.

    Notes: Few works of economic thought have such a close association with the intellectual and economic history of their period as Adam Smith's An inquiry into the nature and causes of the wealth of nations. Three recent careful assessments of the ‘influence’ of Smith's work, however, have found little direct evidence for its positive significance for economic policy in the 1780s and 1790s. Parliamentary debate seldom referred to the Wealth of nations, and then usually negatively unless by Smith's friends, or ‘radicals and Foxite whigs’. John Locke, David Hume, Charles Davenant, Sir Josiah Child, William Petty, Josiah Tucker and Arthur Young were all cited more frequently. A wide range of parliamentary leaders read Smith carefully, and several ministers knew him well and sought his advice, but with the exception of Shelburne they appear to have applied Smith's thought eclectically. Salim Rashid has noted that in 1776 there were already influential economic authorities, notably Arthur Young and Josiah Tucker, who advocated freer markets. Conversely, for over a decade after the publication of the Wealth of nations, articles on economic matters in the major periodical reviews made scanty reference to Smith's work, while the protectionist views of Sir James Steuart, whom Smith had ignored, were often authoritative. Smith's views became respectable among the political after negotiation of the Anglo-French commercial treaty of 1786, but it was this liberal economic policy which gave the wealth of nations currency, not the reverse.

D. Howells in 1881. It is a striking coincidence in American literary history that four major writers of the late nineteenth century – Howells, Henry James, S. L. Clemens, and Henry Adams – all failed to serve in the Civil War. Whether or not they believed that they had turned their backs on their country, each made some excuse in his published work. The most notorious is Henry James's statement in Notes of a Son and Brother that ‘ a horrid even if an obscure hurt ’ had prevented him from volunteering to fight.

    Notes: Motivation: Standard laboratory classification of the plasma cell dyscrasia monoclonal gammopathy of undetermined significance (MGUS) and the overt plasma cell neoplasm multiple myeloma (MM) is quite accurate, yet, for the most part, biologically uninformative. Most, if not all, cancers are caused by inherited or acquired genetic mutations that manifest themselves in altered gene expression patterns in the clonally related cancer cells. Microarray technology allows for qualitative and quantitative measurements of the expression levels of thousands of genes simultaneously, and it has now been used both to classify cancers that are morphologically indistinguishable and to predict response to therapy. It is anticipated that this information can also be used to develop molecular diagnostic models and to provide insight into mechanisms of disease progression, e.g., transition from healthy to benign hyperplasia or conversion of a benign hyperplasia to overt malignancy. However, standard data analysis techniques are not trivial to employ on these large data sets. Methodology designed to handle large data sets (or modified to do so) is needed to access the vital information contained in the genetic samples, which in turn can be used to develop more robust and accurate methods of clinical diagnostics and prognostics.Results: Here we report on the application of a panel of statistical and data mining methodologies to classify groups of samples based on expression of 12,000 genes derived from a high density oligonucleotide microarray analysis of highly purified plasma cells from newly diagnosed MM, MGUS, and normal healthy donors. The three groups of samples are each tested against each other. The methods are found to be similar in their ability to predict group membership; all do quite well at predicting MM vs. normal and MGUS vs. normal. However, no method appears to be able to distinguish explicitly the genetic mechanisms between MM and MGUS. We believe this might be due to the lack of genetic differences between these two conditions, and may not be due to the failure of the models. We report the prediction errors for each of the models and each of the methods. Additionally, we report ROC curves for the results on group prediction.
