    Notes: The CD5 membrane molecule, initially identified as an exclusive T-cell marker, also defines a phenotypically and functionally distinct B-lymphocyte population. In normal individuals, CD5+ B cells are committed to secrete‘natural’polyreactive (auto)antibodies, that is antibodies, mainly IgM, endowed with multiple antigen-binding capabilities, including rheumatoid factor (RF) activity. At variance with this, in rheumatoid arthritis (RA) as well as in other autoimmune conditions, monoreactive autoantibodies binding with high affinity and specificity to a given self antigen have been isolated and the cells from which they originate differently related to the CD5+ B-lymphocyte subset.Here, we studied the proportions of CD5+ B cells and the characteristics, in terms of polyreactivity and monoreactivity, of RF produced by B lymphocytes in RA patients with classified disease activity. Our results suggest that patients with a more severe disease activity have higher proportions of CD5+ B cells and higher frequencies of B lymphocytes committed to secrete RF, with the characteristics of polyreactive antibodies. On the other hand, we did not find a significant difference between the proportions of peripheral B cells producing monoreactive RF in patients with high- versus patients with low-activity RA. However, in two highly active RA patients, we found that synovial fluid, compared with peripheral blood, was significantly enriched for (IgG and IgA) monoreactive RF-producing B cells.

    Notes: Summary The clinical efficacy and adverse effects of nandrolone decanoate (Deca-Durabolin) were investigated in a double-blind placebo-controlled multicentre study in female patients with systemic lupus erythematosus (SLE). The patients received 100 mg Deca-Durabolin or placebo once every 2 weeks for 3 months. At the end of the study, 36 patients (18 on placebo and 18 on Deca-Durabolin) were evaluated. Beneficial effects were observed on arthralgia, and/or arthritis and minor vasculitis. Possible beneficial effects were seen on polymyalgia, active SLE rash, oral and/or nasal ulcerations and pleural and pericardial pain in the Deca-Durabolin-treated patients. The overall clinical assessment score showed that Deca-Durabolin would influence the disease course by lowering significantly the disease activity score. However, this treatment is still hampered by adverse reactions such as voice alterations and hirsutism. Therefore, the development of androgen-derived steroids free from androgenic activity should be encouraged, since these compounds may contribute to a new therapeutic modality in female SLE patients.

    Description / Table of Contents: Summary The altered cortisol and adrenal androgen (i. e., dehydroepiandrosterone sulfate = DHEAS) secretion, observed during testing in rheumatoid arthritis (RA) patients not treated with corticosteroids, should be clearly regarded as a “relative adrenal insufficiency” in the setting of a sustained inflammatory process, as shown by high serum IL-6 levels. Androgens seem implicated in the pathophysiology of autoimmune disorders, including RA, as natural immunosuppressors. Low plasma synovial fluid testosterone concentrations are observed in male RA patients; low plasma DHEAS levels are mainly observed in female RA patients. The menopausal peak of RA suggests that estrogens and/or progesterone deficiency also play a role in the disease, and many data indicate that estrogens suppress cellular immunity, but stimulate humoral immunity (i. e., deficiency promotes cellular Th1-type immunity). Gene polymorphisms for enzymes involved in the steroidogenesis seem to further complicate the role of sex hormones in the susceptibility to autoimmunity. Acquired changes of sex steroid metabolism seem to also play a role in the peripheral sex hormone levels. In conclusion, a complex interaction between the hypothalamus-pituitary-adrenocortical and gonadal axis functions is evident in RA.
    Notes: Zusammenfassung Untersuchungen von bis dahin mit Glukokortikoiden unbehandelten Patienten mit rheumatoider Arthritis (RA) zeigen Abweichungen der Spiegel für Kortisol und adrenalen Androgenen (vor allem Dehydroepiandrosteronsulfat, DHEAS), die in der Relation zu gleichzeitig hohen Serumspiegeln von IL-6 eindeutig als relative Nebenniereninsuffizienz zu bezeichnen sind. Androgene dürften in der Pathogenese von Autoimmunerkrankungen, einschließlich der RA, als physiologische Immunosuppressoren eine Rolle spielen. Bei männlichen RA-Patienten wurden niedrige Testosteronspiegel sowohl im Plasma als auch in der Synovia gefunden, bei RA-Patientinnen waren die entsprechenden Spiegel für DHEAS erniedrigt. Die Häufund der RA um die Menopause legt weiter eine pathophysiologische Bedeutung sinkender Spiegel von Östrogenen und/oder Progesteron nahe. Zahlreiche Befunde deuten auf suppressive Effekte von Östrogenen auf die zelluläre Immunität, während sie stimulierend auf die humorale Immunität wirken (bzw. ein Östrogendefizit steigert die Th 1-Typ-vermittelte Immunität). Genetische Polymorphismen von Enzymen der Steroidbiosynthese scheinen zusätzlich die Rolle von Sexualsteroiden in der Entwicklung von autoimmunen Reaktionen zu begünstigen. Darüberhinaus beeinflussen erworbene Formen eines veränderten Steroidmetabolismus die peripheren Spiegel der Sexualsteroide. Zusammenfassend ist die pathogenetische Relevanz eines komplexen Zusammenwirkens der Achsen Hypothalamus-Hypophyse-Nebennierenrinde und Hypothalamus-Hypophyse-Gonaden bei RA evident.

    Notes: Summary Adult-onset Still's disease is characterized by high spiking fever, evanescent maculopapular rash and arthritis. Several recent reports have associated its development with a variety of infectious agents. We describe the case of a 25-year old woman presenting high fever, maculopapular rash and seronegative polyarthritis associated with lymphoadenopathy, splenomegaly and neutrophil leucocytosis together with acute acquired toxoplasmosis. Other causes of systemic illness were excluded by appropriate laboratory, radiological and histological investigations. Clinical, radiological and laboratory findings as well as possible etiopathogenetic correlations among both pathological conditions are discussed. Toxoplasma gondii should be considered as a further possible triggering agent associated with the development of adult-onset Still's disease.

    Notes: Summary Plasma from 38 patients suffering from one of the five broad clinical subgroups of Psoriatic Arthritis (PA) were studied for soluble plasma Fibronectin (pFn). The mean total concentration of pFn was 453.03μg/ml ± 142.83 SD, with a significant statistical difference (p〈0.01) versus a healthy control group matched with respect to sex and age. In order to evaluate the biological role that pFn might play in this pathological condition, observed concentrations were correlated with the degree and duration of the psoriasis and arthritis. In addition, pFn was correlated to some biohumoral parameters that are modified during inflammatory processes (ESR, CRP, sCu, sFe, Hb) and to uric acid levels. Tissue typing (HLA) was done where possible. From our observations, we suggest that pFn most likely is not an acute phase protein and rather than having specificity for a particular disease, might, in widespread and severe cases be,a general and useful marker of the connective-tissue organizing and repairing response, following its injury.

    Notes: Summary Two cases of nail involvement associated with primary interphalangeal osteoarthritis of the hand, including leukonychia and longitudinal nail ridge, are reported. Osteoarthritic changes of the distal interphalangeal joints may cause nail lesions by exerting direct pressure on the nail matrix or by interfering with local blood flow. Moreover, inflammation of the Heberden's nodes is often present and seems to participate in the development of nail alteration. In our patients, leukonychia disappeared after local steroidal anti-inflammatory treatment of the osteoarthritic node and longitudinal nail ridge disappeared after treatment with nonsteroidal anti-inflammatory agents.
