    Notes: Abstract One-hundred and seventeen patients with acute thoracolumbar vertebral column fracture or fracture-dislocations were analyzed and classified into stable (36%) and unstable (64%). Eight helpful roentgen signs were observed that may serve to direct attention to serious underlying, often occult, fractures and dislocations. The changes fall into four principal groups: abnormal soft tissues, abnormal vertebral alignment, abnormal joints, and widened vertebral canal. All stable and unstable lesions showed abnormal soft tissues, while 70% demonstrated kyphosis and/or scoliosis, and an abnormal adjacent intervertebral disk space. All unstable lesions showed one or more of the following signs: displaced vertebra, widened interspinous space, abnormal apophyseal joint(s), and widened vertebral canal.

    Notes: Abstract 1. Stress fractures result from muscular activity on bones rather than from direct impact upon them. 2. The fatigue variety of stress fracture occurs in normal bone when abnormal muscular tension or torsion is placed upon it. 3. The insufficiency type of stress fracture results when normal muscular stress is placed upon a bone with deficient elastic resistance. 4. Most stress fractures are of the fatigue type, occurring in an individual who engages in repetitive vigorous activity to which he is unaccustomed. 5. The biomechanics, radiologic features, and differential diagnosis were discussed and the literature was reviewed.

    Notes: Abstract A total of 304 patients with injuries to the femoral shaft and ipsilateral hip presented between 1984 and 1990. Some 253 of them suffered fractures of the femoral shaft and dislocated hips or fractures of the acetabulum, and 51 of these sustained fractures of the femoral shaft and neck or trochanteric region. Of this latter group, 20 patients had a combination of femoral shaft and neck fractures, and 31 had a combination of femoral shaft and trochanteric fractures. All of the trochanteric injuries were demonstrated on the initial radiographs. However, in 11 of the patients with combined femoral shaft and neck fractures, the diagnosis was delayed by as much as 4 weeks. This delay related to the fact that these fractures tended not to separate in the initial evaluation period and that there was external rotation of the proximal femoral fragment due to the femoral shaft fracture. Good preoperative and, in particular, good postoperative radiography of the hip is essential to make the diagnosis. Although orthopedic surgeons have been aware of this combination of injuries since 1953, radiologists have not.

    Notes: Abstract Negative ulnar variance is a condition in which the ulna is relatively shorter than the radius at the carpus. It was found in 21% of 203 normal wrists. We have observed an increased incidence (49%) of this anomaly in patients with carpal ligamentous instabilities (dorsiflexion instability, palmar flexion instability, scapholunate dissociation with rotary luxation of the scaphoid, and lunate and perilunate dislocations). While the reasons for this association have yet to be adequately delineated, the presence of a negative ulnar variant may serve as an impartial clue to the presence of ligamentous instability. Many carpal instabilities present with subtle radiographic findings requiring careful evaluation of radiographs. Patients with negative ulnar variance and histories suggestive of ligamentous instability should undergo careful radiologic evaluation to assure early diagnosis of carpal disruption.

    Notes: Abstract The plastic rivets attached to the commonly used Nec-Loc cervical immobilization collar produce linear lucencies which often traverse the cervical vertebrae. Linear lucencies in this location often simulate a fracture. In most instances, the complete outline of the object causing the artifact may be identified. However, identification may not be possible in all cases. Radiologists should familiarize themselves with the appearance of the artifact produced by this particular immobilization device so as to avoid the erroneous diagnosis of a cervical fracture. Whenever the diagnosis is in doubt, the collar should be removed and a repeat radiograph should be obtained.
