    Notes: Abstract Two steroid 21-hydroxylase genes are normally present within the human major histocompatibility complex near the genes encoding the fourth component of complement (C4A and C4B). Steroid 21-hydroxylase is encoded by the CYP21 gene, while the highly homologous CYP21P gene is a pseudogene. We studied steroid 21-hydroxylase and complement C4 haplotypes in 33 Dutch patients (29 families) suffering form classical congenital adrenal hyperplasia (CAH) and in their 80 family members, and also in 55 unrelated healthy controls, using 21-hydroxylase and complement C4 cDNA probes. Eleven different haplotypes, defined in terms of gene deletions, gene duplications, conversions of CYP21 to CYP21P, and “long” and “short” C4 genes, were found. In 23% of the patients' haplotypes, the CYP21 gene was deleted; in 12%, it was converted into a CYP21P pseudogene. In the remaining 65%, the defect was apparently caused by a mutation not detectable by this method. The most common haplotype (with one CYP21 and one CYP21P gene) was significantly more often observed in patients with simple virilizing CAH than in those with salt-losing CAH. Comparison of the 21-hydroxylase haplotypes found in CAH patients from several countries shows evidence for considerable genetic variation between the groups studied.

    Notes: The hypothesis studied is that increased responsiveness in asthma is not limited to the airways. Forty asthmatic children were analysed for their bronchial responsiveness (BR) to exercise. Twenty patients revealed bronchial obstruction after exercise while the remainder did not. These observations were compared with the responsiveness of leucocytes, which was determined by their histamine ‘releasability’. Twenty healthy children served as controls. Release of histamine induced by calcium ionophore-aided calcium influx was significantly higher in both groups of asthmatics than in the healthy children (P 〈 0.005). Similar findings were obtained by induction of microtubule aggregation due to deuterium oxide (D2O). The S-shaped dose-response relationship with D2O was shifted to the left in the patients with BR to exercise compared to patients without (P 〈 0.025). The slope was increased in both patient groups compared with the healthy children (P 〈 0.01). It is concluded that the mean ‘releasability’ of histamine release due to both stimulants correlated well (P 〈 0.01). This suggests that the ‘releasability’ is determined by the responsiveness of the microtubules. This may also apply to allergen-induced histamine release, as was revealed from studies with anti-IgE. The differences in histamine release found in relation to BR due to exercise were also present if the patients were divided according to BR due to histamine. A significant relationship existed between the degree of BR to histamine and the responsiveness of the microtubules (P 〈 0.02).

    Notes: Background Low density eosinophils are more prominent in asthmatic patients compared with healthy subjects, LDH are metabolically more active and produce more tissue-injuring and spasmogenic proteins than normal cosinophils. Objective and methid With a method providing information about eosinophils of 12 different densities we were able to study eosinophil density characierislics in 24 young patients in detail with allergic asthma in a stable phase, and in 21 patients after a bronchial allergen challenge. Results Study of the eosinophil density profile of patients and healthy controls revealed two density populations. Patients had more low density eosinophils than controls. In the patients eosinophil density characteristics and in particular the number of low density eosinophils correlated strongly with both FEV1% predicted (ρ=−0.66, P 〈 0.001) and REV1/FVC (ρ=−0.47, P 〈 0.01) as well as with bronchial responsiveness to histamine (ρ=−0.68, P 〈 0.001) and house dust mite (ρ=−0.37, P 〈 0.05). Allergen induced bronchial reactions were associated with an increase in the number (P 〈 0.001) and percentage (P 〈 0.05) of low density eosinophils. A selective rise in the number of eosinophils collected from fractions with a low density accounted for the observed rise in the total number of eosinophils, Density changes did not differ between patients with an isolated early reaction and patients with both an early and a late reaction, nor was there a relation between the severity of the late reaction and the shift in eosinophil density. Conclusion In conclusion, peripheral blood eosinophil density characteristics and in particular numbers of low density eosinophils are closely related with indicators of the asthma severity under stable conditions. Allergen inhalation induces a further shift towards lower density suggesting additional activation of the eosinophils.

    Notes: The hypothesis that increased reactivity in asthma is not always limited to the bronchi but also exists in the mediator releasing system was investigated in forty-five asthmatic children, approximately half of whom had exercise-induced bronchoconstriction (EIB). The bronchial threshold to histamine was measured as an indicator of the reactivity of the bronchi and the histamine release from leucocytes without adding allergen (spontaneous histamine release) was considered as an indicator of the reactivity of the basophil leucocytes. There was a significant correlation between the histamine threshold and spontaneous histamine release and between these and EIB. These findings support the hypothesis.
