    Notes: Abstract The ultrasonic images obtained in a patient with Menetrier's disease are presented. Identification of the normal gastric wall surrounding the whole organ and of giant mucosal folds projecting from the wall into a partially fluid-filled lumen led to the correct diagnosis.

    Notes: Abstract Twenty-four patients with bowel lesions that could be imaged on a sonogram underwent ultrasound-guided fine-needle aspiration biopsy. Indications for biopsy included: poor condition of the patient precluding the use of barium studies and/or endoscopy (9 cases) or hindering technically adequate examinations (3); nonspecific radiographic images (6); inability to obtain an adequate biopsy sample during endoscopy (6). The results of biopsy were correct in the 18 cases proven by surgery or autopsy; in the remaining 6 patients, histologic results were considered conclusive, and were later confirmed by clinical, radiographic, and ultrasound follow-up. Fine-needle aspiration biopsy may be considered a simple, rapid, and accurate diagnostic procedure when an alternative approach to the study of gastrointestinal tract leions is needed.

    Notes: Abstract We reviewed the sonographic findings in four patients with afferent loop obstruction from tumor of the gastric stump. All had undergone partial gastrectomy with Billroth type II gastrojejunostomy. The obstructed afferent loop was visible in all cases as a distended fluid-filled segment with multi-layered wall and few mucosal folds. Its course could be traced from the region of the hepatic hilum to the gastric stump. In two patients, the relationships between the loop and the mesenteric vessels, aorta and inferior vena cava allowed us to recognize it as postoperative duodenum. Thickening of the gastric remnant and obstructive jaundice were diagnosed in three cases. Since sonography is often used as the first imaging method in patients with abdominal complaints, knowledge about postsurgical appearance of the gastrointestinal tract is very important in detecting such pathological conditions.

    Notes: Abstract The clinical charts and contrast mediumenhanced computed tomographic (CT) examinations of 15 adults with a history of serious shock and resuscitation were reviewed. Several hypovolemia-related abnormalities were recognizable and were variably associated with each other. Diminished caliber of the abdominal vessels with intense vascular contrast medium enhancement and renal abnormalities were the most common findings; decreased and/or increased enhancement of the abdominal organs and other hypovolemic changes were also present. Most of the series in the radiologic literature describe findings in pediatric subjects, but the various vascular and extravascular CT findings with regard to shock in adults may be different from those reported in children. Although the diagnosis of shock is based on clinical signs, hypovolemia-related CT abnormalities must be distinguished from organic lesions. CT may also identify a previously unsuspected shock state.

    Notes: Abstract Two patients with arteriovenous fistulas of the native kidney occurring after needle biopsy were evaluated using duplex and color Doppler ultrasonography. The first patient had a fistula with associated pseudoaneurysm: color Doppler showed the lesion as a small rounded area with whirling flow; spectral analysis allowed recognition of both the afferent artery with low impedance flow and the draining vein with pulsatile, arterialized flow. The second patient had a normal color Doppler study; however, spectral analysis demonstrated signals with low vascular impedance from an intra-parenchymal artery at the lower pole, and a jet of turbulent flow. Following disappearance of clinical findings, such Doppler abnormalities were no longer detectable. When a iatrogenic arteriovenous fistula is considered on clinical grounds, both color and Doppler spectral analysis of waveforms from intra-parenchymal vessels should be performed. Possibly, further advances in color Doppler technology will permit the use of this examination as the first imaging procedure in these clinical situations.

    Notes: Abstract The sonographic findings in five patients with congenital anomalies of the interior vena cava and left renal vein are presented. We encountered two patients with duplication and two with transposition of the inferior vena cava, and one with a retroaortic left renal vein, In four cases the findings were confirmed by demonstrating venous flow within the anomalous vascular structures using duplex Doppler equipment. The diagnostic capabilities of sonographic imaging and Doppler analysis of blood flow allow noninvasive recognition of these vascular anomalies. In our opinion, invasive studies are needed only when retroperitoneal structures are difficult to assess by ultrasound because of obesity and/or abundant overlying intestinal gas.

    Notes: Abstract We report the case of a young girl with recurrent sharp pain in the outer aspect of the ankle. She had no previous trauma. High-resolution US (HRUS) showed a complete intermittent dislocation of the peroneal tendons. Post-traumatic chronic peroneal instability is quite common, whereas complete dislocation is rare. HRUS is an important adjunct to clinical examination and radiographic evaluation of patients with musculoskeletal disorders of the foot and ankle [1]. This paper reports its value in non-traumatic recurrent dislocation of the peroneal tendons.

    Notes: Abstract Smooth muscle neoplasms seem to be particularly frequent in children with AIDS, and several cases of such lesions, both benign and malignant (leiomyomas and leiomyosarcomas), have recently been reported in the literature. We describe here the imaging findings in a boy with AIDS who developed two leiomyomas of the gallbladder wall as well as calcification of the intraparenchymal splenic arteries.

    Notes: Abstract  Objective. To evaluate the sonographic (US) appearance of digital annular pulley (DAP) tears in high-level rock climbers. Design and patients. We performed a retrospective analysis of the US examinations of 16 high-level rock climbers with clinical signs of DAP lesions. MRI and surgical evaluation were performed in five and three patients respectively. The normal US and MRI appearances of DAP were evaluated in 40 and three normal fingers respectively. Results. Nine of 16 patients presented a DAP tear. In eight subjects (seven with complete tears involving the fourth finger and one the fifth finger), US diagnosis was based on the indirect sign of volar bowstringing of the flexor tendons. Injured pulleys were not appreciated by US. Tears concerned the A2 and A3 in six patients and the A3 and A4 in two patients. A2 pulley thickening and hypoechogenicity compatible with a partial tear was demonstrated in one patient. MRI and surgical data correlated well with the US findings. Four patients had tenosynovitis of the flexor tendons but no evidence of pulley disruption. US examinations of three patients were normal. In the healthy subjects US demonstrated DAP in 16 of 40 digits. Conclusion. US can diagnose DAP tears and correlates with the MRI and surgical data. Because of its low cost and non-invasiveness we suggest US as the first imaging modality in the evaluation of injuries of the digital pulley.

    Notes: Abstract High resolution ultrasonography of the knee was performed on 82 young patients with clinically suspected Osgood-Schlatter disease and on 30 normal subjects; in 45 pathological cases (55%) comparative X-ray films were taken. The ultrasound pictures were equally or more effective than X-ray images in 45/45 cases; their value was particularly marked for soft tissue study. The typical sonographic changes of the ossification center, of the cartilage, and of the surrounding soft tissues are described and classified, both for Osgood-Schlatter and for Sinding-Larsen-Johansson diseases. These signs are based mainly upon cartilage swelling and edema, fragmentation of the ossification center, thickening of the patellar tendon, and bursitis of the infra-patellar bursa. Ultrasonography is proposed as a simple and reliable method for the diagnosis of knee joint osteochondrosis. The ultrasound picture is also suitable for periodical follow-up the course of the disease.
